HUMBOLDT SENIOR RESOURCE CENTER Job Description JOB TITLE: DEPARTMENT: REPORTS TO: FLSA STATUS: Home Care - Personal Care Attendant PACE PACE Home Care Coordinator 1.0 FTE Non-Eempt APPROVED: May 2016 JOB SUMMARY: Under the supervision of the Home Care Coordinator, provides home care services to Redwood Coast PACE participants in their homes as authorized in the participant s Plan of Care as determined by the Interdisciplinary Team. ESSENTIAL DUTIES AND RESONSIBILITIES: 1. Assists participants with bathing, grooming, toileting and feeding at home. 2. Provides escort services to and from participant medical and other appointments per the participant s Plan of Care. 3. Routinely monitors vital signs such as weight, temperature, blood pressure and pulse at home. 4. Maintains personal care supplies such as incontinence supplies, nutritional supplements, and personal grooming items. 5. Provides laundry services at home per the participant s Plan of Care. 6. Provides light shopping for necessary items such as toiletries, breakfast and snack items per the participant s Plan of Care. 7. Provides light housekeeping to maintain a clean and sanitary environment in the home per the participant s Plan of Care. 8. Prepares diet appropriate meals for participants who require assistance at home per the participant s Plan of Care. 9. Provides nutritional supplements to participants at home as ordered by the Primary Care Provider and directed by the Dietitian per the participant s Plan of Care. 10. Assists in the monitoring of appropriate diet and food and fluid intake in the home per the participant s Plan of Care 11. Monitors that frozen meals are sent home with participants as specified by the Plan of Care. 12. Assists participants in the home to get ready for transportation to the PACE center in the morning. 13. Implements the Physical and Occupational Therapy maintenance programs in the home per the participant s plan of care. Home Care - Personal Care Attendant Page 1
14. Immediately reports any observed changes in a participant s behavior, health or mobility to the Home Care Coordinator, PACE Nurse and other disciplines as needed. 15. Participates on the Interdisciplinary Team as required and communicates effectively with participants, family members, caregivers and PACE staff. 16. Completes documentation in the medical record accurately and in a timely manner. 17. Complies with safety policies and procedures, identifying and immediately reporting any potential or actual unsafe acts or conditions per the policies and procedures. Takes necessary measures to ensure a safety environment for oneself, co-workers, contractors, participants, visitors and others. 18. Monitors personal safety and attends to good body mechanics. Ensures infection and eposure control and is prepared in case of emergency. 19. Assist in the monitoring of participant physical and chemical restraints per the participants Plan of Care, adheres to the policy and procedures outlining monitoring and communicates with Primary Care Physician, Nurse and/or Home Care Coordinator as required. 20. Consistently meets or eceeds Redwood Coast PACE targets for productivity. 21. Continually seeks better ways for delivering services and communicating with participants. 22. Consistently meets or eceeds Redwood Coast PACE customer services targets. 23. Demonstrates respect for and promotes participant rights, including dignity, selfdetermination, access to care, confidentiality and independence. 24. Understands the importance of community involvement and participates as appropriate in activities that link Redwood Coast PACE to its communities, 25. Effectively collaborates with staff peers and contractors to meet Redwood Coast PACE goals and further success. 26. Complies with all policy and procedures of Redwood Coast PACE. 27. Demonstrates proficiency in delivering age-specific care. 28. Performs other duties as assigned. EDUCATION &/or EXPERIENCE: 1. Degrees and diplomas: High school diploma or GED. 2. Eperience: At least one-year of eperience working with a frail or elderly population. LICENSES OR CERTIFICATES: 1. Certificates, licenses: Verification of completion of CPR (BLS) and first aid training. Valid California driver s license with good driving record. TB screening and successful completion of a health eamination by a PACE designated nurse practitioner or physician is required for employment.
