Position Title: Director of Specialized Day Services Supervisor: Chief Operating Officer Department: Community Care Services (CCS) Status: Exempt Effective Date: August 24, 2017 Major Responsibility: To supervise the operations and management of all Day Care Services Centers and Case Management to ensure performance expectations are met in the areas of quality care and regulatory requirements. Position Description: The Director of Specialized Day Services is responsible for the overall administration and operation of Alzheimer s Community Care s Specialized Adult Day Service Centers (SADSC) and Case Management. The Director of Specialized Day Services coordinates resources and staff to maintain compliance with all regulatory and funding requirements as well as ensuring continuing adherence with internal standards. The Director of Specialized Day Services assists with the implementation and evaluation of new policies, procedures and protocols, as appropriate, and serves as a liaison between the Vice President of Education & Quality Assurance, day center staff and case management. Additionally, the Director of Specialized Day Services is responsible for managing departmental projects as assigned by the Chief Operating Officer. This position requires an independent, selfstarter and someone capable of managing tasks and projects with minimal supervision. The Director of Specialized Day Services is organized, detail oriented, possess strong analytical and critical thinking skills. Additionally, Director of Specialized Day Services consistently demonstrates the ability to communicate effectively with patients, caregivers, staff, Leadership and volunteers. Position Responsibilities 1. Oversees the operation of all Specialized Adult Day Service Centers (SADSC) and Case Management through the establishment & evaluation of annual goals and objectives. Provides measurable outcomes for services provided. 1. Adheres to all Alzheimer s Community Care policies and procedures as they relate to the operations of the SADSC; 2. Ensures compliance with requirements outlined in contractual agreements affecting the SADSC through creating and maintaining a tracking system to identify items due as well as submitting items needed within appropriate time frame; 3. Conducts comprehensive monitoring in each SADSC annually, at a minimum, inclusive of nursing documentation, patient files, QA Book, licenses, permits and certifications, personnel information, environmental safety, etc.; 4. Oversees ongoing quality assurance procedures related to clinical care;
5. Coordinates staffing at each SADSC, inclusive of the utilization per diem and/or agency staff; 6. Substitutes as staff at the SADSC, in emergency situations, including providing care coordination, therapeutic activity intervention and/or personal care, including but not limited to assisting patients with ambulation, transferring, toileting, bathing, feeding, and other personal care issues; 7. Updates and maintains the Specialized Day Services Policy and Procedures Manual, in addition to training staff on policies and procedures contained within; 8. Assists in implementing new programs/initiatives, as requested by the Chief Operating Officer; 9. Develops and monitors the marketing plan for the day centers to increase patient census and the future growth of the organization. 10. Assists in long range planning activities and suggests improvements/changes in policies and procedures; 11. Assists with the administration of regularly scheduled consumer satisfaction surveys related to services provided and utilizes results to identify and implement appropriate changes as needed; 12. Participates in meetings with staff, caregivers, family members and others regarding patient care and services, as needed; 13. Works in coordination with the Operation Managers and VP of CCS to assists with the creation and /or revision of forms and documentation utilized in the SADSC, as directed by the Vice President of Community Care Services; 14. Trains, evaluates, counsels, mentors and supervises staff, with assistance of both the Chief Operating Officer and Human Resources; 15. Facilitates regularly scheduled department meetings including, but not limited to, separate meetings of the Program Managers, Program Nurses and Activity Coordinators; 16. Generates, reviews and/or distributes reports, documents, correspondence and other information as requested by the Chief Operating Officer; 17. Coordinates DOEA annual training for ACFP of Program Managers and collects ACFP data in the day centers biannually. 18. Maintains open channels of communication with staff; 19. Participates in the Organization s Disaster/Emergency Preparedness and Business Recovery Plan as outlined or requested; 20. Assists with staffing at the SADSC, as requested, on an emergency basis, providing care coordination, therapeutic activity intervention and/or personal care, including but not limited to assisting with ambulation, transferring, toileting, bathing, feeding, and other personal care issues; 21. Performs other duties as assigned. Supervisory Responsibilities The Director of Specialized Day Services directly supervises an administrative assistant, all Program Managers, all per diem SADSC staff and the Supervisor of Case Management.
