Perinatal Service Coordinator Field Placement Brooklyn Perinatal Health Information Technology (BPHIT) Program The goal of the New York State Department of Health s Perinatal Health Information Technology (PHIT) program is to address fragmentation in maternal and child health care by closing the referral loop and ensuring better care transitions for at risk and low income women. This is a NY state funded project that comes out of Medicaid Redesign recommendations to improve care delivery by supporting initiatives demonstrating effective use of HIT technology to link health care systems and community based health organizations. Using a newly designed software platform, communication will be facilitated between medical and social services providers to promote timely referrals, coordinated care, and enhanced screening of risks for women at risk of poor maternal and child outcomes. Perinatal Service Coordinators (PSCs) will be assigned to one of the partner medical or social services agencies we term Project Users. PSCs will assist in care coordination for clients at risk of poor birth and/or maternal outcomes. PSCs will perform a variety of duties, including: working on-site to help support implementation of a Community Based Service Navigation (CBSN) system, ensuring timeliness of patient referrals, and supporting communication between BPN and Project Users. PSCs will be trained in the utilization of a health information technology (HIT) solution developed to screen and identify risks, refer women for services, and plan care coordination. Internship Overview: The Brooklyn PHIT PSC Program is a field experience for academic credit. It is ideal for undergraduate or graduate students in the health and human services fields with knowledge of and/or an interest in health equity, community health, public health, and women s health to gain hands-on experience in the expanding health delivery and health technology field. This field experience is unpaid. PSCs are expected to work for at least 8 hours per week (with additional hours as approved). Each PSC is matched with a medical or social service provider and site supervisor. Some of the sites participating in the PHIT Program are: Brooklyn Perinatal Network Caribbean Women s Health Association New Dimensions and Abundant Life Healthy Start Brooklyn BMS Family Health Center CAMBA New Directions Alcohol and Substance Abuse Responsibilities will include: 1. Assist in care coordination between medical providers and social service providers in order to address barriers in care for high-risk women and mothers 2. Support medical and social service agency staff with sending and receiving referrals through Community Based Service Navigation PeerPlace systems including data entry of client data, following up with providers as well as other everyday needs related to the program 3. Troubleshoot issues with the Community Based Service Navigation PeerPlace system and be available for technical assistance to agency 4. Document and analyze ways that Community Based Service Navigation PeerPlace system can be made more efficient for clinicians and providers 5. Collaborate with BPN staff on the PHIT project through meetings and conference calls 6. Overview Brooklyn Perinatal Network (BPN) was established in 1988 from a community task force looking to address high infant mortality in several Brooklyn communities. We work to prevent and reduce infant and maternal illness and death by enabling at-risk residents to access vital information and coordinated care, and assisting families in securing public health benefits and resources. Since BPN s launch over 20 years ago there have been significant reductions in infant death and improved maternal and child health status. BPN collaborates with several community organizations in order to provide medical care, psycho-social health, and educational services; referrals to other providers; and coordinate and provide social health and supportive services.
