Finishing Strong and Planning for Sustainability Follow-up with teams with missing data through March 31, 2018, when current data system will be closed Transition to sustainability data system on April 1, 2018, pending availability of sustainability reports 2018 data can be entered at this time 88% of active teams have submitted all 2017 data!

Checklist For Teams to Complete Golden Hour Initiative Submit ILPQC Golden Hour Data Form data through December 2017 by March 15, 2018 in REDCap Facilitate completion of education with all providers and nurses Review data for 3 key process measures with your team Connect with your Perinatal Network Administrator if you are not yet at the 80% debrief, delayed cord clamping, or temperature goal Develop sustainability plan with your QI team (draft plan provided by ILPQC), submit to your Perinatal Network Administrator by April 1, 2018 Continue to collect / submit data on sustainability measures for compliance monitoring. Compliance data form and reports active on April 1 for entry of 2018 data

Supporting Teams to Sustain Improvement QI Support for teams ILPQC will review measures on a quarterly basis to identify teams not at goals for outreach Perinatal Network Administrators will connect with teams still working towards GH Goals

ILPQC QI GH Commendations All teams that (1) submit all data through December 2017 by March 15, 2018, and (2) meet or exceed the 80% delivery room debrief, 80% delayed cord clamping, and 80% temperature goals by December 2018 will receive (when both criteria are met) an ILPQC QI Excellence Award Certificate and Letter of Commendation to their hospital administration congratulating them for achievements for the Golden Hour Initiative! Reviews of data for Certificates/Letters will be done quarterly for the previous based on 2/3 months for quarter at or above goals. Certificates/Letters will be sent out approximately 2-3 months following the completion of the quarter.

IMPORTANT: Transfer to Sustainability Data Form AS OF APRIL 1, 2018: The ILPQC Golden Hour REDCap data form will close. Teams will not be able to access this form Teams will have access to ILPQC Golden Hour Sustainability REDCap data form. Access will be transferred automatically for teams Any data that has been submitted for 2018 will be transferred into new data system

Paper GH Data Form ILPQC created a GH sustainability paper data form for teams to use Access to download data form on webinar and ilpqc.org

GH Sustainability Plan Has your team created a draft Golden Hour sustainability plan? Make sure to share sustainability plan with your hospital s Perinatal Network Administrator Opportunities to discuss at perinatal network meetings

Team Spotlight: Example of GH Sustainability Plan Advocate Good Samaritan has created a GH sustainability plan & shared with their Network Administrator How was the process for creating the sustainability plan? What are the strategies for operationalizing sustainability plan? How will measures be collected? How are new hires receiving education? GH education and clinical care policies and protocols into hospital new hire education? What education tools will you use for ongoing education? Any other teams have a draft sustainability plan they d like to speak about?
