LETTER OF INVITATION It gives me great pleasure to invite you to the second ANZSPM Medical and Surgical Update for Palliative Medicine. Following the success of 2011 inaugural meeting we would like to continue the tradition of an interactive style meeting which will provide state of the art updates for areas relevant to doctors and nurse practitioners who work in, or have an interest in, Palliative Medicine. This meeting has a broad range of topics with high quality speakers, and promises to be a valuable learning experience for all attendees. I look forward to seeing you there. Assoc. This includes invited speakers, chairpersons and all other delegates. Please read the information in this booklet carefully before completing the form. Registration is open for Palliative Medicine Specialists & Trainees and any GPs, Specialists and Nurse Practitioners with an interest in Palliative Medicine. You can register online (www.anzspm.org.au/update2013) and click on the registration link or you can complete the registration form at the end of this document. We ask that you complete one form for each person attending the meeting. Please print carefully if filling out the hard copy registration form, and keep a photocopy for your records. PAYMENT OF FEES Payment of fees must accompany all registrations and may be made by cheque, credit card (MasterCard or Visa only) or direct deposit. Cheques are to be made payable to ANZSPM Events. ACKNOWLEDGMENTS Your registration will be acknowledged in writing with confirmation of your requirements according to your registration form. CANCELLATIONS AND REFUNDS Cancellations must be notified in writing to the Conference secretariat. Registration Cancellations received: ** Before 24 May will incur a $110.00 cancellation fee. ** Before 10 June will receive a 50% refund of fees paid. ** After 10 June there will be no refund. INSURANCE The Conference organising committee strongly recommends that you take out insurance for your travel and attendance at the Conference. The Conference cannot accept any responsibility for participants failing to arrange their own insurance.