Supervision Received The Director of Specialized Day Services is supervised by the Chief Operating Officer Position Relationships Staff & Volunteers: The Director of Specialized Day Services works closely, on a daily basis with Specialized Adult Day Service Center staff, Leadership, co-workers and volunteers at Alzheimer s Community Care s headquarters location. Governing Structure: Not applicable to this position. Government: The Director of Specialized Day Services may work closely with appropriate regulatory and funding agency personnel regarding Alzheimer s Community Care s contractual agreements, measurable outcomes, regulatory compliance, and steps necessary for service development. Public: The Director of Specialized Day Services has extensive contact with patients, caregivers, family members and various community organization representatives. Work conditions may include, but are not limited to: 1. Walking or standing 25% of the time and sitting 75% of the time; 2. Works away from office 50% of the time driving to the various day centers Driving in Palm Beach, Martin & St. Lucie counties. 3. Lifting or assisting with lifting in patient transfers from wheelchair to chair, wheelchair to toilet, vehicle to wheelchair, etc. when working in the Specialized Adult Day Service Centers; 4. Assisting with personal care needs on an as needed basis; 5. Operating basic office machines/equipment; 6. Working in shared group space with moderate to high noise levels; 7. Working with frequent interruptions; 8. Working and interacting with a Departmental team in excess of 60 people; 9. During emergency conditions, may be exposed to long working hours and/or heat, rain, wind or cold; 10. During working hours, may be exposed to offensive odors; 11. During working hours, may be exposed to bodily fluids and is required to utilize universal precautions; 12. Must be able to multi-task and work well under pressure. Position Requirements Education: Graduate from a diploma, associate, or baccalaureate Registered Nurse program and possess an active, clear Florida RN license, with no restrictions on nursing practice. Must maintain an active, clear Florida RN license at all times during employment.
Experience: The Director of Specialized Day Services must have a minimum of five (5) years of clinical experience in a health care setting with at least three (3) years of experience working with patients affected by Alzheimer s Disease or related disorders. The Director of Specialized Day Services must have a minimum of two (2) years of supervisory experience. Experience working in a dementia specific adult day care center is preferred. Essential Job Requirements of Position 1. Must portray a positive and professional manner at all times; 2. Must be free of communicable diseases. Will be required to update physical and TB testing annually; 3. May require work in excess of scheduled days/hours; 4. Must be articulate and demonstrate excellent communication skills both verbally and in writing; 5. Must maintain high levels of confidentiality regarding patient and family issues, diagnoses, financial status, etc. All federal confidentiality and/or super-confidentiality laws related to medical records apply to this position; 6. Must possess the ability to lift 50 lbs. maximum with frequent lifting and carrying of objects weighing up to 25 lbs; 7. Must dress in professional business attire daily (unless conditions warrant casual attire); 8. Must possess an excellent understanding of the rules and regulations governing adult day care centers in the state of Florida as well as ACC policies and procedures; 9. Must possess an excellent understanding of dementia specific adult day care center operations; 10. Must possess the ability to effectively supervise and manage staff and volunteers; 11. Must be able to effectively facilitate department and team meetings; 12. Must be free of communicable diseases. Required to update physical and TB testing annually. 13. Must possess a safe and valid driver s license, current automobile insurance and reliable transportation as the position requires frequent travel to service locations and/or meetings; Skills/Qualifications 1. Must hold current CPR certification; 2. Must possess a working knowledge of local, community-based services; 3. Must be computer literate and proficient in current Microsoft applications as well as use of all office equipment (i.e. multi-line phone system, fax, copier, postage machinery and meters); 4. Must have reliable transportation to travel to other locations; 5. Must be fluent in the English language, in writing, and orally; 6. Must be multi-task oriented and have the ability to work well under pressure; 7. Must be a self starter with exceptional organizational skills; 8. Must possess a high regard for confidentiality;
9. Must be able to trouble shoot existing problems, and ensure timely follow up regarding resolutions, as needed; 10.
