Caleb) Pages: 6 This supplement implements and extends the guidance of Air Force Manual (AFMAN) 23-220, Reports of Survey for Air Force Property. It applies to all Air Force military, civilian, and contractor personnel under contract by the Department of Defense (DoD) who become involved in the report of survey for Air Force property process on Vance Air Force Base (Vance AFB). Ensure that all records created as a result of processes prescribed in this publication are maintained in accordance with AFMAN 33-363, Management of Records, and are disposed of in accordance with the Air Force Records Information Management System (AFRIMS) Air Force Records Disposition Schedule (RDS) located at https://www.my.af.mil/gcssaf61a/afrims/afrims/. Refer recommended changes, comments, and questions about this publication to the Office of Primary Responsibility (OPR) using AF Form 847, Recommendation for Change of Publication; route AF Form 847 from the field through the VAFB Publications and Forms Manager. (AF Form 847 is prescribed in AFI 11-215, USAF Flight Manuals Program [FMP]. Refer to that publication for guidance on filling out the form.)

2 AFMAN23-220_AETCSUP_VANCEAFBSUP 13 JULY 2017 SUMMARY OF CHANGES This document is substantially revised and must be completely reviewed to bring the Report of Survey program in line with the requirements of the basic manual and AETC supplement. 2.3.25. (Added) Group Commanders will appoint in writing a primary and alternate to serve as the group Report of Survey (ROS) monitor and forward the letter to the wing ROS program manager. The appointment letter must contain name, rank, office symbol, duty phone, and subordinate units for which they are responsible. Appointment letters will be updated annually in January. The group ROS monitor s responsibilities are: 2.3.25.1. (Added) Receive ROS Program briefing from wing ROS program manager and the Judge Advocate s office within 30 days of appointment. 2.3.25.2. (Added) Have a good understanding of the ROS program and be responsible for managing the ROS as it progresses through the process from beginning to end per Attachment 1. 2.3.25.3. (Added) Will ensure a copy of the Investigating Officer (IO) appointment letter is forwarded to the wing ROS program manager and receives a ROS control number. 2.3.25.4. (Added) Will ensure the IO, if not designated as Primary or Alternate Group ROS monitors, receives briefing from Judge Advocate s office and from the wing ROS program manager; if necessary. 2.3.25.5. (Added) Assist the IO to ensure an investigation is conducted and the ROS package is assembled and processed timely and properly IAW AFMAN 23-220, Chapter 5 and supplements. 2.3.25.6. (Added) Maintain a log of all instances when there is loss, damage, or destruction of government property belonging to the group and the action that was taken. This log should contain the property description, amount of the loss or damage, responsible individual(s), organization involved, the appointed investigator, the ROS number provided by the wing ROS program manager, and the status of the ROS. The group ROS monitor will report negative trends/problem areas to the ROS program manager and Unit Commanders semiannually. 2.3.25.7. (Added) Will inform the wing ROS program manager and Unit Commanders of current ROS status and applicable timeframes. 2.3.25.8. (Added) Will forward a copy of the initiated ROS DD Form 200, Financial Liability Investigation of Property Loss, signed by the Responsible Officer to the wing ROS program manager. Upon receipt of the DD Form 200, the wing ROS program manager will forward to the Accountable Officer to clear the item(s) from accountable records. 2.3.25.9. (Added) Will ensure the ROS is forwarded to the Judge Advocate s office for a legal review if financial responsibility is being sought. The legal review will be accomplished after the Responsible Officer has signed the DD Form 200 and prior to the submission to the appointing authority for review and signature. 2.3.26. (Added) The Group Commander will inform the Unit Commander when the legal review is complete. The Unit Commander is directly responsible for ensuring members are briefed in writing within 5 days of notification relating to the liability assessment charge against them.

