My Life Check As the American Heart Association/American Stroke Association (AHA/ASA) and Alpha Kappa Alpha Sorority, Incorporated (AKA) continue to share common goals in educating minority populations in heart disease and stroke health, the American Heart Association (AHA) is providing the My Life Check assessment as a resource to guide communities to live healthy, live smart, and live better. AKA has been a leading organization in both educating and creating awareness in the communities they serve, and the My Life Check assessment will be another resource used to lead the minority communities they serve. What is My Life Check? My Life Check is a short assessment created by the American Heart Association. The assessment is completed by answering a few questions regarding ethnicity, age, and weight. The assessment will then calculate a personal heart score based on the answers given. Heart scores are based on a scale from 1-10, with 10 being an ideal score. This means an individual with a higher score has the lowest possible risk of developing heart disease and stroke. The Goal The goal of the American Heart Association/American Stroke Association and Alpha Kappa Alpha Sorority, Incorporated is to increase the overall health of every individual by just 1 My Life Check point, which is a 10% improvement! Step by Step Guide to Obtaining your My Life Check URL Step 1: Visit the new EmPOWERED To Serve website at www.empoweredtoserve.org. Step 2: Hover over the About tab in the menu at the top of the page until you see Strategic Alliances appear in the drop-down menu. Step 3: Click Strategic Alliances to view a list of some of the current alliances working with the American Heart Association/American Stroke Association.
Step by Step Guide to Obtaining your My Life Check URL (Continued) Step 4: From this list, select AKA s name or logo to access more information. Step 5: From the AKA page, click My Life Check URL from the side navigation bar to begin your request.

This will send your request to receive your chapter s unique URL. Complete all fields with the correct and appropriate information. Any missing information will delay the generation of the unique URL for your chapter. Click the SUBMIT button to send the request to the AHA National Center team. NOTE: When planning and requesting the URL, please allow 21 days to receive your URL. Due to high volume, requests to expedite URL requests cannot be guaranteed. The URL will be emailed to the contact address provided in the request form and will be from empoweredtoserve@heart.org. Once the URL has been received by your appropriate contact, we encourage that person to distribute it via your internal communication methods. For best results Provide members a specified time frame to complete the assessment. At least ten (10) unique individuals must complete the assessment to generate a report and a group Heart Score. Once the specified time frame has been established and the URL has been distributed, please email empoweredtoserve@heart.org with the request to obtain your organization assessment. Please allow two (2) weeks to receive the final group assessment report. The following is required in the email: Email Subject: Request for MLC Group Assessment Organization s URL Organization s Name Contact Person s Name and Email Address Contact Person s Phone Number Specified time frame provided to members to complete the assessment.
