Psitin Objective The AOD Clinician Assessment Care & Recvery and Cunsellr wrks in crdinatin with all AOD service streams and with related sectrs, t facilitate seamless and integrated treatment pathways fr clients. The AOD Clinician Assessment Care & Recvery and Cunsellr plays a critical rle in clients transitins thrugh the AOD service phases, frm intake and assessment thrugh t engagement with treatment and lnger term recvery services, wrking clsely with internal and external service prviders t ensure a smth transitin and assist clients t remain n their treatment plan. The AOD Clinician Assessment Care & Recvery and Cunsellr wrks with cmplex clients t develp hlistic, lng term recvery plans fcusing n recvery frm prblematic substance abuse and prmting health and wellbeing and scial cnnectedness, achieving utcmes that will sustain psitive and nging behaviur change. The AOD Clinician Assessment Care & Recvery and Cunsellr reduces AOD-related harms t individuals and their families by prviding a range f cunselling interventins t assist clients and their family members t change harmful behaviurs and increase their capacity t effectively manage the difficulties they are experiencing that are assciated with prblematic AOD use and that prmte recvery. Ultimately this rle will assist in the prvisin f skills t prevent relapse and supprt the acquisitin f knwledge, skills and mtivatin t initiate and maintain change and imprve quality f life. Rle Respnsibilities: Assessment Prvide high quality engagement, assessment and supprt services t individuals and their families, via hlistic assessment, apprpriate referral and evidence-based, brief interventins using best practice principles including Family Inclusive and Dual Diagnsis practices; Cnduct cmprehensive assessments including: Ascertaining infrmatin n substance abuse, histry, mental health and risk; Determining if a client requires a standard r cmplex treatment respnse; Utilising the mandated AOD assessment tl; Arranging further assessments and secndary cnsults if required; Presenting assessments at daily clinical review, making recmmendatins n risk, pririty and treatment plans. Cntribute effectively t clinical review; Care & Recvery Prvide pre-care supprt fr clients until their engagement with services cmmences. Case manage cmplex clients, facilitate, crdinate and supprt clients thrughut the life cycle f their treatment plan. Utilise the Drug & Alchl Outcme Star and/r ther utcme measure t identify gals, track and mnitr prgress acrss a range f recvery dmains and prvide clients with tangible evidence f the effectiveness f their treatments. Wrk with clients and their families t develp hlistic discharge plans that supprt meaningful scial and ecnmic participatin and ther practical aspects f recvery. Prvide pst care supprt including assisting clients and their families t negtiate their treatment jurney beynd the AOD sectr as required. Cunselling Develp treatment plans and prvide effective cunselling interventins t meet the individual needs f clients and their families, including thse with cmplex needs. These interventins may include: Assisting clients t identify their substance use triggers and high risk situatins Assisting clients t develp practical skills such as prblem-slving, gal-setting and assertive cmmunicatin Assisting clients t develp insight int their thinking and behaviur in rder t help them make and sustain psitive changes Assisting clients t understand the relatinship between their substance use and ther areas f their lives, such as mental and physical health, relatinships and lifestyles Assisting clients t access a range f cmmunity services and supprts t prmte recvery. Ability t cllabrate effectively with a range f peple (including specific grups such as Abriginal, CALD and Frensic clients) and families experiencing prblematic alchl and ther drugs use in a respectful and cmpassinate manner. 6. Prven ability t develp and cllabrate effectively with service prviders and stakehlders. 7.