AFMAN23-220_AETCSUP_VANCEAFBSUP 13 JULY 2017 3 2.3.26.1. (Added) The individual will be given an opportunity to make a written statement prior to ROS being sent to the approving authority. 2.3.26.2. (Added) The individual will be informed of their appeal rights IAW AFMAN 23-220, Chapter 17, paragraph 17.3. 2.3.27. (Added) The approving authority will forward the approved ROS case file to the Group Commander and the wing ROS program manager. 2.3.28. (Added) The wing ROS program manager will file and maintain the ROS case files IAW the Records Disposition Schedule (RDS) and the AETC Supplement to AFMAN 23-220. 5.3.10. (Added) The 71FTW standards and unit responsibility for initiating and completing a ROS is 100 days if there is no assessment of financial liability, 130 days if financial liability officer or board is appointed, and 150 days if financial liability is recommended because a legal review is required. AFMAN 23-220, Chapter 5 can be used as a guideline for processing times. Any deviation from this standard will require a written explanation from the respective commander causing the delay to the wing ROS program manager. The letter will be made a part of the ROS case file. 8.4.1. The 71st Flying Training Wing Vice Commander is designated in writing by the 71st Flying Training Wing Commander as the approving authority for Reports of Survey at Vance AFB, OK in accordance with AFMAN 23-220, Chapter 11, paragraph 11.7. 8.4.4. The wing ROS program manager is designated in writing by the 71st Flying Training Wing Commander in accordance with AFMAN 23-220, Chapter 11, paragraph 11.7. as the approving authority for not processing a ROS if the value is under $500 and a ROS is not required. This does not apply to loss, damage, or destruction of government vehicles. 9.1. Group Commanders are designated in writing as appointing authorities for personnel under their command by the 71st Flying Training Wing Commander in accordance with AFMAN 23-220, Chapter 11, paragraph 11.7.. 9.3.6. The 71st Flying Training Wing Commander in accordance with AFMAN 23-220, Chapter 11, paragraph 11.7., has delegated his authority to the appointing authority to take final action on the ROS for normal retail type items used at base level when the amount of the loss or damage is less than $2,000 and there is no evidence of negligence, willful misconduct, or deliberate unauthorized use. 9.3.7. The 71st Mission Support Group Commander has the authority to approve or disapprove DD Forms 200 when the gain or loss of inventory in Supply System Stocks is less than $100,000 as delegated by the 71st Flying Training Wing Commander in accordance with AFMAN 23-220, Chapter 11, paragraph 11.7.. 11.4.12. The ROS program manager will assign a four-digit number, e.g., 09-01. The first two digits will identify the fiscal year and the remaining two digits provide the sequence number of the ROS. This number is provided to the IO during the initial briefing. 11.8.1. (Added) The group ROS monitor and wing ROS program manager will ensure every effort is made to expedite both the completion of a ROS and the collection action prior to departure of separating/pcs personnel.

If the recommendation in Block 9 suggests an individual(s) is financially liable, include rationale for this determination and the following statement: The individual s negligence, willful misconduct or deliberate unauthorized use (whichever applies) applies of government property was the proximate cause for the loss, damage, or destruction (whichever applies). 14.4.2.1. Individuals who wish to voluntarily pay for loss, damaged, or destroyed government property valued at $500 or less and does not require a ROS may do so by contacting their Unit Commander or group ROS monitor to initiate voluntary payment documents, i.e., DD Form 1131, Cash Collection Voucher, or DD Form 362, Statement of Charges/Cash Collection Voucher. A cost amount verification and signature is also required on the DD Form 1131/DD Form 362 from the supporting agency of the item. The following offices apply: For equipment items, individual equipment (flight/deployment clothing) items, Customer Support, Bldg 244; damage to a government/rental vehicle Transportation, Bldg 297. When restitution is complete,71 CPTF/FMF will forward a copy of the DD Form 1131/362 to the wing ROS program manager. 15.1.2.1. The Online Vehicle Interactive Management System (OLVIMS) will be utilized to calculate fair market value and repair costs for vehicles. 18.4.3.7.1. The group ROS monitor, in conjunction with the unit Vehicle Control Officer (VCO), will track the number of vehicle mishaps resulting in ROS actions by case number. Initiate investigation actions immediately upon notification of vehicle mishap. The case file should include: SF Form 91, Motor Vehicle Accident Report, accident release letter, supporting pictures, and accident estimate work order produced by OLVIMS. 20.6. Accountable Officer Signature.
