The Los Angeles Lakers, Inc. ( Lakers ) and the Los Angeles Lakers Youth Foundation ( Foundation ) are excited to announce our 3 rd Annual Mentoring Road Trip Contest ( Contest ) which offers Los Angeles County mentoring pairs from selected mentoring organizations the chance to earn the trip of a lifetime! The Contest is open to mentoring pairs who are currently registered with the selected Los Angeles County mentoring organizations listed below and in which the mentor is at least 18 years of age and in which the mentee is a student in grades 6 through 12. The Contest has two components: 1. An essay of 350 words or less, written by the student/mentee about their experience with mentoring; and 2. A video of two minutes of less, created by the mentoring pair which introduces themselves and explains what is special about their mentoring relationship. Up to four (4) lucky mentoring pairs and adult chaperones from the Lakers will fly to Washington DC on Wednesday, February 1, 2017 for an exciting 5 day / 4 night adventure and will attend the Feb 2, 2017 Lakers road game versus the Washington Wizards! To enter the Contest, please review the Mentoring Road Trip Official Rules. Your entry must include: 1. An essay of not more than 350 words, written by the student/mentee. 2. A video of not more than two (2) minutes in length, created by the mentoring pair, explaining what is special about their relationship. 3. A completed Student Information/Release and Authorization Form signed by the mentee and his or her parent or legal guardian, and also signed by the mentor. All entries must comply with all of the Official Rules listed below and must be received by the Los Angeles Lakers by 2:00 pm Pacific Time ( PT ) on Monday, January 4, 2017 at 2151 E. OFFICIAL RULES Los Angeles Lakers Mentoring Road Trip Contest NO PURCHASE NECESSARY TO ENTER OR WIN ANY PRIZE. CONTEST VOID WHERE PROHIBITED. 1. Contest: The Los Angeles Lakers, Inc. ( Lakers ) and the Los Angeles Lakers Youth Foundation ( Foundation, or sometimes collectively with Lakers referred to herein as Sponsors ) are looking for mentoring pairs ( Mentoring Pair ) registered with one of the organizations listed below in these Official Rules, consisting of a mentor who is at least 18 years of age ( Mentor ) and a student mentee in grades 6 through 12 ( Mentee, and sometimes with the Mentor, collectively referred to herein as the Mentoring Pair ), who are registered with Selected Mentoring Organizations (as hereinafter defined) located within the County of Los Angeles. Selected Mentoring Organizations are those selected and contacted by Sponsor for participation in this Contest. To enter the Contest, Mentees must write an essay of not more than 350 words ( Essay ) and the Mentoring Pair must create a video of not more than two (2) minutes ( Video, and collectively with the Essay, sometimes referred to herein as the Entry ), within the specified guidelines stated below in these Official Rules. Odds of winning are dependent in part upon the number of Valid Entries (as hereinafter defined), but winners will be selected from among all Valid Entries based upon the quality of the Entry as determined by Sponsors, in their sole and absolute discretion. 2. To Enter: The Contest will be open from 12:00 PM Pacific Time ( PT ) on December 14, 2016 through 2:00 PM PT on January 4, 2017 ( Closing Date ). To enter, you must (a) complete, and have signed, a Contestant Release and Authorization Form ( CRA Form ); (b) complete, and have signed by the Mentee s Parent/Guardian a Consent Agreement; (c) the Mentee must write and submit an Essay; (d) the Mentoring Pair must produce and submit a Video. CRA Forms, Consent Agreements, Essay and YouTube links to a Video can be submitted via email to community@lakers.com. Alternatively, CRA Forms, Consent Agreement, Essays and a DVD or USB memory stick/flash drive containing the Video can be submitted by mail or hand delivery, to the Los Angeles Lakers, 2151 E. Grand Avenue, El Segundo, CA 90245, Attn: Mentoring Road Trip Contest. In addition, in order to be eligible to win, the Selected Mentoring Organization of the Mentor Pair must complete and return an Affidavit and License Agreement. Only entries that meet all the requirements specified in these Official Rules and are received by the Sponsors on or before the Closing Date at the address listed below in these Official Rules will be considered a valid entry ( Valid Entry ). If mailing by U.S. Postal Service, please allow at least 5 business days for entries to arrive before the Closing Date. Entries postmarked on the Closing Date or delivered after the Closing Date will be considered late and will NOT be accepted or be considered a Valid Entry. Only one entry per Mentoring Pair is allowed. 3. Eligibility: Only Valid Entries submitted by a Mentoring Pair currently registered with a Selected Mentoring Organization and for which both the Mentor and Mentee currently reside in the County of Los Angeles will be considered Valid Entries. Entries must be legible and include all elements of a Valid Entry in order to be eligible. By entering, winners irrevocably agree to allow all or any portion of their Entry to be used, displayed, reproduced and distributed by Sponsors in any and all media, including, but not limited to, shown on the video boards at Lakers home games at STAPLES Center during the month of February, March and April of 2017 and on the Lakers website. Children of employees and immediate family members of Sponsors, the National Basketball Association and its Member Teams, NBA Properties, Inc., and each of their respective parents, subsidiaries and affiliates, and their directors, officers, members, managers, owners, employees, agents, representatives, successors and assigns ( Lakers Entities ), and those residing in the households of such individuals, are ineligible. 4. Selection Process: Individuals selected by Sponsors will review all Valid Entries and select a minimum of three (3) winning Entries and a maximum of four (4) winning Entries. Selection of winning Entries will be made by the individuals selected by Sponsors in their sole and absolute discretion, and all decisions of such individuals and of Sponsors are final and binding. 5. Prizes: Each winning Mentoring Pair will receive the following:.
