Message from Toby Hall St Vincent s Health Australia Group Chief Executive Officer I m very pleased to introduce the St Vincent s Health Australia (SVHA) 2017 Innovation & Excellence Awards. Our envision2025 strategy of serving, seeing and striving for something greater is inspiring us onwards and upwards in our thinking and actions. Sharing the best of this is vital to our learning and growing. Our annual awards recognise and celebrate the great work of our people across our health and aged care services who are Inspired to Care every day by our Mission and Values. The theme for this year s awards is Welcome, valued and safe reflecting SVHA s commitment to putting the well-being of our staff, patients, residents, their families and carers at the centre of everything we do. The award categories are based on the 5 pillars of the SVHA envision2025 strategy: 1. Leaders in Catholic health care and service to the poor and vulnerable 2. Excellent care for our patients and residents 3. Leading reputation 4. Best people in health and ageing 5. Growth and sustainability This year for the first time, we have added another award category to recognise exemplary performance by individuals identified by their managers, supervisors or peers as having consistently excelled in their positions and demonstrated integrity, leadership and a strong commitment to the Mission and Values of SVHA. These new Excellence awards will go to one individual from each of the Group Office, Public, Private and Aged Care Divisions. The awards are open to all staff across our facilities and services so no matter what role you play as an individual or part of a team, I encourage you to enter. You ll find everything you need to know about the awards in this Guide Book. Finalists in each category will be invited to the Innovation and Excellence forum and gala presentation dinner in Sydney on 23 August 2017. Award Category Leaders in Catholic healthcare and service to the poor and vulnerable Delivering better health outcomes for targeted populations through our services, research and advocacy programs are the core elements of the legacy of Mary Aikenhead and the Sisters of Charity and represent who we are at SVHA. This Award recognises activities that are focused on the delivery of services and initiatives for the poor and vulnerable and engage people in the area of Catholic identity ensuring that our Mission is evident in daily interactions. Initiatives eligible for this award may include: An effective Mission formation program that engages our people and develops their understanding of what it means to be part of SVHA. An innovative program or project that demonstrably fosters any aspect of the Catholic identity and Mission of SVHA. An initiative where pastoral care has been incorporated into the overall model of care or where developments in pastoral care practice have fostered an increase in understanding of Catholic identity and values. Award Category Excellent care for our patients and residents SVHA is recognised as a leader in patient/resident care and safety and providing excellent patient/ resident care is paramount for the care and recovery time of our patients /residents. Our belief in a holistic model of care is a reflection of the inherent individuality of each patient and resident we care for. Ensuring we achieve the intended patient outcomes through engagement and collaboration with our patients and their families and having systems and processes in place to oversee continuous improvement programs is essential for our success. This Award is broken into 3 categories: Clinical improvement The Clinical improvement award recognises teams who have implemented improvement programs or projects in care delivery that advance quality of care and improve patient outcomes. Initiatives eligible for this award include: Projects that have been successfully implemented within a facility / division to advance quality and improve care delivery where the methodology may have been adopted from elsewhere. Clinical innovation The Clinical innovation award recognises teams who are developing or have implemented innovative and transformative programs or projects in care delivery that advance quality of care and improve patient outcomes. Initiatives eligible for this award include: Projects / or programs that have been developed for the implementation by the facility / division to advance quality and improve care delivery. Person centred care The rights, experiences and views of our patients, residents and clients are at the centre of the care process and drive the way in which we deliver safe quality healthcare. This award recognises the significant efforts of clinicians and managers in improving the quality and safety of patient care and to encourage innovation and improved practice. Initiatives eligible for this award include: Initiatives that demonstrate patient experience is continually measured and improved. Activities that illustrate collaboration with patients, residents, clients, carers, families and health professionals in program and policy development, and in health service design, delivery and evaluation. 8 SVHA Innovation & Excellence Awards 2017

