Population and Primary Health Care delivers a range of primary health care services across the southern metropolitan area of Adelaide, with a focus on providing preventive and health promoting programs in the community, and transition and hospital substitution and avoidance programs targeted at chronic disease and frail aged. SUMMARY OF THE BROAD PURPOSE OF THE POSITION This position is clinically responsible for conducting Occupational Therapy practice, while working under appropriate supervision. Supervision will gradually decrease as
experience increases to a clinically independent level, however always having a direct supervisor who monitors quality and quantity of output. Clinical Services To provide inpatient and outpatient services as outlined in the Occupational Therapy Department Procedures, to promote personal independence. To assess, provide the appropriate therapy and assist in referral and discharge planning of patients to the community and/or other centres, to ensure continuity of patient care. Maintain close clinical liaison with Community or other services associated with FMC patients using: written reports, discharge summaries, formal meetings, and telephone calls. Maintain close liaison with FMC staff in multi-disciplinary teams and services that OT is involved in by attending meetings, report writing and other communications. To keep adequate treatment records to enable review of Department clinical activity. To assist in service monitoring and improvement processes by contributing to meeting and inservice processes Teaching To provide educational services related to OT both inside and outside FMC as directed by the OT Manager or OT Supervisor, as appropriate. Plan and carry out assessment and treatment programmes for allocated patients by defining treatment goals (in consultation with the patient) arising from the assessment procedure to promote maximum functional independence for the patient. 2. Evaluate and modify own assessment and treatment methods to ensure skills are continuously upgraded by requesting direct supervision and/or discussing techniques with the supervisor. 3. Develop professional rapport with patients and staff to ensure effective decision making and goal setting occurs in a team approach with the client's interests being the primary consideration by communicating in the appropriate professional manner. 4. Organise the necessary assessment and treatment resources to ensure the necessary items are easily accessed during sessions with the patient. 5. Ensure work areas are safe and tidy to prevent injury to patients and staff by keeping the work place free of possible causes of injury and clearing work areas directly after use. 6. Contribute to the overall evaluation of the Occupational Therapy service to provide effective patient care by actively participating in case management discussions and treatment evaluation activities with colleagues. 7. 8. Seek advice from Area Supervisor when necessary to clarify assessment, treatment and administrative procedures and to discuss difficult cases by liaising with the Supervisor on a day to day basis. Liaison 10. Liaise with hospital professionals to: a) Ensure the relevant professional is involved by referring patients to them where assessment shows this is required. b) Provide access to community services for patients by referring to the Dom Care and/or RDNS Liaison Officer, and c) Ensure issues relating to patient welfare are given the appropriate consideration by discussing these issues with the relevant professional. 11. Liaise with community resources to ensure the appropriate ongoing care of the patient by communicating with and referring to the relevant agency. 12. Liaise with families/care givers to ensure ongoing appropriate care for the patient by providing support, counselling, and education on patient management and treatment in consultation with Social Work where appropriate. Meetings 13. Attend relevant clinic, ward, team and discharge planning meetings to ensure Occupational Therapy intervention is timely and appropriate and to ensure a coordinated team approach in patient care by actively contributing to discussions relating to the patient. Records and Reports 14. Write up assessment and treatment in order to: a) Provide useful information to the professionals involved. b) Provide a baseline for ongoing assessment and treatment, and c) Ensure continuity for patient care on hand over to team members by documenting information as described in the OT Department's Practice and Work Instruction Manuals 15. Write up discharge summaries to provide information to other agencies for ongoing patient care by following the procedure in the Inpatient Procedure Manual. 16. Departmental records including statistics and timesheets, and time analysis tables to ensure the evaluation of departmental activity by completing the designated forms, as outlined in the department's Policies and Procedures Manual. 17. Report accidents involving personal injury by informing the OT Manager immediately and by completing the relevant Incident Report Form. 18. Report accidents involving patients to the OT Manager by completing the relevant Incident Form and informing the clinical nurse consultant, to ensure the patient receives the necessary medical and nursing care.
Equipment and Stock 19. Assist with the maintenance of equipment by treating equipment with care, using it in the manner that the equipment is designed for, and by notifying the OT Manager regarding malfunctioning of equipment in order to ensure the necessary range of items are available for patient assessment and treatment. 20. Assist with the maintenance of equipment and stock levels by notifying the Occupational Therapy Attendant or Supervisor when these resources are low, in order to ensure the necessary resources are available for patient assessment and treatment. Purchase small items as required and approved by the Supervisor and/or the OT Manager. Quality Assurance and Professional Development 21. Participate in the inservice education programme to ensure own ongoing professional development by presenting talks on areas of interest and of current work area relevant to Occupational Therapy and actively contributing to discussions during educational sessions. 22. Initiate and participate in performance appraisal to assist with ongoing personal professional development by following the guidelines set down by the OT Manager and the F.M.C. Staff Development Unit, in conjunction with the supervisory therapist, twice yearly. 23. Participate in continuing education programmes to improve and broaden knowledge and skills in occupational therapy by attending and contributing to relevant educational activities such as conferences, workshops, seminars and interest groups. 24. Participate in staff meetings in order to facilitate efficient and effective department functioning by actively contributing to agenda items and discussions. 25.
