HUMBOLDT SENIOR RESOURCE CENTER Job Description JOB TITLE: DEPARTMENT: REPORTS TO: FLSA STATUS: PACE Center Manager Redwood Coast PACE Redwood Coast PACE Director FT Eempt APPROVED: August 2017 JOB SUMMARY: Under the supervision of the Redwood Coast PACE Director, has full responsibility for operation of the Redwood Coast PACE Center including management, staffing and budgeting responsibilities. Oversees the Interdisciplinary Team (IDT) services provided through the Center. ESSENTIAL DUTIES AND RESONSIBILITIES: 1. Coordinates all Center activities including care planning and implementation, preparation for IDT conferences, daily morning briefing, family conferences and all related follow-up. 2. Ensures HSRC hiring practices are followed for open positions. In collaboration with the Redwood Coast PACE Director assures recruitment, interviewing and hiring to fill open positions are completed within HSRC standard practices. Provides orientation, education, supervision and corrective action for key disciplines on the IDT. Direct reports may include the roles of: Social Worker, Dietitian, Physical and Occupational Therapists, Activity Coordinator, Center Receptionist/Coordinator, Care Coordinator and Personal Care Attendants staffed in the Center. 3. Provides day-to-day oversight of Redwood Coast PACE contract employees and services. 4. Provides leadership and facilitation of the IDT to achieve their goals. a. Ensures operational efficiency on a daily basis. b. Ensures that the IDT assesses eligibility of potential participants, develops therapeutic plans of care for enrollees and any change in condition or emergent issues or concerns are addressed according to regulatory requirements, PACE standards, policies and procedures, or IDT approved deadlines, goals or timelines. 5. Participates on the Redwood Coast PACE Management Team and serves as liaison between administration and staff. Provides leadership and participates on the Redwood Coast PACE Quality Assessment and Performance Improvement (QAPI) Committee and relevant sub-committees as required. 6. In collaboration with the PACE Social Worker and Enrollment Representative, ensures adherence to the enrollment process. Contributes to the enrollment process 2017 Center Manager Page 1
steps as needed. Ensures coordination between PACE Center staff disciplines to achieve efficiency and meet regulatory requirements for enrollment processes. 7. In coordination with the Redwood Coast PACE Director and Quality Assurance Coordinator, receives documents and resolves reports of participant rights violations, grievances, appeals and other issues related to service and quality of care. a. Accountable for the safety and health of Redwood Coast PACE participants and employees. Promotes safe work practices, conducts environmental rounds, attends to, investigates and reports accidents promptly, assists with medical surveillance, and provides PACE-specific safety training. Intervenes immediately when condition(s) eist which pose an immediate threat to participants, staff, contractors, visitors and others. b. Demonstrates respect for and promotes participant rights, including dignity, self-determination, access to care, confidentiality and independence. 8. Works collaboratively with the Redwood Coast PACE Director and Quality Assurance Coordinator to ensure adequate education, training and competency for PACE staff on the Redwood Coast PACE Policies and Procedures. 9. In coordination with the Primary Care Provider, Nurse, and Quality Assurance Coordinator, oversees the use of physical and chemical restraints for participants and ensures adherence to policies and procedures. 10. In coordination with the Primary Care Provider, Quality Assurance Coordinator, and Medical Record Coordinator, oversees the operation of the medical records system. Identifies improvements, corrections and/or training needs for staff to utilize the electronic health records to achieve accurate and timely evaluation of participant s plan of care. 11. In collaboration with the Facilities Supervisor, oversees the maintenance and repair of all non-medical and non-electrical equipment. 12. In coordination with the Quality Assurance Coordinator ensures Redwood Coast PACE emergency and disaster preparedness and reviews policy and procedures with staff at least annually. 13. Participates in program and policy development of Redwood Coast PACE. 14. Understands fiscal issues and is appropriately involved in meeting budget targets. 15. Consistently meets or eceeds Redwood Coast PACE targets for productivity. 16. Consistently meets or eceeds Redwood Coast PACE quality assessment and performance improvement targets. Continually seeks better ways for delivering services and communication with participants. 17. Consistently meets or eceeds Redwood Coast PACE customer services targets. 18. Understands the importance of community involvement and participates as appropriate in activities that link Redwood Coast PACE to its communities. 2017 Center Manager Page 2
19. Serves as a supervisor to student interns and volunteers working within the professional discipline and program area. Participates actively in their training as a guide, teacher, and mentor. EDUCATION &/or EXPERIENCE: 1. Degrees and diplomas: Bachelor of Science in Nursing or advanced degree in Social Work, Public Health, or other related discipline. 2. Eperience: Five years eperience in Health Care or Social Services with at least two years supervisory eperience and one year working with a frail or elderly population. Knowledge of physical, social and mental health requirements of adult day care and PACE. LICENSES OR CERTIFICATES: 1. Achieve marketing certification for PACE enrollment. 2. Valid California driver s license with good driving record. Verification of completion of CPR (BLS) and first aid training. TB screening and successful completion of a health eamination by a hospital-designated nurse practitioner or physician is required for employment. JOB SKILLS: 1. Ability to sit at a desk and interact with a computer screen for etended periods of time. 2. Physical abilities sufficient to move between different staff work areas, communicate with staff and the public, operate a computer, produce reports, talk on the telephone, and travel to other Agency worksites.
