Bid Type: Request for Qualifications (RFQ 13-006) for Design Build Services for Central Administration Building Due Date: Sealed responses are due in the Edgewood ISD Purchasing Department on Monday, May 13, 2013 at 2:00 p.m. local time. Any submittals received after that will be returned unopened to the originator. At this time the submittals will be opened and the name of the originator only will be read aloud. Packages are to be sealed and clearly labeled Response to RFQ 13-006 for Design Build Services for Central Administration Building. Pre-Conference Meeting: Pre-proposal Conference will be held on Tuesday April 30, 2013 at 10:00 a.m. in the conference room at the Edgewood I.S.D. Central Administration Offices located at 5358 W. Commerce St., San Antonio, Texas 78237. All vendors are encouraged to attend to clarify any questions in response to RFQ. Statement: Edgewood I.S.D. reserves the right to reject any or all bids. The evaluation team will review all proposals and determine which proposal is in the best interest of the District Conflict of Interest: Conflict of Interest Questionnaire (Form CIQ) is required to be submitted by any vendor or other person doing business with a local government entity in accordance with Chapter 176, Local Government Code. Please acknowledge there are no conflicts of interest with any local government officer by completing form located at the end of request for qualifications. 1

Criminal History: Awarded vendor shall certify to Edgewood ISD that any representatives and employees who will be performing services on campuses or will have direct contact with students shall be subject to a criminal history check and shall not have a criminal history which is objectionable to the District. Awarded vendor shall submit appropriate information to Edgewood ISD in compliance with section 22.081 et.seq,. 1. Texas Education Code regarding criminal history of its employees who will have direct contact with students. 2. Submit the Felony Conviction/Debarment notice included in this RFQ with the response. Contract Documents: The School District will use the AIA Document A141 or A142, 2004 Edition, plus any supplementary provisions to the contract. The architect representing the design build firm will be required to carry professional liability insurance and the Builder/Contractor will be required to provide all required insurance and bonds for the project. Please note any exceptions/issues you would raise relative to this document. Professional Liability Insurance: The School District will require all Professional Service Firms selected to maintain professional liability insurance with limits of not less than $1,000,000.00. Rating/Scorecard Criteria: A Rating Score point system of 1-10 points will be used in each category to rate the proposals; 1 point being the lowest and 10 points being the highest. (on separate page) Preliminary General Design and Construction Project Scope (including but not limited to): Work with the Owner to develop spatial needs based on the determined use of the spaces. Prepare complete construction documents. Comply with all required codes, regulations and laws. Secure and pay for all required building permits from the City of San Antonio. Complete the scope of work within the time frames included within this RFQ. Administration Building will be designed to combine all administrative services and personnel for the Edgewood ISD. Currently the administrative personnel are located in separated facilities and it is the intent of Edgewood ISD to combine all central office administration into one location. Based on preliminary considerations, the proposed building will be multi-story and approximately 80,000 square feet. Edgewood ISD is considering several locations, but all locations are within the Edgewood ISD boundary and within the City of San Antonio. The selected design-build firm will be responsible to design a structure in accordance with the Owner s occupant load, design criteria and within the limited space available. Final design and square footage may be adjusted during the design development phase with the Owner s third party Architect. Initial design concerns to be included are parking: a first floor to include: a Wellness Facility, Food Service, Police Room, Board Room, Kitchen, Galley and Records Room. The second floor is anticipated to include office for the Curriculum Staff and the third floor to include offices for the Executive Staff. END OF REQUEST FOR QUALIFICATIONS 6

27. FELONY CONVICTION/DEBARMENT NOTICE State of Texas Legislative Senate Bill No.1 Section 44. 034, Notification of Criminal History, Subsection (a) states a person or business entity that enters into a contract with a school district must give advance notice to the district if the person or an owner or operator of the business entity has been convicted of a felony. The notice must include a general description of the conduct resulting in the conviction of a felony. Subsection (b) states a school district may terminate a contract with a person or business entity if the district determines that the person or business entity failed to give notice as required by Subsection (a) or misrepresented the conduct resulting in conviction. The district must compensate the person or business entity for services performed before the termination of the contract.
