NEW APPLICANTS New applicants must supply the following documentation for consideration: Curriculum Vitae Completed CDSBEO Application Form Three (3) Practice Teaching Reports (new graduates only) Copy of valid Ontario College of Teachers Certificate Recommendation of one Board Principal and two relevant letters of reference, or recommendation of three Board Principals Signed authorization to verify references Copy of TB test results Criminal background check Pastoral reference (optional) 3. RENEWAL APPLICANTS Renewal applicants must supply the following documentation for consideration: 4. PRINCIPAL S ROLE Completed CDSBEO Application Form Copy of valid Ontario College of Teachers Certificate Recommendation of three Board Principals Criminal Background Check New Applicants: By signing a recommendation (see Appendix A ), a Board Principal acknowledges that he/she knows the applicant and that the references provided by the applicant have been verified. For new graduates, a signed recommendation also acknowledges that the Board principal has reviewed the applicant s practice teaching reports. Renewal Applicants: By signing a recommendation, a Board Principal acknowledges that he/she knows the applicant and that the renewal applicant is a suitable candidate to continue on the board-wide Occasional Teacher s List.
