Before You Begin Before you can complete this procedure, the following conditions must be met: You must be a Facility Editor in CROWNWeb. Only a Facility Editor can discharge a patient. The patient must be admitted to your facility. You can only view patients attached to a facility over which you have scope. Let s get started!

Tutorial Patients That Discontinue Dialysis If a patient chooses to discontinue dialysis, the facility would discharge the patient with a reason of Discontinue, following the same process as in Discharge a Patient. If the patient expires less than 30 days from the date of discontinue and a CMS-2746 is submitted, do not change the Discontinue discharge reason. Enter the Date of Death and Primary Cause of Death on the Patient Attributes screen and submit the CMS-2746.
Tutorial Troubleshooting Commonly experienced problems, recognized solutions, and other key information: If you do not see the Edit/Admit Discharge link, you may not have the role of Facility Editor. If you cannot see the patient, you may not have scope over that facility.
Tutorial Summary To discharge a patient: 1. From the Home Page, click Patients. 2. Enter search criteria, and then click Search. 3. Click the desired CROWN UPI. 4. Click Admit/Discharge Summary. 5. Click the Admit Date. 6. Click Edit Admit/Discharge. 7. Enter the Discharge Date and Discharge Reason. 8. Click Submit.
