About the service Perth & Kinross Council - Adults with Learning Disabilities supports adults with a learning disability requiring care and housing support services in their own home. The service aims to develop and encourage decisionmaking skills and involvement in the planning for all aspects of their lives. This service registered with the Care Inspectorate on 1 April 2011. What people told us During the inspection we spoke with some of the people who used the service. They were happy with the support they received and told us that they got on well with the staff who supported them. They were able to describe the sort of support they received, such as "staff help me to go and do my shopping" or "if I need to, the staff can help me go to appointments like the dentist". Self assessment We did not request a self-assessment this year. We discussed and considered the service's own development plan as part of this inspection. From this inspection we graded this service as: Quality of care and support Quality of staffing Quality of management and leadership 5 - Very Good 5 - Very Good not assessed What the service does well Personal plans included comprehensive information on how tenants would like to be supported. These were personalised to suit the needs of individual tenants, and had made use of alternative methods of communication such as Talking Mats and Boardmaker. We saw a communication book which gave good information on what a tenant planned to do on any given day, and what support he needed from staff. It also allowed staff to record any important information. This tenant was also able to confirm that he knew which staff were scheduled to support him. Reviews had been carried out, and minutes were in most of the files we sampled. Passports were now in every file and gave useful information about tenants. File audits were completed on an annual basis, and this identified any actions required to be carried out to bring paperwork up to date. Information was generally of good quality and looked at positive outcomes for tenants. Following a recent recording administration issue, there had been a review of the medication procedure and audits were completed to ensure compliance as well as competency based assessments of staff skills carried out. We could see that these had been carried out and staff were observed to be competent, or that additional observations were being carried out to ensure that staff were competent in specific areas of the administration page 2 of 6
of medication. The service had appropriately notified the Care Inspectorate of these medication errors and the actions they were taking to minimise risk to tenants. New staff had had induction training recorded, and all staff had access to a range of other training. Records were kept of individual staff training, as well as training by course. Staff said they could access mandatory training, although some commented that they felt they still needed some additional training such as Talking Mats. They also said that they generally felt supported by management. We spoke with a variety of staff during the inspection and they told us what they thought about working in the service. They said that they usually felt that they were well-informed about any changes, and that information was passed round the team. They felt that tenants were involved in the development of their care plans, and in the review process. Most staff said that there were enough staff to do the things they felt would benefit tenants. What the service could do better The monitoring of medication continued to be a priority for the service, and during the inspection we identified some minor additions to be made to policies and procedures. These changes related to ensuring the notification of errors in the administration of controlled drugs to the Care Inspectorate, and changes to wording of handover medication checks. Although we were confident that reviews had taken place appropriately we did not always see minutes of reviews which had been carried out. In one file we saw that Talking Mats had been used as a communication aid, but we could not see how this had been used to for example, inform review minutes. Most staff we spoke with said that they would benefit from more staff meetings, and that these did not happen frequently enough. This may reflect the amount of lone working staff do, and additional staff meetings could positively affect staff morale. They also said that they felt they did not have enough time to complete e:learning sessions in work time, or that this was logistically difficult to arrange - for example access to a computer.
