ADDENDUM NO. 3 TO REQUEST FOR PROPOSAL #481 for Modular Office Systems and Free-Standing Office Furniture and Design/Installation Service The following shall be incorporated in RFP#481 by reference: As announced at Pre-Proposal Conference Number 2 held on Tuesday, March 14, 2006, Addendum No. 3, the Prevailing Wage Determination Form SFR-2003-1, and Exhibit O PROJECT LABOR AGREEMENT FORM will be posted to RFP Depot. Also included is a list of people who attended the conference. Information provided and responses to questions asked at Pre-Proposal Conference Number 2: A brief background and overview on Project Labor Agreements (PLA) and its applicability to the RFP was provided. Contractors and sub-contractors may be either union or non-union; however they must abide by the provisions of the PLA if awarded the contract resulting from the RFP process. Q1: Does the County have a PLA requirement for the on-going day to day business? A1: The County does not have a PLA requirement for day to day business. Five bond projects described in Exhibit M Description & Scope of Bond Projects of the RFP require a PLA. The projects are as follows: Valley Specialty Center, Valley Health Center-Gilroy, Valley Health Center-Milpitas, Valley Health Center Fair Oaks, and the Crime Laboratory. Q2: Is the prevailing wage requirement applicable to everything? A2: Yes, prevailing wage is applicable to installation of modular private office and workstation systems furniture as well as the delivery and set in place of free standing furniture for both PLA related projects as well as non-pla related projects. Prevailing Wage Determination SFR-2003-1 made by the Director of Industrial Relations Pursuant to Government Code, Section 41920 shall be applicable to work categorized by the County as delivery and set in place and the rates are applicable for Santa Clara County per the Division of Labor Statistics and Research. Q3: Is the prevailing wage requirement applicable to the mock set-up process? Addendum No. 3 Page 2 of 5
A3: As the mock set-up is part of the proposal phase and there is no contract covering this phase of the RFP process, the prevailing wage requirement is not applicable. Q4: Will Exhibit O Project Labor Agreement Form be posted soon? A3: Yes, Exhibit O will be posted on RFP Depot today, March 14, 2006 along with the questions and answers from the March 14, 2006 pre-proposal conference. Please note, Exhibit O will need to be submitted with your proposal. Q5: Are the proposals still due this Friday? A5: Yes, proposals are due by 3:00 p.m. on Friday, March 17, 2006. Q6: What are the actual presentation date(s) of the mock furniture presentations? A6: The mock-up portion of the RFP process is scheduled for March 27-29, 2006. The installation date is Monday, March 27 th and the outside consultants will evaluate mock ups on the afternoon of March 27 th. The presentation date for County evaluators is Tuesday, March 28 th. Q7: Will sufficient time be given to each vendor for their presentation? A7: The schedule has not been determined at this point, however, the time provided for presentations will be based upon the number of proposals received in response to the RFP. As a reminder, both the Project Manager and Account Manager should be present at the mock-up furniture presentation on Tuesday, March 28, 2006.