Award Category Leading reputation SVHA strives for a reputation for excellence and innovation in our clinical practice, care, research and advocacy. The value of Excellence is one of our core organisational values and signifies our commitment to striving for something greater for all those we serve. We are especially proud of our long history of service to the poor and vulnerable in line with our Mission. This award recognises activities that showcase our work and enhance our reputation in the public domain with the community, governments and other non-government agencies. Initiatives eligible for this award may include: Public advocacy efforts on behalf of the people we serve. Innovative partnerships with the community, governments and/or other leading organisations including other health and aged care providers, community partners, universities and research institutions. Research excellence - discovery of new knowledge or the use of existing knowledge in a new and creative way to generate new concepts, methodologies and understanding which has the potential to deliver major benefits to SVHA patients / residents / clients and or the community. Award Category Best people in health and ageing SVHA aims to attract, develop and retain the best people in a safe working environment across the whole organisation. Our people are encouraged to see, serve and strive for something greater as a reflection of our mission of compassionate care. This Award recognises individuals or teams who: Drive a significant uplift in capability to create and lead people through change. Create a culture where the organisation cares for employee safety as much as patient safety. Integrate systems that drive excellence in people management. Communicate our Mission, Vision, Values and Creed to enable employees to advocate for our strategy. Initiatives eligible for this award include: Initiatives that attract and retain outstanding talent. Programs that improve, inspire or maintain best practice in workplace health, safety and wellbeing. Award Category Growth and sustainability In an ever-changing landscape and with community expectations continually changing, SVHA continues to work on developing and implementing technologies and innovative projects that assist in building and growing a sustainable, efficient organisation. Motivated by our commitment to faithful stewardship and respect for the integrity of creation, this Award recognises projects and programs that integrate systems and/or processes to support our growth strategy and financial performance at the facility, division or group level or assist SVHA in being a leader in healthcare environmental management. Initiatives eligible for this award include: New or improved projects or programs that support the operational efficiency of the business and or its growth strategy. Initiatives that improve operational efficiency and improved outcomes through systems and or process improvements / redesign. Initiatives that demonstrate a reduction in our environmental impact at the facility, division and group level. SVHA Innovation & Excellence Awards 2017 11

Award Category Group & Divisional CEO s Excellence Award for individuals These awards are designed to recognise individuals who go over and beyond their job description to influence leadership, excellence, mission and values of our organisation in their day to day work. There will be four Excellence Awards, one from each of the Group Office, Public, Private and Aged Care Divisions. Managers, supervisors or peers can nominate individuals who have consistently excelled in their position and demonstrated integrity, leadership and a strong commitment to the mission and values of SVHA. Judging of these awards will be completed by the Group & Divisional CEO s. 12 SVHA Innovation & Excellence Awards 2017

Award Category Chairman s Award for excellence The highest Award in the SVHA Innovation and Excellence Awards 2017 recognises leadership by an individual or a team within the SVHA community that best illustrates our Mission, Vision, Values and Creed. Mission: As a Catholic health care service we bring God s love to those in need through the healing ministry of Jesus. We are especially committed to people who are poor or vulnerable. Vision: We lead through research driven, excellent and compassionate health and aged care. Values: Compassion, Justice; Integrity and Excellence. Creed: We believe in the dignity of all people because each one is created in God s image. We are committed to justice and compassionate care for all. This Award is chosen from the field of finalists from each category selected through the judging process. You cannot apply for this Award. An impartial judging panel will convene and review all finalists. The judging panel will include the Chairman of the Board of SVHA, who will make the final decision on the recipient of the Chairman s Award. SVHA Innovation & Excellence Awards 2017 13

Conditions of entry Following are the conditions of entry for the SVHA Innovation and Excellence Awards 2017. In 2017 we expect all submissions to clearly articulate the alignment of the initiative or project with the Mission and strategic imperatives outlined in envision 2025 with particular regard to strengthening the concepts of serving, seeing and striving for something greater. Specifically All projects submitted for consideration must only relate to recent, original projects or projects that have been substantially altered or revised and completed within the last 2 years. Applicants will have actively engaged in work within St Vincent s Health Australia and partner organisations which: Support and promote the SVHA Mission, Vision, Values and Creed and envision 2025 strategic direction of St Vincent s Health Australia. Promote multidisciplinary approaches to providing a high quality, contemporary Catholic health and aged care service, and Be conducted within the framework expressed by the Code of Ethical Standards for Catholic Health and Aged Care Services in Australia. Entries must be carefully checked for accuracy and compliance with the Award entry requirements. Entries that do not comply substantially with the entry requirements may not be judged. Entries must be submitted via the Awards website. Direct submissions from individuals will not be accepted. Entries will be assessed by a judging panel, the decisions of which are final. Individuals/teams are able to nominate a particular project into one category only. Selection of the most relevant category for each entry rests with the team making the submission. NOTE: Submissions that are not sufficiently relevant will not be eligible as a finalist or winner in the category entered. Awards will not necessarily be given in all categories. All applications are to be submitted on or before the closing date, which is 5pm Friday 19 May 2017. SVHA Innovation & Excellence Awards 2017 15

