Thank you for your interest in volunteering at AMITA Health! We have many volunteer opportunities at our Bolingbrook, GlenOaks, Hinsdale and La Grange campuses, as well as at our off-site locations. If you are compassionate, willing to help others and ready to take on a new adventure, we need you! In this packet you will find all the information needed to make an informed decision about volunteering. We hope that upon reviewing it, you will be excited about joining our team and will choose to attend our next Volunteer Orientation. Also included in the packet is everything you will need to get started on becoming a volunteer. This is designed to shorten the length of time between applying, and actually beginning your volunteer position. You can choose to do as much or as little before Orientation as you like. The more you complete, the sooner you can start after Orientation. Again, thank you. AMITA Health Volunteer Requirements Volunteering at AMITA Health is a commitment. Anyone giving service at our hospitals must meet stringent requirements in order to be considered for placement, even our Volunteers! It s part of our ongoing commitment to excellence in patient safety, quality and compassion. To be considered for volunteering at AMITA Health, you must meet the following requirements: Be at least 16 years of age Be compassionate and willing to help others Undergo a background check (the hospital will conduct this free of charge) Attend Volunteer Orientation Complete a 2-step TB test (through the hospital free of charge, or from another provider if you prefer) Show proof of immunization for measles, mumps, rubella (MMR) and varicella (Chicken Pox). If not available, receive titers blood draw through the hospital (free of charge). Note: If born before 1957, you are exempt from titers blood draw. Receive an annual flu shot (through the hospital free of charge, or from another provider) Complete initial and annual safety / infection control education Purchase the assigned uniform and wear it when on duty (uniformed positions only) Wear your assigned AMITA Health ID badge Agree to abide by all hospital policies, regulations and standards of behavior If you are ready to take the next steps toward volunteering, please read on! Getting Started Check It Out! Interested in volunteering? The first thing to do is read this packet thoroughly and decide if volunteering at AMITA Health sounds like a good fit for you. If it does (and we hope it does!) please complete the following requirements to become an AMITA Health Volunteer. Once you have submitted your application, the requirements can be completed in any order. All accompanying documentation must be turned in to the Volunteer Services office before your volunteer service can begin. You may return the completed documentation to the Volunteer Services office by: 1. Bringing them to the Volunteer Services office at your campus 2. Bringing them with you to Volunteer Orientation 3. Faxing them to the Volunteer Services office at your campus (see p. 4 or 19 for fax numbers) Apply to Volunteer How to Apply (choose from these options): 1. You can apply online (click here) or visit https://www.keepingyouwell.com/about-us/volunteer 2. You can apply by completing the application in this packet. After completing, return the application to Volunteer Services Department at any campus. Sign Up / Attend Volunteer Orientation After your application is received, you will be contacted by email to schedule your Volunteer Orientation session. In that email, a schedule of Orientation sessions available will be included, and you may sign up for any location, regardless of where you will volunteer. It is up to you to sign up for and attend an Orientation session; you may not volunteer until you have completed Orientation. During the time between application and your Orientation session, you may complete as many of the remaining requirements as you are able; this will shorten the time it takes to become an active volunteer. References In order to volunteer, you will need two references from non-family members. You will find the reference forms near the back of this packet (p. 17-18). Give one each to a person who can comment on your ability to volunteer. 5 P age
TB (Tuberculosis) Screening You will need to complete a 2-step TB (tuberculosis) screening in order to volunteer at AMITA. This can be obtained through our Employee Health service, or from your own physician. You may choose to complete this requirement before Orientation, or wait until after, but you must complete all four TB testing steps before the first shift of your volunteer position. Note: If you have previously tested positive on a TB test, please contact Employee Health or your physician for alternatives. Please note: Getting a TB test is a 4-step process. It works like this: 1. Receive TB screening 2. Return 2-3 days later for visual examination of the screening site 3. One week later, return for repeat of TB screening 4. Return 2-3 days later for visual examination of the screening site NOTE: Failure to complete any of the steps will cause you to start all over at #1! Just tell the Employee Health nurse at which campus you will be volunteering. To obtain your TB screening from your own doctor: Call your doctor s office for an appointment. Please inform your doctor s office staff that you need a 2-step TB screening. You must complete the same steps listed above, regardless of where you receive your TB screening. Proof of Immunization If you were born after 1957, you must provide us with proof of immunizations. This can often be obtained from your personal physician office or school. If these records are unavailable to you, please contact your Volunteer Coordinator for alternatives. 6 P age
1:1 Interview with your Volunteer Coordinator You must attend a 1:1 interview with your Volunteer Coordinator. At this interview, you and your Coordinator will agree on your assignment, and the Coordinator will answer any questions you may have. The interview scheduling process varies by campus; contact your Coordinator for details. Purchase Your Uniform You will need to purchase your uniform before beginning your volunteer work. This should be done after your job assignment has been designated, due to the fact that some volunteer positions do not require a uniform, or may require a job-specific uniform. Your uniforms may be purchased as follows: Bolingbrook and GlenOaks from the Volunteer Services office by cash or check Hinsdale and La Grange from the Gift Shop by cash, check or credit / debit card Cost: Jacket or Vest $18, Polo $15 (tax may apply) NOTE: If the cost of your uniform will prevent you from volunteering, please speak to your Volunteer Coordinator; we can help! Flu Shot (winter months) For the protection of our patients, their families, our staff and our Volunteer Team, if you volunteer during Flu Season (November 1-March 31), you will be required to obtain a flu shot. This can be done free of charge through AMITA s Employee Health Services, or from another provider. Documentation of the flu shot must be on file in the Volunteer Services office in order to volunteer from November 1 to March 31. If you are unable to receive a flu shot for physical or religious reasons, you will need to wear a mask at all times while on duty during this date range. For more details, please see your Volunteer Coordinator. Whew! That s a lot of Requirements! Yes. It is.