A five day/four night trip to Washington DC, from Wednesday, February 1, 2017 through Sunday, February 5, 2017 for both the Mentor and Mentee, including: o Roundtrip coach airfare between Los Angeles International Airport and Washington DC. o Double occupancy (Mentee/students rooming with other Mentee/students and Mentors rooming with other Mentors) hotel accommodations at a hotel in Washington DC selected by Sponsors. o Visits to points of interest in Washington DC selected by Sponsors. o Ground transportation from the airport in Washington DC to the hotel, during visits to other points of interest arranged by Sponsors, to the Lakers game against the Washington Wizards on February 2, 2017, and from the hotel in Washington DC back to the airport in Washington DC. o Tickets to the Los Angeles Lakers versus Washington Wizards basketball game at Verizon Center on February 2, 2017. The approximate retail value of each Prize ( ARV ) is $3,000. Sponsors reserve the right to substitute all or any portion of any Prize with other elements of approximately the same value, as determined by Sponsors, in their sole and absolute discretion, and no responsibility is assumed by the Sponsors or any of the Lakers Entities for cancelled, delayed, suspended, rescheduled or substitutes Prize elements. Transportation of the winning Mentors and Mentees to and from LAX, and any other expenses relating to the Prize not expressly provided herein, are the responsibility of the winners. 6. Prize Notification: Winners will be notified by phone, at the number provided by the Mentor and Mentee, on or before Friday, January 6, 2017 a 7:00 PM, PT. Winning Mentors and winning Mentees and their parent or legal guardian will have 48 hours from notification of winning to notify the Lakers that the winning Mentor Pair accepts the Prize and are willing and available to participate in all elements of the Prize and abide by these Official Rules. Within five (5) days of the date of the prize notification and acceptance, the winning Mentor and the winning Mentee and the winning Mentee s parent or legal guardian shall be required to sign and return the Prize Affidavit, Release and License Agreement and any other documents required to accept and claim the Prize, as determined by the Sponsors. Each winning Mentee must have appropriate identification to satisfy air travel security requirements. Failure to respond within 48 hours of prize notification or failure to return the required documents within the time provided will result in a forfeiture of the prize which may be awarded to another Winner selected from the remaining Valid Entries in a manner determined by Sponsors in their sole and absolute discretion. 7. Prize Restrictions: The Prize is non-transferable, and must be accepted as awarded, with no substitutions of any kind, whether in cash or otherwise; provided, however, that Sponsors reserve the right to substitute all or any portion of a Prize with a prize or element of approximately the same value as determined by Sponsors in their sole and absolute discretion. Both members of the Mentoring Pair must accept the Prize. If either a Mentor or Mentee of a Mentoring Pair is unwilling or unable, for any reason, to accept and use the full Prize then the Prize offer shall be rescinded and offered to another Mentoring Pair selected by Sponsors in their sole and absolute discretion, and no substitute Prize or compensation shall be awarded to the original Mentoring Pair or either of them. Each winner of a Mentoring Pair is responsible for any and all federal, state and local taxes, including income taxes, if any, imposed as a result of or in connection with winning or awarding of a Prize or any portion thereof. When required, each member of a winning Mentoring Pair will receive an IRS Form 1099 from Sponsors for one-half of the ARV of the Prize. 8. Additional Terms: By participating, entrants agree (a) to abide by these Official Rules, and the decisions regarding eligibility, selection of winners, and any other elements of the Contest, which shall be made by the Sponsors in their sole and absolute discretion and whose decisions shall be considered final and binding; and (b) to release, discharge and hold harmless Sponsors and each of the Lakers Entities from and against any and all liability, loss, cost, expense (including attorneys fees) or damages arising out of or related to any claims, demands, actions causes of action, suits or proceedings (however named or described) with respect to or arising out of any losses or injuries to the person, including death, or property, or both, of any person, including, without limitation, any one or more of the entrants which may result, be sustained, or be received by entrants, as a result of either (i) entrants participation in the Contest; or (ii) the receipt, use, reproduction, display, distribution or
misuse of all or any portion of the Entry or of all or any portion of any Prize. Sponsors are not responsible for lost, late, misdirected, damaged incomplete or illegible entries, which shall be void, or for entries that are not processed according to these Official Rules. Sponsors reserve the right to amend or terminate this Contest if, in the sole and absolute discretion of Sponsors, any factor interferes with its proper conduct as contemplated by these Official Rules. 9. Official Winners List: Names of winning Mentoring Pairs and their Selected Mentoring Organizations will be posted on the Los Angeles Lakers website (www.lakers.com) beginning January 23, 2017.
