CCFS2 Legally-Exempt LE Training Page Data Entry Reference Sheet (October 2017) Facility Name The Facility Name field displays: FCC/IH: the name of the individual the enrollment form listed as the child care provider. GNUA/GUA: the name of the child care group program. Program Type FCC IH GUA GNUA Program Sub- Type FCC FCC-IH GUA: Federal GUA: Nonpublic operated voluntary registered nursery school GUA: NYC Article 43 (School Based) GUA: NYCDOH Summer Day Camp GUA: NYSDOH Summer Day Camp The value in the Program Type field is carried over from the program type selected on the New Enrollment for Legally-Exempt Care window. FCC (Family Child Care) indicates that care is provided in a residence, for at least one child who does not live in the home where the care is given. IH (In-Home Child Care): indicates that care is provided in a residence and ALL children being cared for live in the home where care is provided. GUA indicates that the legally-exempt child care program is a Group Under the Auspices of another federal, state, or local government or a tribal agency. GNUA indicates that the legally-exempt child care program is a Group Not Under the Auspices of another government agency. If the Program Type is In-Home, there is no Program Sub-Type. The values in the Program Sub Type drop-down are legally-exempt group program types identified in 18 NYCRR 415.1. If the Program Type is FCC (Family Child Care), one of the following sub-types is displayed: FCC (Family Child Care) indicates that care is provided in a residence AND none of the children in care live in the home where the care is provided. FCC-IH (Family Child Care In Home) indicates that care is provided in a residence AND at least one child in care does not live in the home where the care is provided (FCC) AND at least one child is being cared for in his or her own home (IH). The Status field displays the current provider enrollment status as indicated in CCFS. The Status Effective Date field displays the effective date of the current enrollment status. The Person Name drop-down menu displays the name of a facility staff person. Allows you to link to information stored in CCFS2 that is specific to this person. This is how you access the Training window for on-site providers and directors. Currently, pre-service training is not tracked for persons in the role of household member (HHM), volunteer, and employee. 4 Professional Development Program
CCFS2 Legally-Exempt LE Training Page Data Entry Reference Sheet (October 2017) Primary Role Director On-Site Provider Person Overall Criminal Assessment Not Yet Determined Allowed Disallowed The Primary Role field displays the person s primary role at the child care site. Currently, the pre-service training is not tracked for persons with roles other than Director or On-Site Provider. Person Overall Criminal Determination is a system generated field that displays the overall compliance status for the Site Person pertaining to the Criminal Conviction history. Not Yet Determined indicates that all Criminal Conviction values are Not Yet Determined, No Disclosure, Incomplete, Allowed, No History, or Exception Granted; AND at least one value must be Not Yet Determined, Incomplete, or Blank. Allowed indicates that the Criminal Conviction data (Criminal Conviction History Categorization, Additional Standards Results, NYS Sex Offender Results, Other Conviction Info Results) for the selected Site Person will not stop the provider from becoming Enrolled. One of the following conditions exists: o All Criminal Conviction History data are No Criminal Convictions or No History. o One or more of the Criminal Conviction History data have a value of Category C; AND the Category C Review Decision is Allowed; AND there are no values of Category B, Category A, or Sex Offender. o One or more of the Criminal Conviction History data have a value of Category B, AND the EC Review Decision is Exception Granted, AND there are no values of Category A or Sex Offender, AND if there is a Category C crime then the Category C Review Decision is Allowed. Disallowed indicates that the Site Person has a Criminal Convictions history, and unless a Category C or EC review is completed favorably, the Provider Status must be Denied or Closed: Terminated. One of the following conditions exists: o One or more of the Criminal Conviction History data have a value of Category A. o One or more of the Criminal Conviction History data have a value of Category B AND the EC review decision is Blank or Exception Denied. o One or more of the Criminal Conviction History data have a value of Category C AND the Category C review decision is Blank or Disallowed. The Training for <Staff/Person Name> window opens and allows you to enter course-specific information. Training for <Staff/Person Name> window Course Completed From the Course drop-down list, select the course that was completed by the provider. A control date has been coded to coincide with the first date each training was available for competition by providers. Foundations in Health and Safety is the e-learning online course. o Control date: February 15, 2017 Health and Safety for Legally-Exempt Providers is the LE classroom course. o Control date: April 1, 2017 SUNY Health and Safety Training: Competencies for becoming FDC/GFDC is the Licensed/Registered classroom course. o Control date: August 1, 2016 Note: OCFS-Approved Alternate: Must not be selected unless OCFS has given written, case specific approval for an alternate course meeting compliance with the regulatory pre-service training requirement. Enter the date completed as stated on the certificate. Date Received Verification Enter the date certificate was received by the enrollment agency. From the Verification drop-down list, select the appropriate value. The values are: Created In Error: Use this value to indicate a training record entry was created in error. Incomplete: Use this value when the provider has indicated the training was completed, but no Pre-service Training certificate is attached. Satisfactory: Use this value when the Pre-service Training certificate appears to be valid. Under Review: Use this value when the Pre-service Training certificate requires further validation. Unsatisfactory: Use this value when, after attempting to validate a certificate s authenticity, you find that it is invalid. 6 Professional Development Program

Click the Cancel button to close the window. Pre-Service Training History table Action (Edit) Completed Course Description Verification Date Received Date Created Created By Date Updated Updated By The Edit link displays in the Action field if the Pre-Service Training History table is populated with training information. Use the Edit link to edit to open the Training for <Staff/Person Name> window and modify any of the course information in the Pre-Service Training History table. This includes the Course, Completed, Date Received, and Verification fields, as described earlier in this document. The Completed, Course Description, Verification, and Date Received fields are populated by the entries made in the Training for <Staff/Person Name> window. Note: The Course Description field represents the course name found in the Course field. Displays date pre-service training entry was first saved. Displays the name of the CCFS user who created the pre-service training entry. Displays the date the pre-service training entry was updated. Displays the name of the user who updated the pre-service training entry. Professional Development Program 7
LE Training Page Data Entry Reference Sheet (October 2017) CCFS Legally-Exempt PRE SERVICE TRAINING REQUIREMENT Pre-Service Training Status Values for the Pre-Service Training Status drop-down list include: Exempt: An individual is exempt if they are related to every subsidized child in care by one of the following relationships: Grandparent, great-grandparent, aunt, uncle, or sibling in a separate residence. Requirement Met: There is at least one entry in the individual s Pre-service Training window with a verification value of satisfactory. Requirement Not Met: There is no entry in the individual s Pre-service Training window with a verification value of satisfactory. Note: When you have an individual who is both a relative who is exempt from the pre-service training requirement AND who has at least one entry in the individual s Pre-service Training window of Satisfactory, then choose Requirement Met. Date Updated Updated By Displays the date the pre-service training status was updated. Displays the name of the user who updated the pre-service training status. Click the Save button to save changes to the Training page. Footer Case Manager The case manager is the person designated by the enrollment agency as the primary person responsible for managing the provider s enrollment. The case manager is responsible for timely and accurate completion of tasks associated with the provider and parent. Enrollment Agency Office Phone Number Displays the enrollment agency responsible for managing the provider s enrollment. Enrollment agency jurisdiction is determined by the site county or the site affiliated county for non-nys providers.
