El Ba ad is situated in a most remote area of Middle Shabelle region whose capital is Jowhar. Currently, no any health facility exists in El Ba ad as patients including those in emergency conditions should travel either to Mogadishu or Jowar. El-Ba ad is far from Jawhar by 210 kilometres and from the Somalia s capital Mogadishu by 240 kilometres. The nearest health facility is situated in El Dhair at a minimum distance of 60 kilometres, But there is no any road infrastructure which connects El Ba ad to El Dhair. Beside the remoteness, other problem of the district include lack of paved roads connecting it to either Jowhar or Mogadishu and transportation problems (public or private) as well. The traffic roads in the district are very rough and sandy and due to this only 4x4 vehicles have access to the district. El-Ba ad, due to its remoteness as a locality in the region, of its unavailability of good roads and pathways between El-Ba ad and Mogadishu, and El-Ba ad and Jawhar and inaccessibility of health services for medical relief, in the event of the occurrence of any emergency or health crisis. Furthermore, there is no public and private transportation in the area that can rescue patients who are in emergency situations except the use of the traditional transportation camels and donkeys, which does not meet the required purpose with respect to the smooth comfort of the patient and his quick access to a hospital treatment. This type of transportation makes impossible to have a quick access to hospitals for patients who need lifesaving operations or treatments. It is worth to mention that almost all women with complicated labor die on the road en-route to Mogadishu because of such long and difficult journeys. It is to be mentioned in the context however, that if a patient s need arises for a referral to Mogadishu, the patient is required to order vehicle from Mogadishu with the inability of the population to afford their fares, knowing that the distance between El Ba ad and Mogadishu is more than 240 kilometers. The survival rate in this case of the sick or the injured is understandable more worse still when it relates to difficult emergency delivery and worst still if the case is of a deadly epidemics which also require immediate action. It is to be noted in the context as well that in the year 2007, it was recorded in the area 400 death cases within just four days because of the cholera disease as patients were unable to reach the hospitals because of the aforementioned reasons. The result of such human catastrophe is displayed in the photos (see attached). UNICEF, the Red Crescent and the Red Cross, provide some services to the people in the urban areas especially Mogadishu, Jawhar and the cities which are located on the main roads however, these organizations do not provide its services to those remote areas such as El Ba ad either for security reasons or lack of resources available to them. As a consequence of this disparity, the population of El-Ba ad had suffered during the rule of the previous governments and still suffering the scourge of underdevelopment, negligence, unavailability of educational, social and health services, lack of water etc. The wells are far from each other by more than a hundred miles, the roads are not paved, and there are no educational schools except for the Holy Quran Schools that teach by using traditional methods. It is to be mentioned that Hiigsi Relief & development organisation has been successful in the accomplishment of several primary schools in the villages of the district and its towns, such as Ad-Adai, Adan Lihay and El-ba ad in the previous three years. Hence, based of such experience and results obtained in the field of education, the board decided to establish a small hospital or clinic in the town of El-Ba ad which is the most populated compared to the other towns, since its population is more than 9000 people. It is envisaged that the inhabitants of the other villages of the surrounding areas with a total population of 30000 will also be the direct beneficiaries of the hospital. In order to change this deplorable and unfortunate situation, the Somali in Europe who appertain to the region decided to undertake a programme of fundraising to extend and to provide all possible assistance to the residents of this area affected, in adherence to our faith as states:"..and if anyone saved a life, it would be as if he saved the life of all mankind". The implementation of this programme is being spearheaded by Hiigsi, a registered Association in Britain on 25-02-2009 as a none profit company with the no. 06829582,and registered as a Charity on 29 July 2015 with the no 1162909. We, as an official Association, are committed to provide support to the population of these areas to minimize their sufferance. Therefore, we at Hiigsi seek the help and support of the Somali diaspora, all donors agencies, national, international NGOs and other philanthropists in order to achieve our targets. The implementation of the project is planned in three consecutive phases: It consisted of planning and raising funds in order to build the hospital in El-Ba ad. This phase consists of the following: a) b) c) Gathering of all preliminary information in coordination with the inhabitants of the area to overcome the obstacles that might hinder the implementation of the project at the local level. Hiring one of the competent engineers in the country to develop the engineering design of the hospital and its estimated cost. The original total preliminary cost for the construction of the hospital is estimated at US$ 432,975.45. However when we reviewed the cost we realized this cost is unachievable. Due to this we hired another engineer to redesign the buildings at a lower price at US$ 243944.00. Find the evaluation of final costs in the annexes. Start of collection of donations and raising funds from various sources. Establishment of the Association s Account Number: 21624547 with its Sort Code: 400632 at the HSBC Bank under Hiigsi Relief & Development organisation as its account holder. a) b) Provision to the hospital with all necessary furniture and office tools. Provision of therapeutic, surgery, obstetric, laboratory tools, x-ray machine as well as other items such as generators, ambulance and other supplies. This phase is the most difficult phase. a) b) c) It is about running the hospital and implementing the medical services. Stipends of hospital staff, nurses, midwives and doctors and others as well as the pharmaceuticals. It is prospected to establish a district hospital at El Ba ad District with inpatient and outpatient medical care for its residents and those of the surrounding satellite villages with facilities of laboratory x-ray and operational services. The inside infrastructure is composed of the male medical section which consists of two inpatient wards, a doctor s room, an OPD with inbuilt W.C, pharmacy, store, admin office and attached to it is the reception room. Running parallel to it and at the opposite side, but separated by a wide corridor, is the female medical ward consisting of two inpatient wards, a doctor s room, delivery room, post- partum room with inbuilt facilities of W.C. and attached to it is the emergency room. Along side of the medical building, there is the laboratory and the x-ray departments building. Further to it, there is the staff meeting hall and attached to it is the kitchen. In the same compound, there is the duty doctors residence bed rooms consisting of one bed room cum canteen, veranda and WC for the obstetric/gynecologist doctor and along side to it there is the bed room cum canteen, veranda and WC for the general practitioner doctor. The compound within which is base the complex infrastructure of the hospital is surrounded by a three meters height of concrete wall with inbuilt two iron gates for the entrance and exit both for the staff and the patients.
