Description of the Service we provide and Client groups who may access it: Community Mental Health Service for patients from 18 years upwards who have an enduring functional mental illness such as Severe depression Treatment resistant depression Schizophrenia Bi-polar disorder Borderline Personality Disorders Severe Anxiety states Eating disorders Phobic disorders Post traumatic stress disorder The team is made up of doctors, nurses, social workers and occupational therapists. The community Mental Health Service provides a range of therapeutic treatments including psychotropic treatments, group work, systemic work, CBT (Cognitive Behavioural Therapy) CAT work (Cognitive Analytical Therapy). Access to service is through GP s, other professional organisations and some self-referrals (in crisis). Patients are allocated daily to practitioners in the team and treatment is commenced with regular reviews as directed by the care record and CPA Policy document and HoNOS, PbR clustering. The principle to involve service users and carers in W.R.A.P. is used as part of the patients treatment plan and relapse prevention. Cases are discussed at weekly clinical meetings, regular supervision and caseload management supervision is implemented to support the practitioners in delivering the appropriate and consistent treatment for the patient.
