Welcome to Sanford Heart Hospital Thank you for choosing Sanford for the care of your heart and vascular needs. We realize that this can be a stressful time and will do everything we can to ensure that you receive exceptional care. Expect Our Calls Before you come to Sanford, a representative from the following areas may contact you: Hospital Registration A representative from pre-registration will call to verify your information and insurance coverage. If your scheduled procedure is within 48 hours and you have not been contacted, please call (605) 328-3100 or (800) 709-4274. Heart and Vascular Coach The Heart and Vascular Coach is a registered nurse who will prepare, educate and communicate with patients and families during their heart and vascular care. If you are arriving for a Cath Lab procedure or surgery, the Heart and Vascular Coach from Sanford Heart Hospital may call you to talk about what time to arrive and to gather your medical information and your medication 3

list, along with information regarding any important tests you have had done. If you have questions or have not received a phone call by the day before your procedure, please call (605) 312-2199 or (855) 263-3540. Pack for Comfort Please bring the following: q Insurance card and information. q All your medications, including over the counter and herbal, in original bottles. q Assistive devices, such as walkers, canes, hearing aids, and magnifying glasses. If you wear contact lenses bring along your contact supplies and/or your glasses. q Loose, comfortable clothing that can be folded for easy storage. q Copy of your advance directives, unless previously provided. Advance directives are given to your doctors and family in case you become unable to make medical decisions for yourself. Advance directives include a living will and durable power of attorney for health care. Any person 18 years of age or older should be informed about advance directives. q This brochure: Welcome Handbook, including your personalized itinerary. Leave the Rest at Home: Valuables such as credit cards, checkbooks, large amounts of cash and jewelry. Please remove all jewelry including all piercings. We cannot be responsible for your belongings. If you must bring valuables, ask our staff to deposit them in the hospital safe. For your convenience, we strongly recommend leaving unneeded items and valuables at home. 4

When you Arrive at Sanford Heart Hospital Arrival When arriving at Sanford, you will enter the Sanford Main Entrance off 18th Street. Sanford Heart Hospital is located on the east side of the U drive. Parking We encourage you to use the complimentary valet parking service, conveniently provided to all patients, families, and visitors. Valet services are available Monday through Friday, 4 a.m. 9 p.m. General parking is available north of the hospital at the intersection of 18th & Grange. Registration Upon entering Sanford Heart Hospital, you will be greeted in our Welcome Center and registered for your stay with us. Your visit with us will require you to wear a hospital wristband. This wristband serves as your identification while you are in the hospital. You will find that this identification is checked frequently while you are in the hospital for your safety. General Information During your Visit My Sanford Chart Information My Sanford Chart puts you in control of your medical care and medical history. Best of all, it is available from the convenience of your computer. Your medical history is important to us and we want to make sure your medical information remains your information. Access your My Sanford Chart at mysanfordchart.org. If you are not already signed up for My Sanford Chart, let us know. We d be glad to assist you. My Sanford Chart will allow you to: View your online medical records Make medical appointments Renew prescriptions Email your care team Receive care on-line from a health care provider through a written e-visit Get test results Access your family s medical records See a health care provider for primary care needs with a video visit 8

ID Badges To assist you in getting to know your care team, each of our staff wears an ID badge that includes picture, name and job title. Staff Uniforms To easily identify who is providing your care and increase patient safety, Sanford has implemented standardized uniforms that differentiate job roles by color. 9

Accommodations Our patient rooms are designed for your family to stay overnight with you during your hospitalization. In addition, our Guest Areas on each level offer public computers, a shower, kitchenette, and lockers to make your stay as comfortable as possible. We welcome your family to use this option; however, if you prefer hotel accommodations, our Patient Access Management representative will assist in making reservations. They may be reached at (605) 333-3064 or (877) 329-1651. For your convenience, Sanford will file all claims to your insurance company and send one bill to you for all of your hospital and clinic services. Please note you may receive a separate bill for other services such as anesthesia, pathology, radiology, and Sanford Laboratories when applicable. We will assist you in understanding and resolving any financial obligations remaining after insurance. If you have any questions or needs regarding insurance billing, self-pay balances, or financial assistance, please contact Patient Financial Services (PFS) at (605) 328-6585 or (877) 629-2999.
