REQUEST FOR PROPOSAL REGISTERED DIETITIAN Dear Prospective Contractor: Mobile Community Action, Inc. (MCA) is seeking proposals from Licensed, Registered Dietitians for (17) seventeen (but not limited) Head Start Centers located in the Mobile and Washington Counties. The Dietitians and the Nutrition Coordinator will provide nutrition assessments, education and resource development for the Head Start Children and families. In order to be considered for this contract, please submit a resume and references. The contract is contingent upon provider passing Local, State and Federal Background Checks. Objective: The objective of this contract is to ensure the nutritional requirements and assess outcomes for childen served by the Head Start Program. Participates in the development, organization and evaluation of the agency nutrition services as needed and support the development of nutrition related policies. Contract Duties and Responsibilities Assess nutritional needs and current health plans to implement dietary-care plans Assist the Nutritionist Coodinator to determine nutrition for children and the families. Assist in developing menu and providing nutritional analysis for the program Educate parents about nutrition and special dietary needs and practices. Review nutrition assessments as needed for up to 1243 children plus others as enrolled throughout the year. Respond to In- House referrals Conduct training with assistance from the Food Service Supervisor and the Nutritionist Coodinator Train Food Service Staff on how to prepare and serve meals Assist in Site Monitoring to observe classroom setting and food service operation and sanitation. Requirements Must be a least 25 years of age Knowledge of USDA s Child Nutrition Program and Head Start Early Development Knowledge of Federal Regulations and Head Start Performance Standards as they relate to nutrition for children. Knowledge of appropriate methods of food service sanitation Knowledge of developmentally appropriate practice in need children Submit a physical to include a TB test within 15 days after the contract date Closing Date May 7, 2018 Page 1 of 6
The selected consultant must provide a (FBI) criminal history records to include fingerprint check report 30 days after the contract in sign and before entering the Head Start Centers. Must pass Local, State and Federal background check. (Provider is responsible for the cost) Must have valid Driver s License and Proof of Insurance Training or coursework in nutrition, nutrition education, and food preparation Demonstrated ability to carry out the physical demands of the position Basic computer skills Understanding of the principles of bilingual and multicultural education Responsible for transportation to and from Head Start Centers (mileage should be included in proposal cost) Education/Certificates and/or Experience Current License Alabama State Board of Examiners Minimum of five years working with children and families License Within five (5) business days the sucessful bidder shall provide license(s), certificate (s) and/or diplomas that are required by the State of Alabama. Contract Period It is the intent of the request for proposal to award a one-year contract, with the option to renew at the discretion of the director for four (4) additional one-year fiscal terms. This contract may be terminated for cause at the end of the contract year without recourse to the agency. A thirty (30) day written notice from either party shall serve as notice of termination of the contract. Hold Harmless and Indemnification By submitting a proposal, the provider agrees that, if it is the successful bidder, it will indemnify, hold harmless and defend MCA, its officers, agents, and employees from and against all liability, suits, claims, demands, and expenses, including attorneys fee, howsoever arising or incurred, alleging damage to property or injury to or debt to any person arising out of or attributable to the provider s performance or non- performance of the contract awarded. Proposals Submission To be considered, proposals must be received at the desk of receptionist at Mobile Community Action at 461 Donald Street, Mobile, AL 36617 by 4:00 p.m. Monday, May 7, 2018. MCA reserves the right to reject any or all proposals submitted or to request additional information or clarifications from proposers, or to allow corrections, errors or omissions. Proposals should be as thorough and detailed as possible so the Evaluation Committee may properly evaluate the bidder s capabilities to provide the required services. Award of Contract Award will be made to the responsive and responsible provider who can provide the scope of services, meet the minimum requirements. Proposals will be reviewed and rated by Mobile Community Action, Inc. personnel. Each of the items listed in the Proposal Submission Requirement will be reviewed. Company selection will not be based solely on LOW BID. Evaluation Procedures An evaluation/bid committee shall review each proposal based on the following criteria. Responsiveness of the proposal clearly, stating and understanding the work to be performed Technical experience of the Registered Dietitian Consultant.
