Welcome to Pembridge We take a holistic, compassionate approach to improving quality of life for our patients and their families. Everyone at Pembridge practices holistic palliative care. This means we consider the physical, emotional, social and spiritual needs of you and the people close to you. Healthcare Assistants Doctors Nurses Spiritual Care Advisor Occupational Therapists Pharmacist Social Workers Complementary Therapists The hospice is part of Central London Community Healthcare, which provides community health services to around two million people across London and Hertfordshire. There are three main teams at Pembridge who all work together... Inpatient Unit 13 beds with a team providing 24-hour care for patients with specialist palliative care needs who require continual assessment. Day Hospice A day centre providing treatment, support and complementary therapies for patients visiting from home. Community Specialist Palliative Care Team Providing advice and support for people in their own homes.

The Community Specialist Palliative Care Team We provide advice and support for people in their own homes. Our team includes clinical nurse specialists (CNS) and doctors in palliative medicine. All members of the team have specialist training and experience in caring for people with life-limiting illness. We focus on quality of life and what it means for each person. Call us: 020 8102 5383

What we do We work closely alongside your GP and district nurses to offer: Advice on pain control and symptom management Emotional and practical support, including referral to other services in your area Support for you, your family and friends in preparing for the end of life Information about bereavement support Who we see We see people at different stages of their illness. From those who have been recently diagnosed to those who are nearing the end of life. Many people with life limiting illness do not need our involvement, as their GP and district nurses are able to provide the care and support that is needed. People can be referred to our service when their needs are more difficult to manage. Referrals can be made by your GP or any other healthcare professional. Our involvement in your care is based on initial assessment and need. Reviews and appointments will be arranged directly with you as appropriate. www.clch.nhs.uk/services/palliative-care

Where we work Depending on individual need, we see people in their own home (including care homes) or in an outpatient clinic at the Hospice. We can visit people at home in the boroughs of Hammersmith & Fulham, Kensington & Chelsea, Westminster and South Brent. As far as possible you will see the same nurse, or a nurse known to you, each time we visit. When we visit, it would be appreciated if you refrain from smoking and keep pets in a separate room Thank you. Call us: 020 8102 5383

Saturday, Sunday and Bank Holidays: 9am - 5pm The clinical nurse specialist on call can be contacted via the hospice inpatient unit on 020 8102 5000. For telephone advice outside these hours, please contact the hospice inpatient unit on 020 8102 5000.
