INNOVATE The Annual Perioperative Surgical Home Summit introduces physicians, health care administrators and care team members to the innovative Perioperative Surgical Home (PSH) model of care. PSH is a patient-centered, physician-led system of coordinated care that guides patients through the entire surgical experience. From the decision for surgery to discharge from a medical facility and beyond, the PSH model of care is structured to improve patient care and outcomes. This two-day Summit offers a more in-depth look at the PSH model and includes didactic lectures, roundtable and panel discussions, encouraging collaboration and discussion about the application of concepts. Participants will learn how the PSH model improves the patient experience, makes surgical care safer and more efficient, and lowers institutional costs. World-renowned experts will share theories about the PSH model, implementation strategies and how to apply basic physiological concepts into clinical practice. Change management and the importance of teamwork in the PSH model also will be highlighted. QUESTIONS?
