SECTION V: CAMPUS ADMINISTRATOR INFORMATION All private career colleges are required to identify a Campus Administrator for each registered location. The Campus Administrator is responsible for the general management and day-to-day operations of the PCC, and ensuring all aspects of the Private Career Colleges Act, 2005 are observed. The Campus Administrator is to be regularly available during business hours to students, the general public and ministry staff. The Campus Administrator is also responsible for all student contracts that the PCC will enter into with students, including the authority to accept or cancel contracts for the PCC. It is a mandatory requirement to ensure that Campus Administrator and head office contact information is current with the Superintendent of Private Career Colleges at all times. Private career colleges are required to submit a sample student contract that will be used when enrolling students into a vocational program (see Sample Student Contract available on the ministry website at http://www.edu.gov.on.ca/eng/welcome.html). If you plan to also offer non-vocational programs that do not require the approval of the Superintendent of Private Career Colleges, you must provide a sample of the contract that will be used to enrol students in these types of programs. Section 21 of Regulation 415/06 sets out the terms that cannot be included in a non-vocational contract. SECTION VII: STUDENT COMPLAINT PROCEDURE/EXPULSION POLICY As prescribed in Section 3(1)10 and 3(1) 15 of Regulation 415/06, every applicant must submit a copy of the college s expulsion policy and a copy of the college s student complaint procedure. Section 36 of Regulation 415/06 sets out the elements that are required to be included in each student complaint procedure. Please attach a copy of your institution s proposed expulsion policy and student complaint procedure to this application. SECTION VIII: THIRD-PARTY AGREEMENT WITH RESPECT TO TRANSCRIPT INFORMATION Section 34 and Section 35 of Regulation 415/06 sets out the rules for maintaining transcripts and what information is required to be in the transcripts. Private career colleges are required to enter into an agreement with a third-party that is responsible for maintaining transcript information off-campus and for issuing transcripts in the event the private career college closes. A copy of any agreement with a third-party or parties who will maintain or issue transcripts must be attached. Third-party transcript arrangements must be in place by September 18, 2007. SECTION IX: ADVERTISING Private career colleges are required to submit a written copy of all advertising you intend to use in the promotion of the PCC or its programs as part of registration. This includes written or oral advertising distributed by any means including newspaper, radio, TV or the Internet. This also includes advertising to be used in foreign markets. All advertising by a PCC must be done in compliance of Part V of Regulation 415/06. If the advertisement is in a language other than English or French, your application must be accompanied by an offi cial translation of the advertisement into English. SECTION I: PRO FORMA FINANCIAL STATEMENTS Appendix A FINANCIAL INFORMATION The Private Career Colleges Act, 2005 requires applicants to submit a pro forma fi nancial statement prepared by a public accountant licensed under the Public Accounting Act, 2004 together with evidence acceptable to the Superintendent that there is suffi cient capital available for the start up and operating cost of the private sector college. When completing the Pro Forma Financial Statements, the following conditions must met: 1. An operating pro forma balance sheet noting all assets and equities (i.e., liabilities and owner s equity) that are associated with the ownership and operation of the college. 2. A statement of income and cash fl ow for the fi rst twelve months of operation of the college. The amounts presented on the statement should be plausible and reasonable, and distinguish between revenues in respect of approved vocational programs and other revenues. 3. The Public Accountant must prepare, sign and date a Notice to Reader. 4. Accompanying notes to the Pro Forma Financial Statements should be presented outlining the basis of accounting policies applied, assumptions and any other information that is deemed material in the reviewing of fi nancial statements. The Public Accountant signing the Notice to Reader must disclose his Public Accountant s license number issued by the Public Accountants Council for the Province of Ontario.
