2018 HIGH SCHOOL SENIOR SCHOLARSHIP AWARD REGULATIONS ELIGIBILITY/LIMITATIONS FOR SCHOLARSHIP AWARD Applicant and sponsor must meet the following eligibility requirements: 1. Applicant must be a senior graduating from a high school or home school program during the 2018-2019 school year. 2. Applicant must be a dependent with a current identification and privilege card. 3. The sponsor of the applicant must meet ONE of the following eligibility requirements: Active duty U.S. Military member currently assigned in Hawaii Active duty U.S. Military member on a remote assignment from Hawaii Retired U.S. Military member residing in Hawaii Full time Hawaii National Guard member** Full time U.S. Military Reserve member residing in Hawaii** Deceased U.S. Military member with applicant residing in Hawaii **Please contact hoscscholarship@gmail.com to verify eligibility** The following applicants are NOT eligible for an award: 1. Anyone who has previously been selected for an HOSC or First Command scholarship award 2. Anyone on a full scholarship 3. A high school transcript is required, but does not have to be sealed Copy of OFFICIAL ACT or SAT report (regardless of whether it is reported on high school transcript) ESSAY QUESTION Answer the following with a one page typed essay on an 8 ½ x 11 single-sided, single spaced page with one inch margins using Times New Roman 12 point font. Essays exceeding more than one page will not be accepted. Explain some of your personal challenges and successes as a military family member that will prepare you for your upcoming academic studies. The HOSC Governing Board will approve the total number of scholarships and total amount awarded. SELECTION COMMITTEE Scholarship applications shall be reviewed and judged by an outside source other than the HOSC governing board, advisors, and any active duty military members and dependents. Decisions of the selection committee are final. AWARD RECIPIENT RESPONSIBILITIES 1. The scholarship must be used by the recipient at an accredited trade school, 2-4 year college, or university, during the fall semester of 2018 and/or spring semester of 2019. In addition, scholarships may be used at a school pending accreditation by an organization approved by the US Department of Education. 2. The scholarship may be used for the payment of tuition, fees, and/or room and board charged by the Finance or Business Office of the school. 3. Recipients must submit to the Scholarship Coordinator a Use of Awards Agreement and Statement of Intent form no later than December 31, 2018. Use of Awards Agreements and Statement of Intent forms will be given to each recipient following the announcement of winners. 4. An official Verification of Enrollment letter from the educational institution s Registrar must be sent to the Scholarship Coordinator and received by December 31, 2018. Electronic verification through National Student Clearinghouse will also be accepted. Any fee associated with this service is the responsibility of the recipient. 5. The HOSC Welfare Treasurer shall issue a check in the full amount of the scholarship payable to the educational institution that has verified the recipient s enrollment. The Scholarship Coordinator shall forward this check within one month of receiving all necessary information from the recipient and the educational institution. 6. Any scholarships not claimed by December 31, 2018 will be forfeited. 7. Recipients who plan to attend only the spring 2019 semester must provide written intent of this to the Scholarship Coordinator by December 31, 2018 will and verification of enrollment by January 30, 2019.
