keynote/general sessions THE HEALTH SYSTEM OF THE FUTURE: LEAD, FOLLOW, OR GET OUT OF THE WAY. Dr. Josh Luke National Bundled Payment Collaborative Skilled nursing facility (SNF) and assisted living (AL) owners, administrators and directors of nursing will not want to miss this cutting edge presentation from a best-selling author and one of the leading healthcare futurists in the country. This presentation, led by a former SNF & AL administrator, who also served as a hospital CEO, breaks down which bundled payment models are most relevant to SNF and AL, as well as the most appropriate way to position post-acute care for each bundled payment model. The presentation provides great detail not just on all of the hospital penalty programs, but shares how hospitals and SNFs are being penalized when post-acute care length of stay is excessive and when SNFs routinely discharge to home health. The Care Plan Act will be a major focus, and strategies on how SNFs best align with referral sources and help them avoid penalties, while avoiding (recently announced) SNF penalties for over-utilization as well. If you have never seen this presenter speak, you won t want to miss it as he has been called the Tony Robbins of Healthcare as he is equally entertaining, inspiring and informative! Recommended for all long-term care audiences LABOR & EMPLOYMENT LAW UPDATE Grant Pecor Clark Hill PLC This program will focus on labor and employment trends and current developing issues. With the ever-evolving state and federal labor and employment laws, it is critical that employers keep up-to-date to avoid legal liability. This program will feature practical tips for employers to comply with developing labor and employment laws, including discrimination and accommodation laws and the new NLRB election regulations. Recommended for all long-term care audiences legal/regulatory/quality sessions HCAM REGULATORY AND QUALITY ASSURANCE PERFORMANCE IMPROVEMENT (QAPI) UPDATE Andy Rothman Lebenbom & Rothman, PC Vickie Burlew Lebenbom & Rothman, PC We are facing the largest federal change to our industry since the Omnibus Budget Reconciliation Act (OBRA) in 1990. We expect over 100 new and revised regulations to be finalized this year, payroll-based journal reporting to the Centers for Medicare and Medicaid Services (CMS), rebasing of the 5 Star System, and dementia surveys. Simultaneously, we are challenged with re-organization at the State and changes to state law permitting us to hire physicians. This session organizes the changes and offers suggestions to implement and manage change. Recommended for skilled nursng audiences only ANTIBIOTICS: A LOOK AT THE INCREASED SCRUTINY OF ANTIBIOTIC USE IN NURSING HOMES Bill Vaughan Remedi SeniorCare Rob Shulman Remedi SeniorCare Antibiotic use in nursing homes has come under intense scrutiny due to studies that suggested antibiotics are overused and poorly managed. Misuse of antibiotics can cause profound clinical implications for the residents and adversely affect the regulatory health of the facilities who administer them. This presentation, given by a board certified geriatric pharmacist and the former Chief Nurse of Maryland s state survey agency, will provide an overview of current and proposed federal regulations which impact antibiotic use. Best clinical practices designed to reduce the risks associated with antibiotics will also be explored. Finally, antibiotics stewardship will be discussed from both a clinical and operational perspective. Recommended for skilled nursing audiences only THE SKILLED SHIFT: HOME HEALTH TO HOME CARE & ASSISTED LIVING Dr. Josh Luke National Bundled Payment Collaborative In this session, long-term care providers will learn about Medicare Spending per Beneficiary (MSPB) and its impact on the hospital. Long-term care providers will learn how they can have an impact on the hospital s penalty and how to open doors through the referral sources to effectively manage this process. The specifics of the Care Plan Act and how it impacts MSPB. Finally, this session will discuss the proposed Post-Acute MSPB penalty and how long-term care providers can minimize exposure. Recommended for all long-term care audiences 4

reimbursement/finance sessions MANAGING COSTS WHILE MANAGING CARE Bert Brady Omnicare a CVS Health Company Paulette Najarian-Knight Omnicare a CVS Health Company Medication costs are rising every year, especially as new items come to market. This presentation will compare the benefits and risks in utilizing the newest, most expensive drugs as well as ways to control drug costs so as not to spend more than necessary while providing effective care. Sometimes the increase in drug cost is offset by decrease in other areas like labor, lab tests, etc. Other times, a more costeffective means of treating the resident can be considered. Participants will come away from this presentation with the knowledge to make more advantageous decisions in regards to managing their pharmaceutical costs. Recommended for skilled nursing audiences only PRIVACY BREACHES, LITIGATION AND PROTECTION FOR LONG-TERM CARE AND ASSISTED LIVING PROVIDERS David Wiegel Kitch Drutchas Wagner Valitutti & Sherbrook John Hessburg Kitch Drutchas Wagner Valitutti & Sherbrook Vickey Voss Voss Insurance