Please Note: Students are required to bear the expenses for travel and housing related to clinical experiences which may be held in outlying communities in Arizona. ADMISSION REQUIREMENTS & CRITERIA F SELECTION FAST TRACK OPTION 1. Proof of BA or BS degree from an accredited college by program start date 2. A minimum grade point average of 2.75 (on a 4.0 scale) in prerequisite courses. Please note, the minimum GPA of 2.75 does not guarantee admission to the program. Average pre req GPA of admitted students is typically 3.5 and above 3. Grades in biology courses are particularly emphasized. Research indicates that success in science courses is strongly related to passing the licensure exam. 4. Two reference forms from professional individuals (forms are included in this packet). 5. Proctored KAPLAN Admission Exam results (refer to page 8 for instructions) 6. Evidence of Community Volunteer Experience and Evidence of Health Care Volunteer and/or Work Experience. Minimum 8 hours each (Community Volunteer work and Health Care work). 7. Completion of all prerequisite courses with a grade of C or better. NOTIFICATION OF ADMISSION STATUS Applicants will be notified by mail of their admission to the Nursing Program approximately 6-8 weeks following the admission deadline. Applicants who are admitted to the Nursing Program must submit to the Student Services office of the School of Nursing copies of the following prior to enrollment into any NUR prefix course: REQUIRED CLINICAL DOCUMENTS 1. Documentation of Physical Exam within 6 months prior to admission to program. 2. Proof of a current negative TB Skin Test. Follow-up treatment is required if the TB Skin Test is positive and must be initiated prior to enrollment in any clinical courses. A TB Skin Test (or evidence of follow-up treatment) is required every 12 months while students are participating in clinical courses. 3. Proof of initial/completion of required two doses of MMR, Measles (Rubella), Mumps and Rubella vaccine. 4. Proof of initial/completion of Hepatitis B vaccine a series of three inoculations over 6 months. 5. Proof of current Tdap (Tetanus, Diphtheria, Pertussis). 6. Proof of CPR Certification (Health Care Provider) that is current for length of program. 7. Proof of Student Health Insurance. 8. Proof of Student Nurse Liability Insurance (a possible provider for liability insurance is: http://www.nso.com Students may choose other companies to enroll with as well). 9. Proof of Fingerprint Background Clearance Card. NOTE: All of the above items have fees associated with their completion. Students will be required to purchase: 1. NAU Nursing uniform 2. A stethoscope 3. A watch with a second hand 4. NAU name badge displaying first & last name 5. Membership to the National Student Nurses Association And participate in the NAU SNA The Student Services Office will accept only photocopies of all documentation of health related materials. Students are to retain all originals in a safe and accessible place. Students are required to maintain current records of the above listed documents until graduation. Furthermore, the student is responsible for providing copies to Student Services (Bldg 72, Room 206) as they are renewed. Students are responsible for maintaining their own health records. Students will be administratively withdrawn from clinical assignments if all required clinical documents are not up to date and on file at the School of Nursing and current through the academic semester. Students may have to meet additional requirements while working in some clinical site. Some examples are additional system training or drug testing. Some faculty require students to maintain a personal folder of all the above listed documents. When attending clinicals, students must keep copies of their own documents. The breakdown is as follows: Reading 22 questions; 45 minutes Math 28 questions; 45 minutes Writing 21 questions; 45 minutes Science 20 questions, 30 minutes For information on how to schedule the exam see page 8. You may only take the Kaplan exam once per application cycle. For Students in the Flagstaff Area 1. BACKGROUND INFMATION The Arizona State Board of Nursing uses a fingerprint background checks to investigate previous felonies, misdemeanors, and arrests. If a felony is less than five years since absolute discharge, the applicant is ineligible to apply for licensure. Northern Arizona University School of Nursing BS in Nursing Fast Track Option Memorandum of Agreement The Bachelor of Science in Nursing Fast Track Option for second-degree students requires full-time commitment to studies because of its intense nature and Fast Track format. While you are enrolled in this program, it is expected that you will not work more than 12 hours per week and that you will refrain from work commitments at least 8 hours prior to class or clinical assignments. Because this is a statewide program, students will be required to travel to sites throughout the state for clinical experiences and will be responsible for travel/lodging expenses related to these experiences. Clinical experiences will involve several weeks of 3-5 days per week at statewide sites. Also note that starting in Summer 2011 students are required to pay a $1600 program fee. Students will need to plan their family and work commitments accordingly. The program of study is 3 consecutive semesters (including summer).
