Ohio s efforts to reduce the infant mortality rate The three leading causes of infant death in Ohio continued to be prematurity, sleep-related issues and birth defects.
Medical Neighborhood Referral Infrastructure The purpose: Spread best practices to improve coordination of care between providers and social services in Greater Columbus. The objectives: Implement cloud-based, shared referral infrastructure among local healthcare providers, social service agencies, and other healthcare stakeholders. 2016 Objectives At least twenty social service agencies and fifteen local healthcare provider sites participating in project implementing CliniSync to send and receive referrals. Collect project measures, document lessons learned, and share learning with public-private partners.
Engaging the Care Continuum & Bidirectional Flow of Information CliniSync will offer at least three methods to allow these organizations the ability for bidirectional data flow. These options will enable any level of organization who is using a certified EMR to participate. 2017 Goal - Key Technology Deployments to enable all levels of participation 2017 Goal - Key Technology Deployments to enable the parsing, de-duplication and normalization of key data sets.
Integration into the Workflow Enhance integration into provider s EHR to enable information to be pulled directly from CliniSync for care of the patient. 2017 Goal - Key Technology Deployments: Enhancements to the feature and functionality that allows for higher level of integration into the EHR.
National Coordination Cross state and entity exchange is another key driver that stakeholders identified as critical to their success. Additionally, there is a need to bring added value and a complimentary strategy to the CareQuality and CommonWell initiatives that are being driven by the EMR vendors. 2017 Goal - To enable expanded Cross State and Entity exchange and add additional functionality to compliment National initiatives.