Services, Inc. This presentation will deal with the growing risk of cyber-related data breaches and related concerns for those caring for the elderly. Examples of such breaches and the ramifications experienced by the facilities will be provided. Relevant litigation and consequences will be discussed as well. Lastly, participants will have the opportunity to ask insurance provider experts questions or share best practices in regards to cyber breaches or mitigation of the risks. Recommended for all long-term care audiences REIMBURSEMENT & FINANCE UPDATE Betsy Rust Plante & Moran PLLC Pat Anderson Health Care Association of Michigan HCAM Executive Vice President for Reimbursement Services, Pat Anderson, and Betsy Rust, reimbursement consultant to the HCAM Board of Directors, will facilitate this session designed to provide a comprehensive overview of the various Medicare bundled payment models, Comprehensive Care for Joint Replacement and managed care arrangements. Explore risk arrangements for these bundled payment models and how to decide if your facility is ready to make this type of contract. Also, the presenters will provide an update on recent changes in Medicare and Medicaid reimbursement and status of the MI Health Link pilot program. Recommended for skilled nursing audiences only skilled nursing focused sessions IMPLICATIONS FOR THE PROPOSED POST ACUTE MEDICARE SPENDING PER BENEFICIARY PENALTY Dr. Josh Luke National Bundled Payment Collaborative The Medicare Payment Advisory Commission has proposed a Post Acute Medicare Spending per Beneficiary penalty which is likely to be introduced by 2017. Learn how this penalty is measured and how skilled nursing facilities (SNF) should immediately be changing behavior to avoid penalty exposure and the impact of this proposal on patients/residents. CMS penalty periods are almost always a three year look back so the time to act is now as SNF s are being measured based on current performance. This session will explore which two key indicators impact a SNF s post acute Medicare Spending per Beneficiary exposure, and how a SNF can begin to address them with relative ease in a short amount of time, to ensure optimal coordinated patient care. Recommended for skilled nursing audiences only MANAGED CARE PANELS Moderated by: Dr. Josh Luke National Bundled Payment Collaborative The provider panel will facilitate this session designed to provide a comprehensive update to members on Michigan s vision for long-term care managed care, both short term and long term. Additionally, a cross section of service providers representing various aspects of the health care continuum will be assembled to share their insights on how the world of managed care continues to affect their strategic planning and processes. The panelists composed of HCAM & MCAL Associate Partner representatives will share how they currently work with managed care organizations related to contracting/payment; closed networks; changes or deviations from traditional practices. A discussion filled with participant questions is not one you want to miss! Recommended for skilled nursing audiences only DISCHARGE PLANNING: UNDERSTANDING THE PROPOSED RULE AND THE IMPACT ON POST-ACUTE PROVIDERS Rebecca Matthews Great Lakes Caring Home Health and Hospice The proposed rule changes for discharge planning are fast approaching. The purpose of this presentation is to provide clarity on these rule changes. Specifically, how discharge planning will change for hospitals and post-acute providers, including its effect on the relationship between skilled nursing facilities and home health agencies. Recommended for skilled nursing audiences only 5

How do you reduce burnout and increase retention without sacrificing quality? Why do some homes gel while others are chaotic? Learn how to shift your hiring paradigm and on-boarding process to have the success necessary to be a star in the industry. Recommended for all long-term care audiences GET IT RIGHT! How to implement a standard workforce and a succession planning model will be shared as well. What exactly are employees looking for in their managers? What are the characteristics of a good manager/leader? This presentation will explore the answers to these questions by looking at two areas: what residents want from their caregivers and what caregivers want from their managers. This presentation will provide an opportunity for the attendee to identify their own managerial style and determine if their style addresses the needs of the residents as well as the caregivers. Attendees will leave this session with the ability to formulate a plan for self improvement. Recommended for all long-term care audiences HOW TO HIRE AND KEEP CAREGIVERS Mary Tellis-Nayak MyInnerView Research has demonstrated that organizations with an engaged workforce outperform organizations who have a lesser engaged workforce. So, what is employee engagement? How does it differ from employee satisfaction and how is it related to motivation and commitment? This presentation will explore each of these concepts and suggest ways in which leaders/managers can support caregivers in their work. We will also explore factors important in the hiring process and behaviors which are important during the early stages of employment.
