We believe it is very important for patients to have a thorough understanding of their condition and the reasons behind a certain treatment or procedure, so that they can be actively involved in their own care. We partner with you, to help you meet your health goals. Our Services Otolaryngologists are physicians trained in the medical and surgical management and treatment of patients with diseases and disorders of the ear, nose, throat (ENT), and related structures of the head, neck, and face. They are commonly referred to as ENT physicians. Our care team offers the highest quality ear, nose, throat, and audiology care whether medical or surgical. We practice a full range of otolaryngology, including medical and surgical management of disorders of the ears, nose, throat, voice, neck, thyroid and parathyroid glands, salivary glands, face, and scalp, in both children and adults. We also dispense, fit, and service hearing aids. Some of the disorders we treat are: Allergies Chronic ear infections (Otitis Media) Chronic sinusitis Dizziness (Vertigo) Facial skin abnormalities Head and neck cancer Hearing loss Hoarseness and vocal cord disorders Nose bleeds Ringing in the ears (Tinnitus) Sleep apnea and snoring Smell and taste disorders Thyroid and parathyroid disorders Tonsillitis

Meet Our Providers Lakeland Ear, Nose and Throat consists of three locations in Berrien Springs, Niles, and St. Joseph. We provide services for all of Berrien, Cass, and Van Buren counties. Providers: Stephen Bovenkerk, DO, FAOCO Debbie Habinect, MD, FACS Dennis Thompson, MD, FACS Audiologists: Cathy Burns, MA, CCC-A, FAAA Craig Rigler, MA, CCC-A Our providers also specialize in the management of problems of the nose, sinus and snoring related sleep apnea, as well as allergy testing and immunization. Facial plastic and reconstructive surgery with complete management of facial skin lesions is also a core service offered. Healthcare Collaboration Policy We think it s important to offer appointments in a timely manner so you can receive care when you need it. Please call the practice as soon as possible if you need to cancel an appointment, so that it can be filled by another patient in need of care. If you provide less than a 24 hour notice, it may be documented as a no show. We understand that there are sometimes unavoidable emergencies and busy schedules that interfere with scheduled appointments. Three no shows or missed appointments in a six-month period may result in dismissal from the practice. In addition, any incident of abusive or rude behavior toward physicians or staff members may also result in a dismissal. Patients who have been dismissed from our practice will receive 30 days of emergency care services. Medical records will be transferred to a new doctor upon the patient s request.

Your Appointment To schedule or reschedule an appointment, use your Lakeland mychart account, or call the St. Joseph Office (269) 982-3368, Niles office (269) 687-2910, or the Berrien Springs office (269) 471-9702. Please arrive 15 minutes prior to your scheduled appointment to allow time to check-in. We reserve the right to re-schedule an appointment if you are more than 10 minutes late. Please bring photo identification and your insurance card to every appointment. Laboratory and Radiology Services Appointments for laboratory and radiology services can be scheduled by calling (269) 556-2810 or (800) 791-2810. Please be advised that because these services are performed outside of the office, you may receive a separate bill in addition to your office visit.

Before Your Appointment A team of medical providers will work with you to help you meet your health goals. You, your doctor, and the rest of the care team will work together to make your visit as helpful as possible. Here are a few things you can do to ensure your appointment is a success: Bring all of your current medications in their original containers to your appointment including over-the-counter medications, prescriptions, and vitamins you take on a regular basis. Make a list of your health questions, with the most important at the top. We will answer as many as we can during your appointment, but may need to schedule another appointment to answer all of your questions. Feel welcome to bring a family member or trusted friend with you to your appointment During Your Appointment Ask the most important questions from your list first and track which questions have been answered. Be sure you know what you need to do before you leave the office. Repeat the instructions you have been given to your doctor or appropriate member of your care team. Talk to your doctor and care team about the health issues you should work on first. Write down the name of each member of your care team and ask how they can be reached in case you have a question or concern that needs to be addressed.

Office Hours Monday through Friday 8:00 a.m. to 5:00 p.m. Our practice is closed on major holidays. We do our best to meet the needs of all of our patients. When calling the practice, you may be asked to hold or leave a message if we are experiencing a large volume of calls or need to assist a patient in the practice at that time. All calls will be returned by the end of the business day. If you have an urgent medical need, please call 9-1-1 or go to the nearest emergency department. After Hours Care A physician is on-call 24 hours a day. If you call when the practice is closed and have an emergency that cannot wait until the next business day, you will be automatically re-directed to our after-hours answering service. Please leave a message with the answering service and they will contact the on-call physician or direct you to your Primary Care Provider.

Protecting Your Medical Information Our doctors follow guidelines established in the Health Insurance Portability and Accountability Act of 1996 (HIPAA) when using and sharing your protected health information with appropriate care providers and insurance companies. Your health information is only shared when it is required to provide you with care, and cannot be released without your consent. Easy Online Access to Your Medical Records Our practice participates in Lakeland mychart, a free, internet-based tool that provides you with secure access to your medical information that is released at the discretion of your physicians and medical providers. Ask our staff to register you, or call 1-800-LAKELAND to request access. Patients who are enrolled with Lakeland mychart can check account balances, manage appointments, request medication refills, communicate with their provider, and view lab and imaging results.

Medication Refills You can request medication refills by calling the office during regular business hours or through Lakeland mychart. Please allow three (3) business days for your prescription to be processed by your physician. Please be advised that your physician may require you to attend an appointment to discuss the progress of your health with your current medication. If your insurance company requires prior authorization, please allow an additional two to three days to process your prescription. Be prepared remember to collect the following information: Full name of the person taking the medication Birth date of the person taking the medication Complete medication name Medication dosage to be ordered and frequency Name of the pharmacy refilling the prescription Your telephone number Name and relationship of the person requesting the medication We are happy to e-prescribe, fax, or call in prescriptions to your pharmacy, or prepare a paper prescription for you to pick up. When you call, let us know which you prefer.

Financial Policy Lakeland Ear, Nose & Throat participates with many insurance companies. Co-payments and deductibles are due at the time of service. We accept cash, most credit cards, and personal checks. Alternate payment arrangements can be made with a patient accounts representative by calling (866) 814-7275 or (269) 428-5007. Uninsured patients may be eligible to receive a discount up to 20 percent on services. Follow these guidelines before your office visit to receive the maximum benefits from your insurance plan: Make sure your insurance is accepted at our practice. Determine which services are covered by your plan. Use your legal name for all insurance information and medical record transactions. Bring your insurance card to each appointment. If you are covered by two insurance companies, present both cards at registration. Patients who participate in Medicaid must present a current form of identification. Contact your insurance company if you have questions about eligibility for claim denials. Cases that involve automobile accidents and Workers Compensation require written or verbal authorization prior to treatment.
