The Standard for SWEP first registration, prescriber upgrade, recredentialing and initial assessment of prescriber level for experienced clinicians Summary The State-Wide Equipment Program (SWEP) offers eligible prescribers the opportunity to initially register as a Green prescriber, upgrade or re-credential their prescriber status, or have their prescriber status as an experienced clinician, assigned for the first time. This standard provides an objective means of structuring, measuring and authorizing the prescriber to; register as a Green prescriber (inexperienced or new graduate clinician) upgrade their prescriber status to Amber or Red in the designated assistive technology (AT) category re-credential at their current prescriber level in the designated AT category every 5 years from their anniversary date have their prescriber status assigned for the first time based on a skills recognition process add additional AT categories to their registration Does this standard apply to me? This standard applies to all eligible clinicians seeking to register for the first time as a green prescriber all registered prescribers seeking to upgrade their prescriber level or to re-credential for the same prescriber level experienced clinicians who are having their prescriber status assigned for the first time: applicants with considerable experience prescribing outside of SWEP e.g. TAC, DVA, interstate, overseas. existing prescribers wishing to add additional AT categories to their SWEP Prescriber Registration What is the SWEP prescriber required to do? First registration as a green prescriber (inexperienced / new graduate clinicians) At first registration, all inexperienced prescribers will be registered as Green prescribers in the nominated AT category(s). To register you must successfully meet all the standards of first registration. This includes meeting eligibility criteria related to threshold credentials for the AT category and completing the induction and training module at https://swep.bhs.org.au/swep-orientation.php. First registration for inexperienced prescribers can be completed at https://swep.bhs.org.au/register.php. Your application to be credentialed as a green prescriber will require you to submit evidence to show that you meet the minimum requirements for your discipline group. Refer to Table 1 for details. 2 P a g e 2017

Occupational Therapists, Physiotherapists, Lymphoedema Therapists, Nurses, Orthotists, Podiatrists, Speech Pathologists & Oxygen Prescribers https://swep.bhs.org.au/swep-orientation.php Lymphoedema Therapists must also complete the Level 1 Australian Lymphoedema Association course Meets eligibility criteria related to threshold credentials for the AT category (refer to Appendix 1) All other applications: upgrade, re-credentialing and or initial assignment of prescriber status for experienced clinicians Depending on whether you are applying for an upgrade to your prescriber level in an AT category, applying to be re-credentialed at the same prescriber level in an AT category, adding a new AT category to your registration or applying for your prescriber level to be assessed for the first time in an AT category, you must successfully meet all the standards of a selected pathway and submit an application form which includes a declaration against the standard and a continuing professional development (CPD) point summary. Figure 1 outlines the process for prescribers seeking upgrade, re-credentialing or initial assignment of prescriber status. The application form including a CPD point summary and declaration is available on the SWEP website via the registration page or prescriber portal located here https://swep.bhs.org.au/account.php, and is included as Appendix 3. 3 P a g e 2017

APPLICATION TYPE 1: upgrade prescriber level or re-credential at same prescriber level (Table 2) APPLICATION TYPE 2: initial assessment of prescriber level based on a skills recognition process for experienced clinicians or adding new AT categories to existing registration (Table 3). Which PATHWAY are you using for this AT category? What are you eligible for? (refer to Tables 2 or 3) Which PATHWAY best meets your CPD plans or completed activities? Have you met all the standards stipulated by the PATHWAY? (Minimum of 15 Category 1 CPD points required, refer to Table 4) Maintain a portfolio that documents all prescriber CPD points undertaken, including documentary evidence and a record of learning aims Participate in activities that contribute directly to maintaining and improving your competence in the scope of your prescriber status 3. Experience and currency prescribing in the AT category 2. Threshold credentials for the AT category PATHWAY 1: Initial assessment of prescriber level based on a skills recognition process for experienced clinicians Minimum requirements for the pathway a) Provide evidence in a CV that demonstrates assistive technology prescribing outside of the SWEP system in other contexts such as TAC, DVA, Private practice or, Overseas. (Please ensure your CV includes detail of your roles and responsibilities related to AT prescription) AND b) Evidence over previous 5 years of practice regularly prescribing in an equivalent category OR if you are applying to add additional AT categories to your existing registration c) Submit a statement to SWEP (as part of Appendix 3) detailing your relevant skills and experience in this AT category Meet eligibility criteria related to threshold credentials for the AT category they are applying for (refer to Appendix 1) Lymphoedema Therapists must complete an ALA course (Level 1 for green/ amber, or Level 2 course for red, or recognised equivalent) and be eligible for Category 1 practitioner registration with the ALA National Lymphoedema Practitioners Register (NLPR) 3. Complete SWEP Induction training All applicants Please refer to https://swep.bhs.org.au/swep-orientation.php 4. Accrue 30 prescriber CPD points Demonstrate an accrual of a minimum of 30 prescriber CPD points over the previous 5 years of practice, which cover the breadth of the assistive technology categories being requested. (Minimum of 15 Category 1 CPD points required, refer to Table 4) Except Orthotists/ Prosthetists, Oxygen Prescribers or Lymphoedema Therapists (Category 1 registered practitioners only) 9 P a g e 2017

