CMS-2728 END STAGE RENAL DISEASE MEDICAL EVIDENCE REPORT MEDICARE ENTITLEMENT AND/OR PATIENT REGISTRATION This form MUST BE completed within 45 days for ALL the following: Patients who start a regular course of dialysis for the first time ever Patients who initially receive a kidney transplant instead of a course of dialysis Patients 3 or more years post transplant who return to dialysis or receive another kidney transplant Patients who stopped dialysis more than 12 months ago and are now restarting dialysis or receiving a kidney transplant Patients who receive a transplant or are trained for self-care dialysis within the first 3 months of starting dialysis and submitting an initial form Don t forget to send a copy of the form to Social Security
CMS-2746 ESRD DEATH NOTIFICATION This form MUST BE completed within 30 days of the Date of Death If a patient discharges and expires after 30 days, then you are not responsible for completing the 2746 form If you receive notification of death, edit patient attributes in CROWNWeb to add date of death and cause of death Click on submit
Missing 2728 & 2746 Forms Under the 2728 tab, the due date is displayed Don t forget to click on Submit after completing the forms in CROWNWeb The form will be considered missing if it s in a Saved status Acute Transfer in/new start discrepancy Email reminders from Network data staff
PART verification Please note: If there is a duplicate treatment under the treatment summary, this will cause patient names appearing twice. However, one of the updates to CROWNWeb is to prevent duplicate treatment records.

What are Notifications and Accretions? Notifications and Accretions provide the ability for facilities to use CROWNWeb to troubleshoot and correct discrepancies generated by REMIS. Notification: A discrepancy in patient data between what the REMIS database has received from another CMS database and what exists in CROWNWeb (e.g., date of birth, last name, or a transplant that wasn t recorded in CROWNWeb). Accretion: A patient the REMIS database has learned about from another database and believes to be ESRD, but has not been admitted into CROWNWeb as a patient.
Notification Action Selections After reviewing or resolving the discrepancy, you will assign an Action to the Notification: Accept Reject Investigate Escalate You agree with the external source; the patient is part of your facility and changes are made. After research, you do not agree with the Notification and no changes are made. Lets another user know you are investigating this patient. If the patient is not part of your facility; the notification is reassigned to the Network for help.
CROWNWeb Clinical Updates Carry Over Data Review: Many clinical fields such as vaccination entries, will now carry over from month-to-month. All required data must be initially entered for their patients in the Clinical screen These fields must be reviewed monthly and updated if there are any changes to ensure that the data continues to carry over Most fields that carry over form month-to-month can be marked as N/A. You must remember that any fields marked as N/A will NOT carry over to the next month. That field will appear as a blank field for the following month.
Clinical Data - N/A CROWNWeb now allows users to provide reporting data for patients that did not have a treatment at the facility during the current reporting month. For these patients, CROWNWeb gives users the ability to select an overall N/A checkbox that states No Clinical Data Available for All Collection Types. Example: Patient is admitted to your facility in CROWNWeb at the start of the month, but passed away BEFORE RECEIVING ANY TREATMENTat your facility during that clinical month. NOTE: The EDI Clinical N/A selection process may varies from organization to organization. Contact your corporate office or EMR vendor for assistance.
Single Clinical Lab values - N/A: All fields require an input. Facilities must enter available clinical or indicate the information is NOT AVAILABLE (N/A) This provides a way for facilities to say I did not take or do not have this measurement for this patient for this month
Add New Facility Personnel 1. Click Personnel From the CROWNWeb Home Page, click Personnel. The Search for Personnel screen displays. 2. Click Add Personnel Click Add New Personnel in the sub-menu. The Add New Personnel screen displays. 3. Enter Employee Info Enter information for the employee, then click Submit. The Personnel Details screen displays. Make sure to select a Job Description such as Nephrologist, Facility Medical Director, or Other Physician in order to have that physician's name show in the pull-down menu when completing the CMS-2746 Form. 4. Personnel Submitted A confirmation message Personnel Submitted displays on the Personnel Details screen, if the submission was successful. *You cannot delete personnel who are associated with patient treatment, CMS-2728 or CMS- 2746 records.
