lower adirondack regional arts council learn share create COMMUNITY ARTS GRANTS 2017 FOR INDIVIDUALS The Community Arts Program is made possible with public funds from the New York State Council on the Arts, a state agency, administered locally through the Lower Adirondack Regional Arts Council. Applications must be postmarked by Friday, October 21 or hand-delivered to LARAC by NO LATER than 4pm Friday, October 21 MAIL TO: Community Arts Grants, LARAC, 7 Lapham Place, Glens Falls, NY 12801 LATE APPLICATIONS ARE NOT ELIGIBLE FOR FUNDING & WILL NOT BE REVIEWED. A COMPLETE APPLICATION PACKET MUST CONTAIN: EIGHT COPIES OF + Signed, three page application (One should be the original. No emailed or faxed copies will be accepted.) + Artist s resume as related specifically to the area in which applied (Limit resume to 2pg) + Letter from presentation sites(s) or organization(s) confirming cooperation on this project (if applicable to the project) + Writing sample (applicable only to literary or directorial proposals) ONE COPY OF + WORK SAMPLES (Weblink, CDs, DVDs - see guidelines} + PROOF OF PRIMARY RESIDENCE (i.e., driver s license, voter registration form, tax bill) FOR RETURN OF WORK SAMPLES, INCLUDE A SELF-ADDRESSED, STAMPED ENVELOPE TO BE ELIGIBLE FOR APPLICATION, ALL APPLICANTS MUST ATTEND AN APPLICATION WORKSHOP OR MEET BY APPOINTMENT WITH THE COMMUNITY OUTREACH COORDINATOR.

YES NO If so, where? PROJECT INFORMATION 1. Clearly describe the NEW WORK that you will create. State your artistic vision for the final product. Be specific; provide details. (The description of your request should begin here and may include one additional 8 ½ x 11 page using a font no smaller than 11. This additional page is allowed for question 1 only. No additional pages will be reviewed.)

2. A community component is required for funding. Describe the way in which your artwork will reflect Warren or Washington County s social, ethnic, or geographic environment, or culture, and/or how you plan involve the community. 3. Collaborating with groups and/or organizations can be a valuable component of your proposal. If applicable, name any individuals and/or organizations that you plan to involve. (Include in your application packet, 8 copies of a confirmation letter on letterhead paper from any organizations that have agreed to be a part of this project. See instruction page.)

4. Provide a timeline for the project from beginning to end, including community involvement. Give specific dates where possible. 5. Final phase of project must be open to the public. Where will that take place? How will you publicize it? 6. For LARAC s administrative use only, please state in 70 words or less, an abridged version of your proposal from Question 1.

2015 PROJECT BUDGET (ROUND OFF TO THE NEAREST DOLLAR) YOUR REQUEST MUST EQUAL 2500. A budget is requested so that the Review Panel understands the expenses that the proposed project entails. Fill in as applicable to your project. In addition to your artistic fees, funding requests may include materials, travel, transportation, and other professional fees, but they may not include capital expenses (for example: no equipment purchases or costs not directly associated with the proposal.)
