INTRODUCTION JOB DESCRIPTION PAEDIATRIC SENIOR CLINICAL FELLOW (ST3+) PAEDIATRIC DEPARTMENT MEDICAL CARE GROUP JOB DESCRIPTION We are seeking to employ one whole time post on a fixed term contract for 2 years. This post provides excellent paediatric experience in both in-patient, out-patient and Emergency Department settings. The successful candidate will take part in a 1 in 11 second on call, EWTD, new contract compliant, full shift rota. TRUST PROFILE We provide a full range of acute inpatient, out-patient, day case and emergency services to the local communities of St Helens, Knowsley, parts of Halton and Liverpool, extending to the whole of Cheshire, Merseyside, North Wales and the Isle of Man. As a Trust our aim is to provide a high quality service to all our patients. We strive to meet the best standards for professional care whilst being sensitive and responsive to the needs of individual patients. Our valued workforce is recognised for their unique skills and talents with our teams of experts winning national awards on a regular basis. OUR ACHIEVEMENTS 2018 staff survey indicates Trust as best place to work in the NHS. In 2017, Trust named as the best in England in the Patient Led Assessment of the Care Environment Programme (PLACE). A Care Quality Commission (CQC) inspection in August 2015 resulted in an overall rating of GOOD with Outstanding elements. Rated as EXCELLENT for a number of years running in the Patient Environment Action Team (PEAT) assessments for both Whiston and St Helens Hospitals. Specialist Regional Burns and Plastic Surgery Unit serving a population of around four million people. The hospital includes 875 beds, of which 516 are in the medical division. State of the art CT and MRI facilities exist on both sites.

THE WORK OF THE PAEDIATRIC DEPARTMENT The primary aim of the Paediatric Unit is to provide the best possible quality of acute medical care for the children from our local community. This includes services for the newborn as well as the care of children and young people. Our philosophy of care is one of quality, both in excellence of medical care and in the flexibility of service provision. The Department was awarded an OUTSTANDING score for Caring, and GOOD for the remaining Key Lines of Enquiry, with an overall score of GOOD at the most recent CQC inspection in August 2015, and consistently scores over 95% in the Friends and Family Test patient satisfaction survey. In the most recent National Children s and Young People s Inpatient and Day Case Survey (results published in Nov 17), the Department scored within the top 5 Trusts in England for the experiences of children and young people aged 8-15 years. The Department has pioneered different approaches to Paediatric care. We have a successful Children s Hospital at Home nursing team which enables us to extend the range of acute care that we can offer in a way that is very responsive to the needs of children, young people and their families. It allows medical staff to concentrate on the most severely unwell children and babies and also offers our excellent team of children's trained nurses the opportunity to care for patients in a very individual and responsive way. All shift leaders on the wards are Advanced Paediatric Life Support (APLS) trained. There are 5 Paediatric Nurse Specialists who work with children who have Diabetes, Respiratory diseases (including cystic fibrosis, BPD, asthma and allergies), Epilepsy and Complex Health Needs. There are 2 Advanced Paediatric Nurse Practitioners (APNPs)/Nurse Clinicians who provide clinical service supporting on occasion the SHO rota in addition to senior nursing duties and there are a further 3 APNPs currently in training. The Department has a Nurse Specialist for Safeguarding Children who works closely with the clinical staff and with the Named Consultant, Named Nurse and Named Midwife for Safeguarding. The Paediatric Health Visitor Liaison Service for St Helens, Knowsley and Halton is also based in the Department. PAEDIATRIC AND NEONATAL FACILITIES Paediatric inpatient services are based at Whiston Hospital: Ward 3F: Combines all inpatient speciality paediatric beds both medical and elective surgery including 2 dedicated paediatric high dependency beds, open 24 hours a day. Ward 4F: Combines the Children s Observation Area (CHObs), a dedicated Paediatric Day Surgery area and a children s phlebotomy service. The Department is responsible for looking after all children admitted with head injury, and also provides pre and post-operative oversight of children admitted to the hospital under the care of Consultants from other specialities within the Trust. Neonatal Unit