Volunteer Application Packet Thank you for your interest in the TechTutor Volunteer Program. This Application Packet contains the Application Checklist, and Request for Criminal History & Disclosure forms. Please complete as instructed. APPLICATION CHECKLIST View the Welcome and Orientation videos 1. KCLS Volunteer Welcome: 1.kcls.org/VolunteerWelcome (10-15 mins.) 2. Tech Tutor Orientation & Training: 1.kcls.org/TechTutorTraining (35-40 mins.) Complete the TechTutor application online 1. Go to 1.kcls.org/TechTutorApply. 2. Click SIGN UP only if you re available for one of the current TechTutor Volunteer opportunities. 3. Create your volunteer profile. Make note of your username and password; if accepted, you will need to log volunteer hours or update your volunteer profile. Complete and submit the KCLS Signature Forms 1. Read, complete and sign Request for Criminal History and Disclosure forms in this packet. 2. Submit forms to the library TechTutor Liaison (you will be contacted to arrange a brief meeting). Complete Volunteer Agreement Form 1. Upon successful completion of other steps, you will receive and complete this form from staff. NOTES! The application process takes 1-2 weeks, depending on applicant response-time and staff schedule.! KCLS volunteers must be able to pass a Washington State Patrol Background Check.! NetMaster KCLS Volunteer Application Volunteer Applicant Disclosure Form Under Washington law, King County Library System (KCLS) is required to obtain a written disclosure from anyone desiring to volunteer in an assignment that will or may involve unsupervised access to children under sixteen years of age, developmentally disabled persons, or vulnerable adults. In conjunction with this law, KCLS conducts a background inquiry with the Washington State Patrol on anyone who applies to volunteer at KCLS. Successful completion of this background inquiry is a condition of volunteering with KCLS. Information obtained through this process may not necessarily prevent volunteering. However, it will be used to help determine suitability and competence to perform the assignment(s) and, on this basis, may result in denial of voluntary services. If you undergo this inquiry, KCLS will provide notification to you within ten days about whether you have sucessfully cleared the background inquiry. You must answer YES or NO to each listed question when you complete this form. If you answer yes to any question below you must provide all of the following information on the attached sheet of paper along with your signature: 1. Question Number; 2. Date of Conviction; 3. Talk Time Citizenship NetMaster KCLS Volunteer Agreement Note: This Agreement is required of all KCLS volunteers, regardless of assignment. I, (please print volunteer s name clearly) hereby agree to volunteer my services to King County Library System (KCLS) at (library or department). I understand that this is uncompensated volunteer service. No offer of regular employment is sought or implied. I agree to volunteer with KCLS in the following capacity (Volunteer Supervisor, please list tasks here): I agree to volunteer according to the following schedule: I understand that either I or KCLS can end my volunteer service at any time. I agree to participate in KCLS volunteer orientation and/or training. Staff will provide orientation to the library, the staff workroom, break areas, and a place for my personal items while scheduled. I agree to wear KCLS volunteer identification (lanyard and badge) while volunteering. I acknowledge that, although serving as a volunteer, the public may regard me as a representative of KCLS, and thus I will dress and conduct myself accordingly. I will be courteous and helpful to patrons and other staff members and I will not engage in the unacceptable behaviors listed at the end of this Agreement. I agree to contact my Volunteer Supervisor or other library staff as soon as possible if I unexpectedly cannot fulfill a scheduled volunteer commitment. Failure to notify staff that I cannot fulfill a scheduled assignment, may lead KLCS to end my volunteer service. I understand that KCLS has comprehensive general liability coverage limited to coverage for volunteers working within the scope of their assignment and on behalf of KCLS. I will report any on-the-job injury or illness, no matter how minor, to my Volunteer Supervisor and/or to a KCLS manager. I will abide by KCLS policy and will not: Reveal any KCLS confidential information, including patron information. Use the patron database (Integrated Library System). Handle Library System money. Perform library work off KCLS property, except as described in virtual volunteer assignments and specific offsite volunteer assignments. Work without direct supervision until I have cleared all required background checks. Note: volunteers under age 14 will have staff supervision. My signature indicates that I have read the above statements and a KCLS staff member has satisfactorily answered my questions about them. Page 1 of 2

Talk Time Citizenship NetMaster KCLS Volunteer Agreement Unacceptable Behaviors Enumerated. The following practices are unacceptable and may lead to ending your volunteer service. a. Destruction, damage, avoidable wastage, theft or unauthorized use of property that is not the property of the volunteer. b. Reporting for my assignment while under the influence of alcohol, narcotics or other controlled substances. c. Possession of alcohol, narcotics, controlled substances or weapons on KCLS premises. d. Failure to observe safety practices, rules, regulations and instructions. e. Failure to promptly report to your assignment supervisor or to another KCLS staff member any injury or accident sustained while fulfilling your volunteer assignment f. Negligence that results in injury to others. g. Misrepresenting yourself as speaking for the King County Library System and making statements or taking other actions that are intended to or could reasonably be expected to damage the integrity or reputation of the Library. h. Sexual or racial harassment or other unlawful discrimination toward another staff member or member of the public. i. Engaging patrons in religious instruction, proselytizing, or worship while working with them during a volunteer assignment. j. Acting in a manner inconsistent with common sense rules of conduct necessary to the welfare of the King County Library System, its staff members and the general public we serve.