Entry form requirements and guidelines The SVHA online submission website will require the following information for completion of your entry. Please note, the website template will only allow you to enter the specified word or character count. Criteria Abstract (150 words) Provide a brief summary of the project including the objectives, results, main conclusions and recommendations. This information will be used in preparing the abstract book of Award finalists should the entry be successful. Rationale (150 words) Provide a brief background on the project including the reasons for undertaking the project and alignment with the Mission and Values, remembering to align the initiative / project with the strategic imperatives of envision 2025 with particular regard to strengthening the concepts of serving, seeing, striving for something greater. Method (400 words) Provide details related to the implementation of the project, including the actual project steps and activities undertaken including, interventions, tools used and timeframes. Include evidence of participation and/or partnerships with consumers, the community and/or other stakeholders in the development and delivery of the project. Results (400 words) Summarise the outcomes of the project providing sufficient evidence (quantitative or qualitative data) to support the results including what changes have emerged. The evidence must support the conclusions in the next section. Conclusion (300 words) The conclusion must address the following: After analysis of the results, what is the opinion or judgement of the author(s)? Has the project achieved its objectives? What new knowledge was gained from the project? What has changed or happened as a result of the project? What are the ongoing implications of the study? Future directions (150 words) Describe how you can ensure the project outcomes continue and remain sustainable and what opportunities there are for sharing and replicating your project across other SVHA facilities. Supporting Documentation Tables and graphs Tables and graphs to support your results are to be uploaded in the supporting documentation section of the online submission site. You may upload 1 supporting document as.pdf or.doc format. Graphs and tables should be embedded as part of the Word document. If uploading large graphs or tables, please compress them where possible to reduce file size to less than 2MB to facilitate electronic submission of your entry. Photographs Each entry is to include up to four (4) photographs of the team, individual, project or program upon which the entry is based. At least one (1) photograph MUST be of the team or individual. Photos must be attached as separate files (in JPG format). File size for each photo should be no more than 1MB. Please note the inclusion of at least one (1) photograph with each entry is compulsory. Photographs are an important part of the SHVA Innovation and Excellence Awards ceremony and will be screened as part of a montage throughout the event. Video Files In 2017, you have the option to now include a video as part of your Award submission. You may upload in the following formats:.mov.mp4.mpeg.mpeg4.wmv You may also create a link to youtube or vimeo that supports your submission. Permission Ensure appropriate permission is obtained for using photographs as per the SVHA staff/patient consent form for media (available from your local Quality Coordinator). References When referring to other publications in the text, state the author s name followed by the date of publication. Please list full details of the publication in a reference list at the end of the entry in alphabetical order according to the Harvard system Compliance with requirements Entries that do not comply substantially with the requirements may not be judged. Finalist presentations Following the announcement of Award finalists on Monday 17th July 2017, each finalist team will be asked to prepare a PowerPoint presentation to showcase their project at the Innovation and Excellence Awards forum being held on Wednesday 23 August 2017 in Sydney. The forum will precede the Innovation and Excellence Awards dinner on the same day where the winner of each Award category will be announced. The format for the afternoon will be a series of rapid fire presentations from the finalists. Each finalist submission will be required to present for 5 minutes with 5 slides on their project in front of the Senior Leadership Team and their peers. There will be a People s Choice Award made following the afternoon s presentations. A template will be provided to assist you with this process. 18 SVHA Innovation & Excellence Awards 2017

Online submission guide The following information will help guide you through the process of creating your own account and submitting your entry into the St Vincent s Health Australia Innovation and Excellence Awards. Log in to the 2017 online submission portal which is located at https://svha.awardsplatform.com Please note: the following instructions relate to all categories EXCEPT the Group & Divisional CEO s Excellence Award for Individuals which will require Nominator and Nominee details as well as supporting criteria. Step 1: Create your own account Enter your details to create a User Log In account. This information is about the account owner and is not the entry. You will by required to create a password which must by at least 8 characters. Then click on the REGISTER button. You may also register using a social media account such as Facebook, Google or Twitter. Note: To enter a project for judging you must proceed to the next step, you have not nominated at this point. Step 2: Successful account creation Once you have completed step 1, an account verification email will be sent to you. Click on the link, or copy and paste into your browser to complete your account set up and continue your awards submission. You will be taken to the Awards home page where you will see the key dates and you will now be able to START NEW ENTRY. Step 3: Creating your awards submission Submission Details - Select your facility from the drop down box. Then please indicate in which of the 11 award categories your entry belongs. Judging criteria Each entry will be judged according to the following criteria. Alignment to SVHA Mission and values Degree to which the submission demonstrates a clear connection to the Mission and values of St Vincent s Health Australia.
