- COMPLAINT INSPECTION (559) 300-5742 PR0000349 August 31, 2017 CERTIFIED FOOD HANDLER: EP DATE: A complaint was recently received by our Department alleging violations of the California Retail Food Code at this facility. In response to the complaint, an inspection of the facility was conducted and our investigation findings are provided below. If violations are noted, then appropriate corrective action is required as listed in the main section of this report. Thank you for your cooperation. One reinspection will be conducted (if needed) at no charge. A service fee is assessed for each additional reinspection required. Inspection Findings and A complaint was received by our department regarding mouse droppings and the concern of the temperature inside the facility due to non-functional AC unit. Complainant stated that he observed an employee handling ready to eat pizza while the employee was perspiring and the sweat was dripping on or around food areas. Observation: Noticed a hole above the coving inside the employee restroom. The manager will have to repair and have all walls and floors sealed to avoid the entry way for vermin. Keep Eco lab pest control services on a bi-weekly service until the problem is completely resolved. The manager will have until September 5th 2017 to comply. Please submit proof of repair via fax or email. - Due to the malfunction of the AC unit, and considering the weather conditions in Hanford, CA., the facility will have to provide adequate devices to cool off the kitchen area to avoid uncontrollable, unhygienic conditions and make the environment comfortable for safe food prep. This is also in direct violation of adequate ventilation law in the California Retail Food Code. Send in the work order request by Friday September 1, 2017 at 5pm as a proof that attempt was made. This is the third complaint in the past one year, any further complaints or re-inspections done by our department, or non-compliance to fix the above violations will result in a Administrative hearing with our department that can result in revoking your permit to operate and/or fines. Thank you IMPROPER ECLUSION OF VERMIN OR ANIMALS [HSC 114259-114259.3] Observed a hole on the wall inside the restroom used by employees at the back of the store. Several places along the coving is also damaged and appeared in deteriorating condition. This is most likely the source of the mice entry. VERMIN INFESTATION [HSC 114259.1] A fly infestation was noted during the inspection. This facility will be required to install fly exclusion devices such as a air curtain or approved fly zappers from Eco lab or other approved sources. IMPROPER CLEANLINESS OR HYGIENE OF FOODHANDLERS [HSC 113967-113973] This facility has no functional AC for the past few weeks. This in turn is creating a hot, unworkable, unhygienic environment for the employees. Outside temperatures for the past week and on going week will be above 100F in the afternoon in addition to the heat generated inside by the pizza cooking ovens. Be sure to throw trash away regularly. Observed in door trash cans full. Throw and empty out frequently to the outdoors. Also, keep the organization of all food products. Use 3 compartment sink for sanitizing all trays and utensils. (559) 300-5742 PR0000349 May 30, 2017 EP DATE: VERMIN INFESTATION [HSC 114259.1] Flies were noted inside the food facility. Food debri and unsanitary conditions attract vermin. Last Pest Control Service was done 5/1/17. Manager was unaware of the frequency of the service. He stated the owner calls them whenever. IMPROPER MAINTENANCE OF FACILITY OR EQUIPMENT [HSC 114161-114182 & 114257] No daily cleaning schedule was noted. IMPROPER REFUSE STORAGE OR DISPOSAL [HSC 114244-114245.8] Refuse inside boxes were noted by the prep area and inside restroom. Trash shall be taken out daily to avoid flies and keep the food prep area clean. FOODS & EQUIPMENT NOT PROTECTED FROM CONTAMINATION [HSC 113980, 114025-114027] All foods need to be covered inside the walk-in and reach in units. IMPROPER CLEANING OF UTENSILS AND EQUIPMENT [HSC 114095-114099.5 & 114101-114119] Facility observed with old debri and trash inside and around the corners. All equipment was observed with food debri and dirt/dust around. All floors, walls, ceiling, equipment handles, pizza warming units, corners of all areas, inside walk-in, inside restroom shall all be cleaned and sanitized. IMPROPER MAINTENANCE OF HANDWASH FACILITIES [HSC 113953-113593.2] Hand washing station observed really dirty with old food debri. Equipment needs to be kept clean and sanitized at all times.
