Summary This report and grades represent our assessment of the quality of the areas of performance which were examined during this inspection. Grades for this care service may change after this inspection following other regulatory activity. Time is being spent with individuals and their families to support any changes that occur. The service uses an assessment framework around identifying the level of staffing required dependant on which group of service users are using the service. This will ensure that service users staying will continue to have their needs met to a high standard and will assist the planning of people's stays. The care and support is person - centred as far as possible within a group living situation. Involvement of service users and carers is actively promoted in this service. What the service could do better The service should continually strive to maximise the involvement of service users and carers within the service as it goes through it's transitional period. It should also ensure that the monitoring of quality in all aspects of the service is maintained. What the service has done since the last inspection The service is gradually changing and is supporting new people into breaks at Glenallan and for some people moving on to other respite hostels is happening. This may be to services which may meet their physical needs better but also looking at different options for people. Conclusion In Glenallan the care and support is person - centred as far as possible within a group living situation. Involvement of service users and carers is actively promoted in this service. 1 About the service we inspected Glenallan was registered with the Care Commission in April 2002 to provide respite care and support for up to eight adults with learning disabilities. Approximately 60 adults use the service on a regular basis, with planned visits throughout the year. The home is owned and managed by the City of Edinburgh Council Health and Social Care Department. Since April 2011 this service has been registered with Social Care and Social Work Improvement Scotland (SCSWIS) which is to be known as the Care Inspectorate. The home is a two storey detached house in a quiet cul de sac on a housing estate in the Inch area of Edinburgh. There are gardens to front and side of the property. A range of local amenities and a bus route are close by. The home comprises of a communal sitting room, kitchen, laundry, toilet and office on the ground floor. Single bedrooms, bathing and toilet facilities are on the upper floor. There is also a small quiet sitting room on the first floor. Access to the upper floor is by a stairway, there is no lift. The provider has plans to redesign this service and move to an upgraded and fully accessible building in the future and users / carers are being kept informed of the progress with this. 2 How we inspected this service The level of inspection we carried out In this service we carried out a low intensity inspection. We carry out these inspections when we are satisfied that services are working hard to provide consistently high standards of care. What we did during the inspection We wrote this report after an unannounced inspection of the service. The inspection was carried out by Donald Preston, Care Inspectorate Inspector. We visited Glenallan Hostel on 14th & 21st October 2013. As part of the inspection, we took account of the completed annual return and self assessment forms that we asked the provider to submit to us. During the inspection we met the Manager, Depute, five carers and five service users. Time was spent with all of them. The views of service users and staff on the service were taken into account in writing this report and grading this service. A review of records and other documentation including the following: - service user's individual support plans including risk assessments - minutes of service user and relatives meetings - staff training records - service user medication folders - accident and incident logs - audits carried out by the service. Staff practice was observed at the time of the visit. We observed the environment having been shown around the house and seeing service user's rooms with their permission. Grading the service against quality themes and statements We inspect and grade elements of care that we call 'quality themes'. For example, one of the quality themes we might look at is 'Quality of care and support'. Under each quality theme are 'quality statements' which describe what a service should be doing well for that theme. We grade how the service performs against the quality themes and statements. Inspection Focus Areas (IFAs) Inspection report continued In any year we may decide on specific aspects of care to focus on during our inspections. These are extra checks we make on top of all the normal ones we make during inspection. We do this to gather information about the quality of these aspects of care on a national basis. Where we have examined an inspection focus area we will clearly identify it under the relevant quality statement. Fire safety issues We do not regulate fire safety. Local fire and rescue services are responsible for checking services. However, where significant fire safety issues become apparent, we will alert the relevant fire and rescue services so they may consider what action to take. The annual return Every year all care services must complete an 'annual return' form to make sure the information we hold is up to date. We also use annual returns to decide how we will inspect the service. Annual Return Received: Yes - Electronic Comments on Self Assessment Every year all care services must complete a 'self assessment' form telling us how their service is performing. We check to make sure this assessment is accurate. The self assessment was completed to a very good standard and highlighted many strengths and areas that the service wished to develop further. It showed a good awareness of what was good practice in this type of service. Taking the views of people using the care service into account We met with five people using the service. One person had verbal communication and was able to say that they enjoyed coming to Glenallan and liked all the staff. The others were observed to interact well with staff members who obviously knew their communication needs and had positive relationships with them. Taking carers' views into account There were no carers spoken to as part of this inspection. Glenallan Hostel, page 8 of 21
3 The inspection We looked at how the service performs against the following quality themes and statements. Here are the details of what we found. Quality Theme 1: Quality of Care and Support Grade awarded for this theme: 5 - Very Good Statement 1 We ensure that service users and carers participate in assessing and improving the quality of the care and support provided by the service. Service strengths The service has a Participation Strategy regarding service users, carers and staff involvement and has regular contact / feedback from relatives / carers. The service has a key worker system which involves individuals & their families in the support they receive at Glenallan. Regular reviews are held where individual's requirements for their holidays e.g rooms can be discussed and put in place for when they come to stay. At these meetings service user and carer satisfaction is checked out. Glenallan holds Carer's meetings where the current changes will be discussed, any staff changes, staff training, medication procedures, welfare fund, respite planning cycle and any general issues that people wish to talk about. The service continue to hold twice weekly Service User meetings which gives people an opportunity to talk about what they would like to do while having a respite break and staff will plan and support these activities. It was seen that what people had asked for had been followed through on many occasions. Questionaires have been issued to service users on a regular basis and have focussed in on certain aspects e.g. bedrooms, meals & introduction to the service - these have symbols included on them. These have been completed