The Neonatal Unit is a Level 2 unit, located adjacent to the Delivery Suite at Whiston Hospital. There are ventilators and nasal CPAP machines on the Unit and facilities to ventilate babies short-term prior to transfer out to a Level 3 unit. Babies under 27 weeks gestation and also those who require long-term ventilation/intensive care are transferred to the Regional Level 3 Neonatal Units. The Neonatal Unit also has a portable ultrasound scanner. PAEDIATRIC OUT PATIENT FACILITIES General Paediatric Outpatient Clinics are held at Whiston, St Helens, and Newton Community Hospitals. Specialist clinics for diabetes, asthma, cystic fibrosis, BPD, cardiology, epilepsy and haematology are held regularly in the Hospitals. Joint clinics are held with visiting specialist consultants in the fields of paediatric neurology, epilepsy and cystic fibrosis. The Junior Establishment consists of: 1. Five Specialist Registrars (on Mersey Rotation), ST4 and above (one in community) 2. Three full-time ST3 Registrars (one post shared with Community) 3. Three s These 11 Specialty doctors share a 2 nd on call night rota. 4. Seven whole time equivalent junior doctors (6 GP trainees and 1 ST1/ST2 career grade paediatrician) 5. Two F2 Trainees 6. One full-time Trust Grade junior doctor All rotas are EWTD and new contract compliant. THE JOB This is a full-time post and the holder will be expected to work flexibly on the ST3+ rota of 1 in 11. Duties will cover all areas in the Department including inpatients, the Children s Observation Unit, Outpatients, the Neonatal Unit, Paediatric Emergency Department and Maternity Unit for high risk deliveries. The role also encompasses elements of Clinical Audit, Clinical Governance, teaching of medical students, nursing staff and junior doctors. Regular Departmental activities include: 1. Hot Topics in Neonatology Monday PM The successful candidate will have the opportunity to rotate through a sub specialist clinic of their choice in cardiology, diabetes, epilepsy and respiratory The On Call Sessions A summer and winter rota is in place (see page 5). This allows for increased clinical cover of the ED department and inpatient wards during the winter months and increased outpatient activity during the summer months. Page 4 of 8

When on call, the will supervise F2-ST2 doctors working on the General Paediatric Wards, Emergency Department, Neonatal Unit as well as the Labour and Postnatal Wards. The post holder will be resident when on call. Consultant Paediatric advice will be available when needed. The post holder will also undertake to be available exceptionally for such irregular commitments outside normal scheduled hours as are essential for continuity of care. AUDIT Middle Grades/Registrars will be expected to co-ordinate, contribute and participate in both Department and Regional Audit initiatives. CONTINUING EDUCATION All Medical Staff are expected to participate in continuing medical education to maintain standards. There are Departmental Grand Rounds on Friday morning and a Hospital Grand Round on a Tuesday lunch time. The post holder will be expected to contribute towards both grand Rounds on a rota basis with other middle grade staff. APPRAISAL The post holder will undergo regular appraisal by the Clinical Director/Educational Supervisor in accordance with Trust Policy. The post holder will have a named Consultant as his/her mentor. CLINICAL GOVERNANCE The Paediatric Directorate has a robust Clinical Governance Structure involving the whole multi-disciplinary team. All are encouraged to play a leading role in Clinical Governance within the Department and facilitate Departmental Junior Doctors forum meetings. ANNUAL LEAVE The post holder will be entitled to 30 days annual leave and 10 days study leave per year; the latter at the discretion of the Clinical Director. CONDITIONS OF SERVICE The post is whole-time. Salary scale and conditions of service for Clinical Fellow posts as negotiated for Hospital Medical and Dental Staff. SAFEGUARDING RESPONSIBILITIES To be competent at safeguarding children and implementing the Child in Need and Child Protection procedures as appropriate includes the following skills, competencies and knowledge:- 1. Have understanding of own (and others ) professional roles and boundaries, including the Named professionals, trust managers and social workers. 11. Knowledge of the implications of key national document/reports. 12. Understand the assessment of risk and harm. 13. Understand multi-agency frameworks, investigations, working and child protection assessment processes including the use of the Common Assessment Framework / investigation/working. 14.
