Position Definition Position: Agreement: As relevant to the individuals profession Classification: Dependant on Qualifications & Experience Position reports to: Manager HACC / Community Services Effective date: January 2015 Approved by: Director of Community Services Organisational Context Plenty Valley Community Health (PVCH) provides primary healthcare, social and psychological support services to people of all ages, focusing on the growing outer northern metropolitan area of Melbourne. Our services have a strong focus on treatment, prevention, early identification, rehabilitation and maximisation of independent living skills. PVCH is a major provider of primary healthcare in our region and is committed to establishing a well-integrated primary healthcare service that is accessible to people in the City of Whittlesea and neighbouring metropolitan and rural municipalities. PVCH partners with General Practitioners, Hospitals, the three levels of government, local agencies and community members to achieve an effective universal primary healthcare system. PVCH is a registered Community Health Centre and a Company Limited by Guarantee. It is funded by government grants and other self-managed revenue activities. It is governed by a Board of Directors elected and appointed from the community. Our expectation is that staff will operate as a co-operative team. Our staff will be flexible, multi-skilled and able to work with a diverse range of individuals, multidisciplinary groups and community agencies. Our staff will continually update their knowledge to take on new responsibilities and challenges.

Purpose and Scope The Coordinator position supports the Manager of the service in the overall coordination and delivery of the clinical program areas and in the management of quality and risk across the programs. It supports a multidisciplinary team of health professionals in the HACC/Community services team by providing operational support and monitoring clinical safety, appropriateness, effectiveness and efficiency within the service program areas. The role also assists in the provision of support, supervision and leadership to staff members within the team to assist the service to reach its goals and fulfil service delivery obligations. This role will also have a clinical component requirement attached to it. The Community services component of the team consists of Women s Health and Chronic and Complex care services. The Chronic and complex care service has a strong role in early intervention, treatment and the management of chronic conditions by providing programs that target, in particular, Type 2 diabetes and cardiovascular and respiratory conditions through individual consultation and group based work. The Chronic and Complex Care Team works closely with General Practitioners, specialists, acute and sub-acute services in providing holistic care and chronic disease management. The Chronic and Complex Care Team also provides secondary consultation for individuals identified at risk of developing chronic conditions and will focus on preventing unnecessary hospital presentations and admissions. The team consists of Physiotherapists, Dieticians, Allied Health Assistants, Diabetes Educators, Nurse and Endocrinologists. The Women s Health service provides preventative screening and education services to the community and consists of a team of Nurses. The HACC component of the team consists of Allied Health staff servicing all eligible HACC clients with Services including: Allied Health Assistance, Cardio-respiratory Nursing, Care Coordination, Diabetes Education, Dietetics, Endocrinology, Occupational Therapy, Physiotherapy, Exercise Physiology, Podiatry, Planned Activity groups and Project Coordination. Services from the team are provided at multiple PVCH sites within the City of Whittlesea catchment. The Coordinator will work collaboratively with other Coordinators at PVCH to ensure clinician and team cohesiveness in client management across the organisation. 2 Private & Confidential Plenty Valley Community Health

Key Responsibilities and Duties Service delivery: To meet, and strive to exceed, personal and workgroup targets for Key Performance Indicators as set from time to time by PVCH and/or funding bodies in areas such as Safety, Efficiency and Effectiveness, amongst others. To practise effective infection control procedures Assist in the provision of direct clinical care to PVCH clients in their respective professional discipline consistent with the Practice Areas of the Team. Conduct comprehensive clinical assessments and develop personalised care plans. Support recruitment and HR processes, including orientation of new employees, ongoing supervision and annual performance management of staff. Promote collaboration between clinicians across PVCH service areas and support the further development of inter-professional and across sector partnerships to improve the quality, safety and integration of services. Work collaboratively with the PVCH Senior management team regards service delivery, development and promotion Provide clinical input in professional area of expertise. Key Selection Criteria Mandatory: Successful applicants are required to provide certified copies of the mandatory qualifications listed in this section. A qualification in a relevant health profession with substantial post graduate experience. Eligibility to obtain and maintain professional registration with the Australian Health Practitioners Registration Agency (AHPRA) or other relevant body entitling practice in Victoria and must provide evidence of that registration to PVCH annually. Evidence of staff management/support experience Evidence of experience in utilising advanced professional knowledge in leadership, guidance and direction of staff. Demonstrated experience in the provision of supervision to individuals and groups. Well developed written communication skills and interpersonal and advocacy skills. Demonstrated effective organisational skills and a capacity to work flexibly and within tight timelines. Desirable: Post Graduation qualifications in a relevant discipline. Conditions of Employment Remuneration: The employee will be paid in accordance with the Award/Agreement. Salary Packaging is available for permanent and fixed term employees, in accordance with prevailing legislation and PVCH Policy. Novated Car Leasing is available for all permanent employees. Location: The employee will be expected to work, either temporarily or permanently, at any site operated by PVCH. Hours of duty: The hours of duty will be as rostered by the employee s Supervisor. Pre-employment checks: A Police Check is mandatory. In accordance with PVCH Policy, relevant convictions will be taken into consideration when considering applicants for this position. All employees of PVCH must be either an Australian Citizen, Permanent Resident or have an appropriate Australian visa that will legally enable them to fulfil the obligations of the contract of employment. A Working With Children card (WWC) is mandatory for this position. Failure to obtain and retain a WWC card will make the employee automatically ineligible for employment in this position. Probationary period: Confirmation of new employees to PVCH is subject to a 6 month probationary period, during which time the employee must demonstrate satisfactory capability to perform the Key Responsibilities and Duties. This probationary period is regarded as an extension of the interview process.
