Doncaster services Information for service users and carers RDaSH Older People s Mental Health Service

Introduction The Doncaster Older People s Community Mental Health Service offers a wide range of services based on the St. Catherine s site. We provide assessment and treatment to older people aged 65 and over with mental health problems and under 65 with a diagnosis of dementia. All services work on a Monday to Friday basis between the hours of 8.30am and 5.00pm but there is also an emergency service. All referrals come from a GP who will contact the Community Mental Health Team (CMHT) via fax to the single point of access 01302 796500. Tel: 01302 796104. Out of hours: 01302 798400 The CMHT will screen the referral and decide whether a member of their team should visit you or whether another team would be more appropriate. Community Mental Health Team Cherry Tree Court 01302 796104 The CMHT offers advice and support to older people living in the community with mental health problems. Once referred to the service a Community Mental Health Nurse, Social Worker or an Occupational Therapist (OT) may see you. Community Mental Health Nurse A Nurse may visit you at home and assess your mental health. They will then co-ordinate care and treatment. Occupational Therapist OT s look at all the aspects of daily life a person may be having difficulty with, from self-care, to cooking, to accessing meaningful activities. Social Worker Social workers form the link between social services and the NHS. A social worker may visit you at home to help sort out problems with care, health, housing or benefits www.rdash.nhs.uk 1

Carers Support Group Forest Gate Day Hospital 01302 796708 There is a carer support group run on the 1st and 3rd Wednesday of every month for those with dementia. Memory Clinic Forest Gate Day Hospital 01302 796708 Memory Clinic staff will complete an assessment of your memory and monitor this if required. Forest Gate Forest Gate Day Hospital 01302 796080 Forest Gate is open two days a week for dementia patients and once weekly for patients with mental Illness. The Staff here provide ongoing assessment and treatment. Community Memory Therapy Services Catherine s Close 01302 796138 The Community Memory Therapy Service provides therapeutic day care and ongoing carer support for those supporting people with dementia. Young Onset Dementia Service Forest Gate Day Hospital 01302 796080 The Young Onset Service will provide support and assessment for those adults who were diagnosed with dementia while under the age of 65. Liaison Service Tickhill Road Hospital 01302 798467 Fax for referrals 01302 798474 The Liaison Service provides advice, assessment and support to people in both residential care and general hospital care. They provide carer support, a befriending service and a memory café. South Yorkshire Centre for Inclusive Living (SYCIL) 01302 892949 www.sycil.org The independent living unit provides impartial advice about equipment and practical aspects of daily living for disabled people.
