Montgomery County Public Schools Application Form for the 2015-2016 Medical Science with Clinical Applications Pathway The application deadline to be considered for the 2015-2016 Medical Science with Clinical Applications Pathway is March 6, 2015 The Medical Science with Clinical Applications (Medical Science) program is a two semester career course that enables students to explore careers in the health care industry and to gain skills related to patient care practiced in hospitals and long-term care facilities. Students receive Cardiopulmonary Resuscitation (CPR) training and may be eligible for certification as a Certified Nursing Assistant. Students may also be eligible to take the Geriatric Nursing Assistant examination upon completion of the program. Medical Science meets the graduation requirement for completion of a career development program. Students completing the full-year Medical Science program will earn a total of two or three credits, depending upon program location. Students successfully completing the first semester of the school-based Medical Science program may be invited to participate in a second semester clinical rotation at a local hospital and/or a long-term care facility. Students will spend four days a week, for up to three periods per day, at the clinical rotation site. One day a week students will participate in a seminar related to their clinical rotation experience. The student must: Be 16 years of age by December 31, 2015. Be either a high school junior or senior by August 2015. Have a minimum cumulative unweighted Grade Point Average of 2.5. Have completed Biology A & B or Chemistry A & B achieving a grade of B equivalent or better for each semester. Have completed Foundations of health and medicine A & B achieving a grade of "C" or better for the year. Have completed structures and functions A & B achieving a grade of "C" or better for the year. Program Description Program Entrance Requirements Concurrently enrolled in one of the above sciences if both courses have not been previously completed. Have fully completed the application by the March 6, 2015, deadline. Completing the Medical Science application by the deadline does not guarantee acceptance into the program. The attached Medical Science Scoring Rubric is used as a tool to identify qualified candidates. Be willing to provide appropriate documentation required by the hospital and/or long-term care facility that may include a social security number and/or proof of citizenship/green card. Additional requirements may include fingerprints, background checks, drug screening, evidence of insurance coverage, a flu shot, blood immunity titers for Hepatitis, tuberculosis screening, etc. For more information, contact the Medical Science teacher at the appropriate program location. Possess excellent communication skills, maturity, and the physical ability to independently perform patient care skills as outlined in the Omnibus Budget Reconciliation Act (OBRA) regulations. During the second semester clinical rotation, students are involved in hands-on patient care at a local hospital and/or a long-term care facility under the supervision of the Medical Science teacher/registered nurse. A copy of the OBRA Required Nursing Assistant Skills Checklist (Page 2) is included in this packet for your review. Second Semester Entrance Requirements The student must: Be concurrently enrolled in Chemistry B or Biology B (if not previously completed) at the student s home school. Successfully complete the Medical Science A course, achieving a minimum semester grade of a C and final exam grade of C; maintain a 95 percent attendance rate; obtain the American Heart Association Health Care Provider CPR/Basic Life Support (BLS) certification; and secure a favorable recommendation from the Medical Science teacher prior to enrollment in the second semester Medical Science B course. Comply with expectations and requirements of the cooperating health care agencies. 1
