Research by Orygen (2016) Wrking with cultural diversity in early psychsis, highlights that Abriginal and Trres Strait Islanders cmmunities have cnsistently lwer rates f access t mental health services (cmpared t the general ppulatin but have higher rates f invluntary admissins and psychsis. The Yuth Psychsis prject is funded by NWMPHN. One f the prject aims is fr VAHS t cllabrate and build partnerships with mainstream mental health services t assist in reducing the barriers assciated with Abriginal Yuth accessing mainstream services and getting equitable mental health care. This prject will invlve establishing cmprehensive assessment, care crdinatin and develpment f a fllw up treatment mdel fr yuth experiencing early and first episde psychsis. VAHS is in a unique psitin t prvide Indigenus yung peple wh may be experiencing first episde psychsis with culturally safe and apprpriate supprt services. POSITION SCOPE The Kri Kids Prgram perates within a clinical and cultural mdel f care that delivers mental health and wellbeing services t Abriginal children, yung peple and their families. The Yuth Psychsis Prject will invlve cmprehensive assessment, care crdinatin and fllw up treatment, thrugh culturally safe and apprpriate supprt services, fr Abriginal yuth (aged 12-25 years) experiencing first episde and early psychsis, r at risk f develping first episde psychsis. This will als invlve building strnger relatinships with mainstream services t prvide Yuth and their families better access t acute r inpatient care when needed and imprved transitin between services in rder t supprt the yung persn s recvery within the cmmunity. utilising an assertive utreach apprach t build n ur partnerships with services in the NWMPHN Catchment area and t identify and engage with yung Indigenus peple wh have been unable t access supprt. KEY RESPONSIBILITIES AND ACCOUNTABILITIES 1. Service Delivery Wrk in partnership with the Abriginal Health Wrker t gain the trust, understanding and engagement f Abriginal yuth and their families presenting with early psychsis and ensure that AHWs interpreting f family and cultural infrmatin is utilized in all clinical assessments in rder t ensure cultural and clinical integrity Participate in intake prcesses Prvide Primary, Secndary and Tertiary cnsultatins t enable staff in each sectr t respnd mre apprpriately t the particular needs f Abriginal yuth wh are experiencing early psychsis Carry a case lad f mental health clients, prviding a range f mental health assessments, interventins and supprt Prvide case management and care crdinatin fr Abriginal Yuth experiencing early psychsis Develp systems and netwrks t better supprt health service prviders and primary health care prvider respnses t the mental health/sewb needs f Abriginal yuth wh are experiencing early psychsis Imprve access t culturally-apprpriate and safe mental health services fr Abriginal yuth wh are experiencing early psychsis Undertake cnsultatin with relevant netwrks and agencies t develp an Abriginal Yuth Psychsis Frum fr the NWMPHN area. Develp significant relatinships with a range f agencies, in particular with 2 key mental health services in the NWMPHN catchment area and with agencies in the Hume and Meltn areas. 2. Case Management and Family Supprt In cllabratin with AHWs participate t identify, assess, treat and supprt yuth and families f the cmmunity presenting with r at risk f early psychsis: Prvide advice regarding yuth requiring clinical and therapeutic interventins t VAHS staff members and ther external stakehlders Onging assessment, dcumentatin and supprt f clients thrughut their invlvement with the prgram Supprt and cntributin t treatment gals in cllabratin with yuth, their families and ther staff Participate in case management, prgram and general staff meetings Supprting and wrk with families (parents/carers/guardians) f yuth engaged with the prgram Maintain relevant reprting duties such as case ntes and data entries Prvide utreach and mentring t yuth at hme, in the cmmunity and ther relevant settings in accrdance with the agreed care plan Exercise judgment and initiative in prviding utreach that ensures a hlistic, apprpriate and safe service t clients Page 2 f 5

Prgram Linkages Facilitate linkages t client systems such as: VAHS internal prgrams including: Adult Mental Health Team, AOD Team, Healthy Lifestyles Team, Wmen s and Children s, Medical, etc Abriginal services such as VACCA, VALS, AAL, Elizabeth Mrgan Huse, W.T Onus Hstel, Bert Williams Hstel, and ther agencies Mainstream health services including The Alfred, Mercy, Austin, Nrthern, Eastern and the Children s hspitals Department and mainstream cmmunity rganisatins and prgrams including Headspace, Orygen, Berry Street, Westcare, YSAS, Child Prtectin, Yuth Justice, Victria Plice, etc Secndary schls and Alternative Educatin settings lcated within the nrth/west catchment 4. Prgram Administratin / General Duties Maintain detailed case ntes in the client s file Ensure regular and nging reprting as per NWMPHN requirements Ensure all client cntact infrmatin is entered int the VAHS Cmmunicare system Receive nging supervisin and prfessinal develpment frm the Kri Kids Team Leader and ther relevant staff fr directin and supprt Develp an understanding and prmtin f the philsphy f the Victrian Abriginal Health Service Undertake ther duties, which are incidental and peripheral t the main tasks, prvided that such duties are reasnable within the emplyee s cmpetence and training Ensure service delivery is in line with Mental Health Act and ther relevant legislative requirements 5.
