This publication applies to all 25 AF active duty Air Force, 25 AF-assigned Air Force Reserve (AFRES) and Air National Guard (ANG) members, and civilian employees paid through appropriated funds assigned to the HQ 25 AF Staff, 625th Air Communications Squadron (ACOMS), 625th Air Support Squadron (ASUS) and 625th Operations Center (OC). It does not apply to Centers, Wings, or Groups subordinate either to the 25 AF, or to units assigned to these organizations located in San Antonio, Texas. Refer recommended changes and questions about this publication to the Office of Primary Responsibility (OPR) using the AF Form 847, Recommendation for Change of Publication; route AF Forms 847 from the field through the appropriate functional chain of command. This publication may be supplemented at any level, but all direct supplements must be routed through the OPR prior to certification and approval. The authorities to waive wing/unit level requirements in this publication are identified with a Tier (e.g., T-0, T-3) number following the compliance statement. Requests for waivers must be submitted through chain of command to the appropriate Tier waiver approval authority, or alternately, to the publication OPR for non-tiered compliance items. This publication does not generate information collection and reports as outlined in Air Force Instruction (AFI) 33-324, The Air Force Information Collections and Reports Management Program. 1. Purpose. The HQ 25 AF Awards and Recognition Program is designed to recognize and highlight military and civilian personnel who go above and beyond in their duties and greatly contribute to the 25 AF worldwide mission. 2. Award Categories. A professional will be chosen as the HQ 25 AF quarterly and annual winner in the below categories. 2.1. Field Grade Officer (FGO) Category. FGOs in grades O-4 and O-5 (excluding O-6 selects). 2.2. Company Grade Officer (CGO) Category. CGOs in grades O-1 through O-3. 2.3. Senior Noncommissioned Officer (SNCO) Category. SNCOs in grades E-7 and E-8 (excluding E-9 selects). 2.4. Noncommissioned Officer (NCO) Category. NCOs in grades E-5 and E-6. 2.5. Airman Category. Junior Enlisted Airmen in grades E-1 through E-4. 2.6. First Sergeant (1st Sgt) Category. First Sergeants who have graduated from the USAF First Sergeant Academy, been awarded the 8F000 Special Duty Indicator (SDI) and are currently performing duty in an authorized/funded position. This is an annual category only. 2.7. Civilian Category I (non-supervisory). Civilians in grades GG-01 through GG-09 (or equivalent) in non-supervisory positions. 2.8. Civilian Category II (non-supervisory). Civilians in grades GG-10 through GG-14 (or equivalent) in non-supervisory positions. 2.9. Civilian Category I (supervisory). Civilians in grades GG-01 through GG-09 (or equivalent) in supervisory positions. 2.10. Civilian Category II (supervisory). Civilians in grades GG-10 through GG-14 (or equivalent) in supervisory positions. 2.11. Team. All HQ 25 AF active duty Air Force, HQ 25 AF-assigned AFRES and ANG members, and civilian employees paid through appropriated funds assigned to the HQ 25 AF Staff, 625 ACOMS, 625 ASUS and 625 OC. This award will be quarterly and not included in the annual awards. 3. Award Periods. 3.1. Quarterly Awards. Quarterly awards periods are 1 January through 31 March (1st Quarter), 1 April through 30 June (2nd Quarter), 1 July through 30 September (3rd Quarter), and 1 October through 31 December (4th Quarter). 3.2. Annual Awards. The annual awards period is 1 January through 31 December.

