FLORIDA 2016 ISSUE II Better Quality Is Our Goal We at Staywell want to deliver great care and service to our members. That s why we created our Quality Improvement (QI) Program. The program s goal is to find ways to deliver better care. To do this, we work together with our providers. In 2016, we will keep working with our providers. We want to make sure members get the best care. Some 2015 Goals We Achieved: Worked with providers to deliver preventive health care services to members who needed them; Improved member satisfaction; and Improved provider satisfaction. Our Goals for 2016 Include: Keep working with providers to improve members health; Change quality processes to be more efficient; Reach out to members in ways that help quality measures; and Keep giving members reasons to engage with us.

How Case Management Can Help You Case Management helps members with special needs. It pairs a member with a case manager. The case manager is a nurse. He or she can help the member with issues such as: Complex medical needs; Solid organ and tissue transplants; Chronic illnesses such as asthma, diabetes, hypertension and heart disease; Children with special health care needs; and Lead poisoning. A Staywell staff member will tell you about the program. This program is no cost to you. It gives you access to a registered nurse Monday Friday, 8 a.m. to 5 p.m. We re here to help you! Call us for more information. The number is 1-866-635-7045. We Can Help With Transition of Care Getting you the care you need is important to us. That s why we ll work with you to make sure you get your care when: You re leaving another health plan and just starting with us; One of your providers leaves our network; You leave our health plan to go to another one; and You re growing up and need help choosing an adult primary care provider. We want to be sure you keep seeing your doctors and getting your medications. Please call or have your provider call. We can be reached at 1-866-334-7927. Learn About Your Dental Benefits Do you know what your dental benefits are? These services are available for adults (ages 21 and older): Annual oral exam; One X-ray per year; Two cleanings per year; Care to lessen pain or infection due to a dental emergency; Full dentures and removable partial dentures; and Denture-related procedures. These services are available for children (ages 20 and younger): Annual oral exam; One X-ray per year; Two cleanings per year; Fluoride treatments; Sealants; Fillings; Oral surgery; Crowns; Orthodontic treatment with prior authorization; and Endodontic and periodontal care. VERY IMPORTANT! You must use a dentist in the Liberty Dental network to get these services. You will have to pay for any dental service you get that is not covered. Do you want to change your dentist? Or do you have questions about what is covered? Call Liberty Dental Member Services. The number is 1-888-700-0643. Disease Management Program Have you heard about the Healthy Rewards program? Any member age 20 or younger could receive $10 for going to the dentist. Call Customer Service to learn more. The number is on the back page. Disease Management is a similar program to Case Management. It gives support to members with health problems. These may include, but are not limited to: Asthma; Diabetes; Hypertension; and Heart disease. Do you have any of these conditions? Are you interested in this program? Please call us. Behavioral Health Appointments We want to help you make your next behavioral health appointment. Do you think you may be a danger to yourself or others? This is an emergency. Go to the nearest emergency room. You can also call the Crisis Hotline. The number is 1-855-606-3622. How do you feel? What do you need? Were you just released from a psychiatric hospital? Tell your behavioral health provider right away. He or she needs to see you within seven days of you leaving the hospital. Could your problem turn into a dangerous emergency very soon? This is an urgent care visit. Your behavioral health provider needs to see you within 24 hours. Could your problem become an emergency sometime in the future if you don t get help? This is a sick care visit. Your behavioral health provider needs to see you within one week. Are you already seeing a behavioral health provider? Do you need to schedule a follow-up visit? This is a well-care visit. Your behavioral health provider needs to see you within one month. Which behavioral health provider do you need? Do you need a psychiatric evaluation and/or medication management? See a psychiatrist or child psychiatrist. Do you need psychological testing or counseling? See a psychologist. Do you need counseling for you, your relationship or your family? See a licensed clinical social worker, licensed marriage and family therapist, or licensed mental health counselor. Do you have questions about scheduling a behavioral health appointment? Are you having problems getting the appointment you need? Please call Customer Service. Could a Support Group Help You? Cancer, chronic fatigue syndrome and postpartum depression are widely different problems. But they do share at least one thing in common. Women with these conditions and many others have turned to support groups. Joining a support group may help you get past negative feelings, ease anxiety and improve your quality of life. A group can also help you stick to better behaviors, such as healthier eating. Groups can be a good source of ideas, too. For example, a parenting support group can offer helpful tips if you re a new mom. Not everyone is comfortable in a support group. But if you re thinking about trying one, choose one that: Has a facilitator who makes you feel at ease; Includes a mix of newcomers and people who have been dealing with the issue for some time; and Puts members in touch with other resources. The Benefits of Staywell Add Up Staywell s Healthy Rewards Program is simple. It encourages you to take steps toward living a healthier lifestyle. And it rewards you when you take those steps. Member Discount Card You can get a discount card that can be used at certain stores. Just do any of these things: Fill out and send us your Health Risk Assessment (HRA). Do it within the first 90 days of joining our health plan. See your primary care provider (PCP) within the same 90 days. Complete a smoking cessation program. Join a substance abuse or weight-loss management program. Free Baby Stroller or Portable Playpen Go to six prenatal doctor visits and one postpartum visit. You will get rewards. You will get a $30 debit card. You also get your choice of a free stroller or portable playpen. Items are sent to your home. Seeing your PCP within the same 90 days; Going to any preventive dental visit (for members ages 1 20); Getting all the recommended wellchild checkups for children younger than age 21; Getting screened for breast cancer; Going to all prenatal visits if you re pregnant; and Working with your providers to manage your chronic illness. Take Your Medications Properly Take your prescription medicine the way you re told to take it. This is one of the best things you can do for good health. What happens if you don t take your medicine properly? You may need more doctor visits. You may need unnecessary treatments. And you may need to visit the emergency room or hospital. Do you have concerns or questions about your medicine? Do you have reasons you want to stop taking it? Talk with your doctor or pharmacist. How Can You Get the Most Benefit From Your Medicine? Take your medicine as your doctor directed. Do you want to stop taking your medicine or change how you take it? Talk with your doctor first. Make sure that you always have enough medicine. Mark your calendar with the date your next refill is due. You may be able to get 90-day supplies of your medicine. Call the Customer Care number on your ID card.
