PRESIDENT'S BOOK REQUIREMENTS FOR SUBMISSION Name: Club: PRESIDENT Turn in President Mini Manual and required forms in an appropriate binder or folder as stated below to the Extension Office annually by August 31. Minimum requirements must be completed for the book to be accepted for banner club or officer book competition. 1) President book must be submitted in a 3 ring binder or a folder with 3 brackets. No Loose Papers! (may be computer generated) 4) Must provide completed 4 H Club Meeting Agenda Guide page for every meeting (manual page 7) 5) Must complete a Parliamentary Procedure Terms activity page (manual page 12) 6) Must submit a list of all guests that attended meetings by their name and date. 7) Requirements for Submission page (Name, Club, # of Members, and # of 8) All records must be either handwritten, preferably in ink or typed by the 9) All work should be done by the member, unless a special needs child requires adult assistance JUDGING SCALE FOR OUTSTANDING PRESIDENT BOOK AWARD * Minimum requirement completed 10 points * List/explain one motion and one new skill practiced * At least three examples of delegated responsibilities to other club members (w/ description) * Neatness (including grammar & spelling) TOTAL /2

SECRETARY'S BOOK REQUIREMENTS FOR SUBMISSION Name: Club: Secretary Turn in Secretary's Mini Manual and required forms in an appropriate binder or folder as stated below to the Extension Office annually by August 31. Minimum requirements must be completed for the book to be accepted for banner club or officer book competition. 1) Secretary's book must be submitted in a 3 ring binder or a folder with 3 brackets. No Loose Papers! (may be computer generated) 4) Note pages (from every meeting) are handwritten (can be written on a typed agenda or may use the form provided) and the minutes from every meeting may be handwritten or typed. 5) Completed "Club Roll" for all meetings 6) Completed "Meeting Minutes Form" for each meeting (manual page 8, or generate own notes with same information/same title to replace page 8) 7) Secretary and President signatures on each meeting's minutes (Meeting Minutes Form) 8) Requirements for Submission page (Name, Club, # of Members, and # of 9) All records must be either handwritten, preferably in ink or typed by the 10) All work should be done by the member, unless a special needs child requires adult assistance. HEALTH AND SAFETY OFFICER'S BOOK REQUIREMENTS FOR SUBMISSION Name(s): Club: Health Chairman Turn in Health & Safety Officer Mini Manual and required forms in an appropriate binder or folder as stated below to the Extension Office annually by August 31. Minimum requirements must be completed for the book to be accepted for banner club or officer book competition. 1) Health Chairman's book must be submitted in a 3 ring binder or a folder with 3 Brackets. No Loose Papers! (may be computer generated) 4) Documentation of members participating in the club health theme activity at a club meeting (i.e., minutes w/ names, sign in sheet) 5) List of all health and safety talks 6) Requirements for Submission page (Name, Club, # of Members, and # of 7) All records must be either handwritten, preferably in ink or typed by the 8) All work should be done by the member, unless a special needs child JUDGING SCALE FOR OUTSTANDING HEALTH CHAIRMAN BOOK AWARD * Minimum requirement completed * All Health and Safety Talk scripts (written or typed) * Additional Safety Activity Record 10 points * Neatness (including grammar & spelling) TOTAL /2

RECREATION LEADER'S BOOK REQUIREMENTS FOR SUBMISSION Name: Club: Recreation Leader Turn in Recreation Leader Mini Manual and required forms in an appropriate binder or folder as stated below to the Extension Office annually by August 31. Minimum requirements must be completed for the book to be accepted for banner club or officer book competition. 1) Recreation Leader's book must be submitted in a 3 ring binder or a folder with 3 brackets. No Loose Papers! (maybe computer generated) 4) Include a list of all club recreation activities recorded on the Recreation Record Sheets (manual pages 6 & 7) 5) Requirements for Submission page (Name, Club, # of Members, and # of 6) All records must be either handwritten, preferably in ink or typed by the 7) All work should be done by the member, unless a special needs child JUDGING SCALE FOR OUTSTANDING RECREATION LEADER BOOK AWARD * Minimum requirement completed * Special events, picnics, activities led by the recreation leader (recorded in manual) * Neatness (including grammar & spelling) * Creativity and extra effort of presentation (i.e. scrapbook with pictures, articles, extra pages, etc.) 10 points TOTAL /2

HISTORIAN'S BOOK REQUIREMENTS FOR SUBMISSION Name: Club: Historian Turn in Historian Mini Manual and required forms in an appropriate binder or folder as stated below to the Extension Office annually by August 31. Minimum requirements must be completed for the book to be accepted for banner club or officer book competition. 1) All historian's records/materials must be bound in a binder. No Loose papers including the mini manual! (may be computer generated) 4) Requirements for submission page (Name, Club, # of Members, and # of 5) Pictures should list names of members, advisors, etc. (manual page 6) 6) All records must be either handwritten, preferably in ink or typed by the 7) All work should be done by the member, unless a special needs child JUDGING SCALE FOR OUTSTANDING HISTORIAN BOOK AWARD * Minimum requirement completed * Member involvement in activities pictures & stories * Listing of projects completed (fundraising, community service, family outings, etc.) * Creativity and extra effort of presentation * Neatness (including grammar and spelling) TOTAL /2

Minimum requirements must be completed for the book to be accepted for banner club or officer book competition. 1) News Reporter book must be submitted in a 3 ring binder or a folder with 3 brackets. No Loose Papers!