25AFI36-2805 15 JUNE 2017 3 4. Nomination Requirements. 4.1. Unit of Assignment. Personnel must be assigned to the HQ 25 AF, 625 ACOMS, 625 ASUS or 625 OC for a majority of the nomination period to be eligible for consideration in HQ 25 AF quarterly or annual awards program. 4.2. Rank or Grade. Personnel must have served in one of the applicable ranks or grades from paragraphs 2.1 through 2.10 of this instruction for a majority of the nomination period for the category they are nominated for. Personnel with a promotion sequence number will compete with their respective peers in the rank they possessed for the majority of the recognition period except as noted in paragraphs 2.1 and 2.3. 4.3. First Sergeants. Nominees must have served in the position of First Sergeant and held the SDI 8F000 for at least 6-months during the award period. 4.4. Quality Force Requirements. Nominees must not have an open/active unfavorable information file (UIF), Article 15 or Court Martial action. 5. Nomination Procedures. 5.1. Each Directorate, Special Staff, 625 ACOMS, 625 ASUS and 625 OC may submit one nominee in each category. 5.1.1. An endorsement letter signed by the Director/Commander must accompany nominations and include the name, email address, fax, and duty phone of the unit point of contact. 5.1.2. Nomination write ups must be security reviewed by the 25 AF Security Office (A2S) in person or via Joint Worldwide Communications Systems (JWICS). Security reviews will be confirmed by a stamp and initials by A2S or an A2S review statement with the name, date, and phone number of the A2S official included in the endorsement letter. Nomination packages received without proper security review will not be accepted. 5.2. Nominations will be emailed to the 25 AF/Awards Recognition Program mailbox. 5.3. Accomplishments must have occurred during the nomination period. 5.4. Use an AF Form 1206, Nomination for Award (latest version). 5.4.1. Bullet format will be used for all awards except the team category. Narrative format will be used for the team category. 5.4.2. Nominations for military categories are restricted to 12 single-spaced lines (including the headings) for quarterly competition. Nominations for annual competition will follow the format listed in AFI 36-2805, Special Awards and Trophies, paragraph 3.10.6. Nominations for civilian categories are restricted to 13 single-spaced lines (including the headings) for quarterly competition and 30 single-spaced lines (including headings) for annual competition. Nominations for the team category are limited to a maximum 20 single-spaced lines. 5.4.3. The following headings are mandatory and will be used on military nominations for quarterly awards:

LEADERSHIP AND JOB PERFORMANCE IN PRIMARY DUTY (8 bullets): Describe significant leadership accomplishments and how well the member performed assigned primary and additional duties. Define the scope and level of responsibilities and the impact on the mission and unit. Include any new initiatives or techniques developed by the member that positively impacted the unit/mission. Include results of Air Force, Major Command (MAJCOM), Numbered Air Force (NAF) Level inspections and/or evaluations. Include awards received (e.g., Lance P. Sijan, and so forth). 5.4.3.2. WHOLE AIRMAN CONCEPT (2 bullets): Show how the member developed/improved skills related to primary duties (e.g., formal training, career development, course enrollment or completion, on-the-job training, certification, offduty education related to primary duties, and so forth). Include completion of any Professional Military Education (PME) as well as awards earned during in-residence attendance. Include any off-duty education not directly related to primary duties (e.g., class, course, degree, enrollment/completion and grade point average). Cite any other relevant training or activity that significantly enhanced the member's value as a military citizen. Define the scope and impact of the member's positive leadership and involvement in both the military and civilian community. Include leadership, membership, or participation in First Sergeant Council, Unit Advisory Councils, Professional Military Organizations, associations, and events (e.g., President of Top 3, Enlisted Dining-Out Committee, Member of Air Force Sergeants Association, Air Force Association, Sunday School Teacher, and so forth). 5.4.4. Headings for military nominations for annual competition will follow the format listed in AFI 36-2805, Special Awards and Trophies, paragraph 3.10.6. 5.4.5. The following headings are mandatory and will be used on civilian nominations: 5.4.5.1. JOB PERFORMANCE IN PRIMARY DUTY: Describe how well the nominee performed assigned primary and additional duties. Define the scope and level of responsibilities and the impact on the mission and unit. Also, identify recognition of duty performance earned at the unit, installation, command, Air Force or Department of Defense levels. 5.4.5.2. LEADERSHIP QUALITIES: Describe significant leadership accomplishments. Include the development of new techniques contributing to increased mission effectiveness. These qualities could be evidenced by supervisors or by non-supervisory personnel through management of a project associated with their primary duties. 5.4.5.3. OTHER ACCOMPLISHMENTS: Recognize contributions to the mission such as volunteering for on-duty projects in other work centers or units. Include any significant self-improvement through off-duty education, training or achievements in professional associations directly related to official duties. Include base or community involvement.

