Educators Credit Union 2018 Member Scholarship Cover Sheet Educators Credit Union awards twenty-five $2,000 scholarships. Scholarships are awarded based on a student s academic record, participation in school and community activities, and demonstration of one or more of Educators Credit Union s core values honesty, integrity, fairness, respect and excellence. The completed scholarship application is due by February 27, 2018, to the person listed below. Scholarship recipients will be notified during the second week of April 2018. All scholarship funds will be made payable directly to the institution of higher education. CRITERIA THE STUDENT MUST: 1. Be the primary owner of an active account (Prime Share Savings) at Educators Credit Union at the time of application. 2. Be enrolled in a Dane, Jefferson, Kenosha, Milwaukee, Ozaukee, Racine, Rock, Walworth, Washington, or Waukesha County high school.* 3. Be eligible for graduation at the end of the 2017-2018 school year. 4. Have a cumulative GPA of 3.0 or higher at the end of first semester senior year. 5. Be accepted to an accredited college or technical school at the time of award. 6. Show a demonstration of one of the core values of Educators Credit Union. *Students participating in a home-based educational program, privately operated correspondence or virtual school must reside in the qualifying counties listed above. Applicants attending these programs must also provide proof of enrollment with the Wisconsin Department of Public Instruction (form PI-1206) for the 2017-2018 school year, ACT or SAT scores, and any records relating to curriculum and academic progress.

RESUME OF AWARDS, COMMUNITY SERVICE, WORK, ETC.: A typed list in resume form of awards and/or academic honors; your community service and volunteer activities; your extracurricular activities and your work experiences from 9 th through 12 th grade. The list should be no longer than one page. B. PERSONAL ESSAY (typed using a 12-point font): The values of Educators Credit Union are honesty, integrity, fairness, respect and excellence. Prepare a typed essay of approximately 500 words in which you describe how you have demonstrated one of these values. Grammar and punctuation will be considered. C. ONE LETTER OF RECOMMENDATION: The letter should explain why the writer believes you are a strong candidate for the Educators Credit Union Scholarship. D. DISCLOSURE FORM: A signed and completed disclosure form must be included with the completed scholarship application. 2018 Member Scholarship Disclosure Form In the event of my selection for this scholarship, I authorize Educators Credit Union to share my name, image, general student information (major, school, year, etc.) and award amount to my school and the media. I am aware that if I am awarded an Educators Credit Union Member Scholarship, I will be required to: 1. Submit a picture to be used in publicizing the award by April 25, 2018. 2. Deliver a thank you note to the committee for the selection before May 21, 2018. 3. Provide proof of acceptance/enrollment to the college you will be attending by August 1, 2018. 4. Complete nine modules of online financial education program EverFi through the Educators Credit Union website prior to disbursement of the award. Modules completed as part of required schoolwork can be counted. Deadline is July 16, 2018. Failure to complete these items by the deadlines may result in forfeiture of the award.