a. Orthotists required to provide proof of full membership with AOPA b. Oxygen Prescribers required to provide AHPRA registration and evidence of prescriber requirements as defined in The Framework, Appendix 3 for AT Categories 21 & 22. c. Lymphoedema Therapists provide proof of Level 1 practitioner registration with the ALA National Lymphoedema Practitioners Register (NLPR). If not registered with NLPR provide evidence of 30 CPD points Maintain a portfolio that documents all prescriber CPD points undertaken, including documentary evidence Participate in activities that contribute directly to maintaining and improving your competence in the scope of your prescriber status 5. Prescription review process Participate in the SWEP prescription review process as determined by SWEP 6. Audits Participate in audits on request 10 P a g e 2017

What does this mean for me? When you apply for registration as a Green prescriber you will need to meet the minimum requirements of the standard for first registration as a green prescriber (inexperienced clinicians). This can be done at https://swep.bhs.org.au/register.php. When you apply for an upgrade of your prescriber level or to re-credential at the same level, in the assistive technology category, you will need to meet this standard for each of the assistive technology categories you are applying for and submit a summary of prescriber CPD activity and make a declaration against the standard at the time. Your compliance with this standard may be audited from time to time to ensure adherence. Appendix 2 outlines the CPD activity types and rules that relate to your application. Evidence You should maintain records of your prescriber CPD activity for five years in case you get audited. If you are required to provide SWEP with documentary evidence of your prescriber CPD activities, your personal portfolio will help demonstrate that you have met prescriber CPD requirements. A template to be used to keep a summary of your portfolio is included as Appendix 4. Are there exemptions to this standard? SWEP may grant an exemption, variation or extension from this standard in exceptional circumstances that results in a substantial absence from practice such as; serious certified illness or bereavement with incapacity to work for 4 months or more (5 category 2 CPD points) maternity leave 12 months dispensation (10 category 2 CPD points) extended overseas travel 6 months or more (5 category 2 CPD points) On application for an upgrade to prescriber status or re-credentialing application, prescribers may qualify for their CPD point requirements to be reduced, providing they provide a statutory declaration and/ or supporting evidence. Can I apply for special consideration? If you believe you possess the appropriate level of skill and competence to prescribe AT, but do not meet the required criteria of any of the defined pathways you may apply for special consideration directly to the Chief Allied Health Officer at swepcaho@bhs.org.au. Right of Appeal Any prescriber may appeal to have decisions made by SWEP reviewed, by contacting the SWEP Chief Allied Health Officer on swepcaho@bhs.org.au. However, after the appeal has been reviewed, the outcome of the appeal is final. What happens if I don t meet this standard? Possible consequences for not meeting this standard include; your request to have your prescriber level upgraded will be denied your request to be re-credentialed in the assistive technology category will be denied additional measures to monitor your prescriber performance will be implemented such as prescription reviews your prescriber level may be revised Page 11

Appendix 2: CPD activity guidelines Different values have been placed on CPD activity types with a maximum number of points per activity type that can be accrued. A total of at least 30 CPD prescriber points must be accrued which cover the breadth of the assistive technology categories for which you are applying. Appendix 4: Prescriber continuing professional development, summary portfolio The following are for your personal use and only need to be provided to SWEP in the event an audit is requested. If you are applying for an upgrade to your prescriber level, re-credentialing at the same prescriber level or applying for an initial assessment of your prescriber status as an experienced clinician, you will need to keep evidence of your professional development activities, so you can complete your application form. This template can be used to plan and record a summary of your prescriber CPD activities. If you are required to provide SWEP with documentary evidence of your prescriber CPD activities, your CPD summary portfolio together with and other documentary evidence, will help demonstrate that you have met the requirements outlined in the standard. You should maintain records of your prescriber CPD activity for five years in case you get audited.