25AFI36-2805 15 JUNE 2017 5 5.4.6. Team nominations must demonstrate how the team significantly forwarded one or more of the 25 AF/CC s priorities as presented in the 25 AF Strategic Plan. The nomination will include the narrative limited to a maximum 20 single-spaced lines with team members listed on the back. Teams will consist of two or more people and are not limited to personnel assigned to the same work center. (NOTE: Government contractors may not be included in team awards.) 5.4.7. Signatures on the AF Form 1206 are not required. 6. Selection Board Composition and Procedures. 6.1. Board Composition. Board composition will be made up of FGOs, SNCOs and civilians assigned to HQ 25 AF. A minimum of three members constitute a board as listed below; additional voting panel members are authorized. The board president is the senior panel member and will only vote when a tiebreaker is needed. 6.1.1. FGO/CGO: Three O-5/O-6s or GG-14/GG-15s. The 25 AF/CV (or delegate) will serve as the board president for all officer boards. 6.1.2. SNCO/NCO/Airman: Three E-7 through E-9. The 25 AF/CCC (or delegate) will serve as the board president for all enlisted boards. 6.1.3. First Sergeant of the Year: The 25 AF/CCC will serve as the board president. The board will consist of at least two additional Chief Master Sergeants or Chief Master Sergeant Selects. 6.1.4. Civilian Supervisory/ Non-Supervisory Categories. Three O-5/O-6s or GG- 14/GG-15s. The 25 AF/CS (or delegate) will serve as the board president for all civilian boards. 6.2. Conduct of the Board. All boards will be records-only unless directed otherwise. Quarterly packages will be scored using Attachment 2 for military and Attachment 3 for civilians. Scoring will be completed by each board member individually and each package will be scored on a 6 to 10 point scale with half points allowed. After scoring, packages will be ranked 1-n with the highest point total being the number 1 package. The board president will break all ties and address any scoring inconsistencies. 6.2.1. Award Winner Selection. The board president will submit all completed score sheets and final rankings to HQ 25 AF/CSE. The list of proposed winners in all categories will be submitted to 25 AF/CC for approval. 6.3. Recognition. 6.3.1. The 25 AF/CS will fund the purchase of annual and quarterly awards. The 25 AF/CSE is responsible for ordering awards. 6.3.2. The 25 AF/CC, or the appointed representative, will present quarterly and annual awards at a recognition ceremony. 6.3.3. HQ 25 AF Quarterly Award winners will receive a one-day pass (military) and eight hours time off award (civilian). HQ 25 AF Annual Award winners will receive a three-day pass (military) and 24 hours paid leave (civilian). Individual directors are responsible for submitting time off awards via the human resource liaison.

6 25AFI36-2805 15 JUNE 2017 6.4. HQ 25 AF enlisted Annual Award winners will be the headquarters nominee to compete in the Air Combat Command (ACC) annual awards program. Each enlisted Annual Award winner must comply with the requirements for the 12 Outstanding Airmen of the Year (12 OAY) Award program as described in AFI 36-2805, paragraph 3.10. 7. The Quiet Professional Award. 7.1. The Quiet Professional award is a tool for directors and commanders to recognize their personnel (both military and civil service) for exceptional duty performance, initiative or leadership. 7.2. Award Criteria. Military and civilian members can be recognized for their achievements in five areas of concentration: accomplishment of NAF goals or objectives; demonstrated exceptional leadership; improved work environment; improved morale; and/or delivered outstanding customer service. 7.3. Nomination Requirements. Any person within the HQ 25 AF can nominate a military or civilian professional for this award by completing and signing 25 AF Form 112, The Quiet Professional Award. 7.4. Nomination Procedures. HQ 25 AF employees who nominate an individual for this award by initiating the 25 AF Form 112 forwarding it to the Directorate s Executive Officer or the Headquarters First Sergeant. The 25 AF Form 112 is forwarded to the Commander for final approval. Approved awards are forwarded to the First Sergeant for processing. 7.5. Recognition. The recipient will receive a Certificate of Recognition signed by the 25 AF/CC. Military personnel will be authorized a one-day pass and civilians will be awarded an eight-hour time off award.
