EFFECTIVENESS OF APPLICATION CLINICAL CASE STUDY CITI Healthcare is currently working with a large University Hospital where Patient Buddy has been customized and is currently in use. Through our studies at that facility, we are already seeing the benefits in several key areas as a result of using the Patient Buddy App. Patient Buddy has simplified the process of communication between Patient and Provider with our secure messaging feature, which is HIPAA Compliant. Patient Buddy was effective in removing the communication barriers between Patients and Providers. Patients are able to able to conveniently see all of their discharge information, request an appointment, request a medication refill, or message their Provider with questions or concerns all securely through the App. It was also noted that Patient Buddy was effective in keeping patients engaged in their overall health and wellness, as well as compliance with their instructions after discharge. Another area where Patient Buddy was effective in this case study has been better decision-making by Providers with streamlined workflow and improved efficiency. As patients use the App, actionable data is available to the Providers in an easy-to-read dashboard format with drilldown features. We have also effectively addressed high hospital readmission rates through Patient Buddy s feature functionality. During the first few months of use, Patient Buddy was able to reduce readmission rates by almost 30%. Patient Buddy will also be effective in helping Providers meet Meaningful Use Stage 3 by using the App for the Patient Engagement and Secure Messaging. 2016 Creative Information Technology, Inc. Go To Market CITI Healthcare s Patient Buddy Mobile Application is fully developed and is being integrated with Allscripts TouchWorks and other EHRs. Patient Buddy can be quickly implemented and deployed at any size Healthcare Facility. It is fully functional on IOS, including ipad and iphone, and is in the final stages of development for Android and Windows. Patient Buddy is ready out of the box to help track medications and provide secure messaging between Patient and Provider, but it can also be customized to track any details pertinent to the patients medical history, disease state, or medication regimen. Such features can include tracking things like weight, girth, blood sugars, cognitive tests, etc. These enhancements can effect the Go Live Timeline, however, CITI Healthcare has staff ready to begin working on these projects immediately. Since the Application is already live and in use, once customizations and training happen, the Providers and Patients can immediately being using the App. We currently promote Patient Buddy to large hospitals across throughout the United States as well as the world. As HIMSS Corporate Members, we demonstrate Patient Buddy at the show, as well as various other tradeshows. This can be deployed on a per provider fee or on a per patient fee. The pricing models are customized to each prospective client s requirements.
