ANNEXURE -3 THE OPEN UNIVERSITY OF SRI LANKA FACULTY OF HEALTH SCIENCES ADD/DROP OF COURSES INSTRUCTIONS a. Course materials obtained for courses to be dropped should first be returned to Regional Service Centre before submitting this form. b. Students must fill cages 1-4 of the form given in annexure 4 and hand over the completed form to the Academic Counsellor with the student file. c. Courses at lower level cannot be dropped unless all courses offered at higher level are also dropped. d. You are not allowed to add courses unless you have paid the full voucher sent to you without any changes. If you have got your voucher reduced, you cannot add courses on the basis of additional fees paid. e. Please refer Add/Drop schedule given in Annexure 1 for the dates and venues for Add/Drop. 4

ANNEXURE- 5 THE OPEN UNIVERSITY OF SRI LANKA FACULTY OF HEALTH SCIENCES DROPPING OF COURSES WITHOUT ACADEMIC PENALTY 2016/2017 (The drop date is 29 th September 2016 There will be no extension of this deadline under any circumstances) INSTRUCTIONS (A) (B) (C) (D) Courses CANNOT be dropped at a lower level unless those offered at a higher level are also dropped. All course materials obtained for courses to be dropped should first be returned to any Regional/Study Center and an appropriate entry be made in the student s registration book. However, you have to report to Dean/ Health Sciences and carry out the instructions below in order to affect the drop of courses. Fill the form given below and hand it over to the Dean s Office with your record book by 10 a.m. on 29 th September 2016. If you hand over the form by 10 a.m., then every effort will be made to have the approved form ready for collection by 2 p.m. at the latest. Request for dropping of courses will be entertained only ONCE per student. Once the form is signed by the Dean, it will be your responsibility to take it to Student Affairs Division and have the dropping of courses and officially recorded in your student record book.
