JOB DESCRIPTION JOB DETAILS Job Title: Pay Band: General Practitioner- Primary Care Support Team Medical and Dental Directorate: Primary, Community and Mental Health Department: Primary Care ORGANISATIONAL ARRANGEMENTS Managerial Accountable to: Reports to: Clinical Lead- Primary Care Support Team Clinical Lead- Primary Care Support Team Professionally Responsible to: Clinical Lead- Primary Care Support Team JOB SUMMARY / PURPOSE The Primary Care Support team will provide additional support to GP practices across Hywel Dda to meet patient demand and expectation through new ways of working. The team will initially be focussed on supporting practices who going through difficult periods of recruitment and sustainability. The GP role is key to the team and supporting the development of sustainable General Medical Services. The team will work across Hywel Dda and members will be expected to travel across the 3 counties to deliver support to practices. MAIN DUTIES AND RESPONSIBILITIES The post holder will be a fully participative member of the Hywel Dda Local Health Board Primary Care Team. As part of the Primary Care Support Team, duties will include consultations by phone (including triaging of calls), day to day clinical supervision of Advanced Practitioners working in the same practice and any administration associated with patient care. The post holder will be expected to work with the Clinical Lead in the development of new systems of delivering GMS Services and support the dissemination of best practice which would include the delivery of staff training for other disciplines within the service. The post holder will act as a change agent for service improvement within Primary Care as well as a clinical mentor for existing staff and students from all clinical disiciplines. Service Management Clinical. As a clinican the postholder will be expected to:
Provide primary care services to the residents and visitors to Hywel Dda working with other team members to a high quality Respond to medical problems presented by patients, by means of history taking, examination, investigation where appropriate, diagnosis, treatment and referral as needed. To be part of a Multi disciplinary team that delivers care as appropriate to the individual within Primary Care Deputise for the Clinical Lead to provide basic general medical services to patients on the HDUHB Violent Patient Scheme via telephone consultations; including medical advice, safe prescribing and signposting on to other services as appropriate. To include the development and monitoring of care plans for the management of any chronic conditions or other medical needs. This will be provided on a rota basis with other Salaried GPs. Service Improvement The post holder will be expected to take part in service reviews on the team with a view to modernise and develop the team as a result. This will be through service improvement methodology. Communications The post holder will communicate with a wide range of stakeholders both externally and internally including patients, families, Health Care Professionals, Health Board personnel including Executive Directors. This will be through a variety of means including written and oral, training sessions to peers and presentations to both internal and external stakeholders. Finance and Resources The post holder will be responsible for producing and maintaining timesheets relating to their own work Personal and People Development and People Management The post holder will will take part: Undertaking PDPs Provision of Clinical Supervision including 1:1 case reviews as appropriate Ensure that the training needs of the individual team members are met through the development of training opportunities through informal and formal mechanisms. The postholder will take part in the development of opportunities for students and trainees to undertake placements through the team and provide appropriate supervision/feedback. Provision of education and training to GPs and Practice Teams to aid the development of new ways of working. Information Processing As a clinician, the postholder will be required to ensure that the GP clinical system is kept up to date with accurate records of consultations which are either through the triage or face to face consultation. The postholder will be required to produce any data relating to their own work for the Clinical Lead to enable written and verbal reports to be produced on the activity of the team. the postholder will be responsible for their own Health, Safety and Security. Quality The postholder will ensure that appropriate clinical standards within the team are achieved and maintained. The Post holder will be required to take part in any governance reviews as deemed necessary by the Clinical Lead. The postholder may be required to impart unwelcome news to patients on a frequent basis or about performance issues to members of staff. The post holder might also be exposed to anger, distress or upset on the part of patients or staff as result of this news. The postholder might be exposed to dirt, fluids as part of their daily work in dealing with patients General Other tasks and duties may be determined by appropriate line manager as the role develops