through the user group led by an advocacy worker. These meetings take place 4 times a year. There is also a general questionnaire which is based around the Quality Themes. Also people preference in particular bedrooms, who they would prefer to come in with, meals and activities are catered for if possible. This information is recorded in support plans and / or the planning database for the service. The house meetings have led to suggestions from service users which have been followed through in relation to activities both in the hostel and outside, also furniture and furnishings for the house have been influenced by individuals choices. Service users have the option to bring personal items with them on holiday to ensure that they are settled in the respite hostel. Dependant on people's wishes and abilities they can be involved in household activities and helping in the garden. Individuals can have keys for their rooms and lockable cabinets in their rooms are available and they can choose to spend time in the different areas of the house. In the past service users have been involved in the recruitment of staff or asked what questions they would like asked of people to be interviewed. This involvement will vary depending on the timing of recruitment and the interest of the service users who are available at any particular time. Management respond to questionnaires, house and carers meetings and issues coming from individual reviews. The service provider has a Complaints Policy in place and there is a Complaint / Suggestions / Feedback Box available within the house. Areas for improvement The service is piloting a new Carers' Feedback form with communication between service and home being a very important aspect of the support being provided to families. Service users to be involved in future interviews and support staff are to look at best ways of doing this. Training has been provided for all staff in respect of health issues relevant to the people they are supporting. This included moving and handling, epilepsy, diabetes, administration of medication - observed / competency based, mental health first aid, CALM training and infection control. Policies and Procedures are in place for all the health issues relevant to running a care service including infection control and medication. All staff have had training in Protection of Vulnerable Adults (POVA). The service has been involved with and awarded the Healthy Eating Plus Award and the cook is heavily involved in the ordering and along with staff is aware of likes / dislikes and special diets for the respite users. The service works closely with the multi-disciplinary Community Learning Disability Team around individuals that they are supporting. Health and safety checks are carried out on a regular basis within the service. Records were observed which evidenced that health concerns were being identified, acted on or communicated to family or other health professionals. Areas for improvement This area is characterised by major strengths which we would expect to continue in the future. The service has lots of strengths in relation to involving service users / carers and evidence of this was seen at the time of the inspection and detailed previously in this report. Areas for improvement This level of involvement should be sustained in this service and any opportunities to increase participation should be explored. This will be observed at the next inspection. Grade awarded for this statement: 5 - Very Good Number of requirements: 0 Number of recommendations: 0 Statement 2 We make sure that the environment is safe and service users are protected. Service strengths The service has risk assessments in place and personal emergency evacuation plans in place for all service users. These were found to be up-to-date. All staff receive training in manual handling, food hygiene, epilepsy, medication and coping in a health emergency. Some staff have completed CALM training to be able to support people with challenging behaviours in a practical and safe manner and have an increased awareness of their environment. The service has all the necessary health and safety checks in place with regular monitoring of equipment - records are kept & processes in place to ensure these checks happen. The service has radio handsets, room monitors (permissions gained for use of these) and door alarms can be used dependant on the needs of the services users and staff members. Protection of Vulnerable Adult training has been undertaken by all staff members and is mandatory for any new recruit. Incidents and accidents are logged and actions followed up. This was evidenced through the log and the issues arising concerning individuals and how this had led to changes in the support being provided which was clearly documented in the individual support plan. The necessary policies and procedures are in place which back-up the monitoring and training that is provided in this setting. These include adult protection, restraint, recruitment and selection, challenging behaviour, volunteers, and health and safety. Areas for improvement The service provider recognises that the building has its limitations in relation to the service users that come for breaks and plans are in place for the service to be moved to a new purpose-built site in 2014 which will enhance this area. Service strengths The service provider has an induction programme for all new staff members, some of this is external but a lot of it is within the hostel. All the necessary policies are in place to support the staff team in respect of their role and the issues that may arise within the service. Regular supervision / Performance Review Development (PRD) is carried out with Glenallan Hostel, page 14 of 21
all staff members by the manager and depute of this service. Training records were seen to be in place and mandatory training / refreshers are to be organised on a regular basis. The service has referred to codes of practice, policies and the National Care Standards through team meetings, training events and in compiling evidence for the self evaluation form. The service closes for a few days once a year when all staff get together and can receive refresher training and look at issues that are affecting the service now and in the future. At this point they identify objectives for the service and people's training needs. These can be reviewed throughout the year at staff meetings and supervision. The service has a high percentage of its staff group who have completed the appropriate qualification in respect of their role within the service. The roles of the senior community support workers are changing and they will, once trained, take on the supervision of other staff in the service. The service provider has a range of on-line training available to all staff. Areas for improvement This area is characterised with major strengths which should be maintained within this service. As the service develops into a specialist autistic resource and further training is to be put in place to support service users with these specific needs. Suggestions from meetings - house and carer's ones and from questionnaires were seen to have been acted and outcomes were evidenced. feedback or changes required from discussions are followed through. This is either by their actions or through the key worker / line management system that is in place. Medication and Financial checks were seen to be carried out on a regular basis as per Policies and Procedures. Health and Safety audits are carried out between service staff and health and safety staff from the provider. Other systems in place are monitored on a regular basis by management. These include incidents and accidents, complaints / suggestions and maintenance. The service has received the higher Healthy Eating Plus Award for the meals provided in the service. Regular training, supervision and support was evident in this service. The service led by management does team planning which was seen to be effective in this service. Areas for improvement This area has evidence of a very high standard and should be maintained in this service.
