Tutorial Patients That Discontinue Dialysis If a patient chooses to discontinue dialysis, the facility would discharge the patient with a reason of Discontinue, following the same process as in Discharge a Patient. If the patient expires less than 30 days from the date of discontinue and a CMS-2746 is submitted, do not change the Discontinue discharge reason. Enter the Date of Death and Primary Cause of Death on the Patient Attributes screen and submit the CMS-2746.

Tutorial Troubleshooting Commonly experienced problems, recognized solutions, and other key information: If you do not see the Edit/Admit Discharge link, you may not have the role of Facility Editor. If you cannot see the patient, you may not have scope over that facility.

Tutorial Summary To discharge a patient: 1. From the Home Page, click Patients. 2. Enter search criteria, and then click Search. 3. Click the desired CROWN UPI. 4. Click Admit/Discharge Summary. 5.
