Putting Patients First with the CMS-2728 Form CMS and the ESRD Networks are focusing on having CMS-2728 Forms submitted within 10 days of admission This campaign will support CMS goal to Put Patients First National compliance is 21% for January 2018 -- let us know how we can support you/your facility s improvement 6
Top 10 Reasons to Submit the CMS-2728 within 10 days 1. Put Patients First Establish/facilitate ESRD Medicare Coverage for beneficiaries 2. CROWNWeb and the CMS-2728 for Foreign Nationals CMS has re-confirmed that Foreign Nationals/Visitors receiving treatment(s) in dialysis facilities should be entered in the ESRD National Patient Registry - CROWNWeb The N/A box may be checked for the SSN and Claim # fields to Admit as a New ESRD Patient and complete the CMS-2728 Facilities should continue to complete the CMS-2728 Form and submit to CROWNWeb (within 10 days; no later than 45 days) The CMS-2728 for Foreign Nationals require labs within 45 days prior to admission in order to submit the form in CROWNWeb Labs should be provided prior to patient s arrival, or the facility may choose to have required labs drawn on the first day of dialysis to meet the 2728 requirement Completed CMS-2728 forms for Foreign Nationals do NOT get mailed or sent to SSA (unless person applying for ESRD Medicare benefits). 9
